A re-evaluation of the role of rotational forceps: retrospective comparison of maternal and perinatal outcomes following different methods of birth for malposition in the second stage of labour.

OBJECTIVE: To compare the outcomes of operative cephalic births by Kielland forceps (KF), rotational ventouse (RV), or primary emergency caesarean section (pEMCS) for malposition in the second stage of labour in modern practise. DESIGN: Retrospective observational study. POPULATION: Data were included from 1291 consecutive full-term, singleton cephalic births between 2 November 2006 and 30 November 2010 with malposition of the fetal head during the second stage of labour leading to an attempt to deliver by KF, RV or pEMCS. METHODS: Maternal and neonatal outcomes of all KF births were compar

Prevention of cerebral palsy during labour: role of foetal lactate.

OBJECTIVES: Intrapartum foetal monitoring goal is to prevent foetal asphyxia and its most severe consequence: cerebral palsy (CP). In this paper we describe the detection methods and the criteria needed to assess asphyxia during labour for preventing CP. Foetal cerebral damage assessment is considered from the medical-legal point of view. CP represents the most frequent pathology of childhood related to pregnancy and childbirth with an incidence of 0.2% in children born alive. It is clinically regarded as the result of a spectrum of diseases due to damage or to faded development of the nervous

Does the type of pushing at delivery influence pelvic floor function at 2 months postpartum? A pragmatic randomized trial-The EOLE study.

INTRODUCTION: Maternal pushing techniques during the second stage of labor may affect women's pelvic floor function. Our main objective was to assess the impact of the type of pushing used at delivery on the mother's medium-term pelvic floor function. MATERIAL AND METHODS: This is a secondary analysis of a randomized clinical trial (clinicaltrials.gov: NCT02474745) that took place in four French hospitals from 2015 through 2017 (n = 250). Women in labor with a singleton fetus in cephalic presentation at term who had undergone standardized training in both of these types of pushing were random

Are racial and ethnic disparities in brachial plexus birth injuries explained by known risk factors?

OBJECTIVE: To investigate the association of maternal race/ethnicity with risk factors for brachial plexus birth injury (BPBI) and quantify the proportion of excess BPBI risk due to these factors. STUDY DESIGN: This retrospective cohort study of all livebirths occurring in California-licensed hospitals from 1996-2012 included 6,278,562 maternal-livebirth infant pairs with 7762 BPBI diagnoses. Multiple logistic regression and causal mediation analyses were used to evaluate the relationship of race/ethnicity and BPBI risk factors. RESULTS: Black and Hispanic birthing-individuals were at increa

Obstetrical and epidemiological factors influence the severity of anal incontinence after obstetric anal sphincter injury.

BACKGROUND: Obstetric anal sphincter injury (OASI) is one of the most severe obstetrical complications. Although risk factors for OASI have been identified, little is known about various parameters that can influence symptoms' severity. The aim of this study is to explore whether obstetrical and epidemiological factors can have an effect on the severity of symptoms after OASI. METHODS: 11.483 deliveries between January 2010 and December 2014 were reviewed, and data from 88 women with OASI are presented. RESULTS: The only statistically significant differences between symptomatic and asymptoma

Severe perineal laceration during operative vaginal delivery: the impact of occiput posterior position.

OBJECTIVE: To identify risk factors for severe (third/fourth degree) perineal laceration with operative vaginal delivery (OVD, forceps or vacuum). STUDY DESIGN: Case-control study comparing singleton OVDs with or without severe laceration (n=138). RESULT: In multivariable analyses, severe perineal laceration was associated with occiput posterior (OP) position at delivery, vaginal nulliparity, use of forceps, longer period pushing in the second stage and lower gestational age, but not birth weight, labor induction or episiotomy. Among 29 OP patients at full dilation, 9/13 (69%) attempted rota

On the biomechanics of vaginal birth and common sequelae.

Approximately 11% of U.S. women undergo surgery for pelvic floor dysfunction, including genital organ prolapse and urinary and fecal incontinence. The major risk factor for developing these conditions is giving vaginal birth. Vaginal birth is a remarkable event about which little is known from a biomechanical perspective. We first review the functional anatomy of the female pelvic floor, the normal loads acting on the pelvic floor in activities of daily living, and the functional capacity of the pelvic floor muscles. Computer models show that the stretch ratio in the pelvic floor muscles can r

Shoulder dystocia: guidelines for clinical practice from the French College of Gynecologists and Obstetricians (CNGOF).

Shoulder dystocia (SD) is defined as a vaginal delivery in cephalic presentation that requires additional obstetric maneuvers to deliver the fetus after the head has delivered and gentle traction has failed. It complicates 0.5-1% of vaginal deliveries. Risks of brachial plexus birth injury (level of evidence [LE]3), clavicle and humeral fracture (LE3), perinatal asphyxia (LE2), hypoxic-ischemic encephalopathy (LE3) and perinatal mortality (LE2) increase with SD. Its main risk factors are previous SD and macrosomia, but both are poorly predictive; 50-70% of SD cases occur in their absence, and

Obstetric Lacerations: Prevention and Repair.

Obstetric lacerations are a common complication of vaginal delivery. Lacerations can lead to chronic pain and urinary and fecal incontinence. Perineal lacerations are defined by the depth of musculature involved, with fourth-degree lacerations disrupting the anal sphincter and the underlying rectal mucosa and first-degree lacerations having no perineal muscle involvement. Late third-trimester perineal massage can reduce lacerations in primiparous women; perineal support and massage and warm compresses during the second stage of labor can reduce anal sphincter injury. Conservative care of minor

Effectiveness of fetal scalp stimulation test in assessing fetal wellbeing during labor, a retrospective cohort study.

BACKGROUND: It is discussed whether fetal scalp stimulation (FSS) test is a reliable complimentary tool to cardiotocography (CTG) to assess fetal wellbeing during labor. The test is based on the assumption that a well-oxygenated fetus, in contrast to the depressed fetus, will respond to a certain stimulus. The aim of this study was to investigate the effectiveness of the FSS-test. METHODS: A retrospective observational study carried out Copenhagen University Hospital, Herlev, Denmark. Laboring women with singleton pregnancies in cephalic presentation after gestation week 33 and indication for

Risk factors associated with deep perineal wound dehiscence after second-degree perineal tears or episiotomy.

INTRODUCTION: Second-degree perineal tears and episiotomies, involving the bulbocavernosus and/or superficial transverse perineal muscles, are common after vaginal birth and may lead to wound dehiscence, infection, and long-term pelvic floor dysfunction. Despite this, risk factors for deep wound dehiscence involving perineal musculature remain insufficiently studied. This study aimed to identify risk factors for wound dehiscence in second-degree tears and episiotomies to inform potential use of antibiotic prophylaxis. MATERIAL AND METHODS: A single-center case-control study (1:2 ratio) was co

Towards lightweight coastal litter monitoring: A synergistic deep learning framework integrating efficient architectures with model compression and knowledge distillation.

The pervasive accumulation of anthropogenic litter along global coastlines poses severe threat to marine ecosystems and socio-economic activities. Traditional manual monitoring is labor-intensive and time-consuming. Deep learning based monitoring has emerged as a promising solution, yet was characterized by high computational complexity and large parameter counts, limiting practical application. We propose OceanTrashNet, an efficient framework reduces computational cost and parameter count while preserving high accuracy, which applies three components targeting the scale variation, elongated s

Diabetic nephropathy in pregnancy: Report of two cases progressing to end-stage renal disease within one year postpartum.

BACKGROUND: Diabetes mellitus is a leading cause of nephropathy and end-stage renal disease. However, diabetic nephropathy during pregnancy in patients with normal glomerular filtration rate and subsequent progression to end-stage renal disease has not been well studied. CASES: This report presents two patients with poorly controlled type 1 diabetes mellitus who had diabetic nephropathy with preserved estimated glomerular filtration rate (Case 1: 117 mL/min/1.73m2; Case 2: 79 mL/min/1.73m2) and shared a similar clinical course, with glomerular filtration rates decreasing by approximately one-

Kidney morcellation in laparoscopic nephrectomy for tumor: recommendations for specimen sampling and pathologic tumor staging.

Laparoscopic nephrectomy is a novel approach for small renal tumors in selected patients; however, removal of the kidney through the small laparoscopic abdominal wall incision site requires the kidney to be morcellated into small fragments while still in situ. Morcellation presents two problems for the pathologist. First, guidelines for optimal sampling of morcellated fragments have not been described. Second, morcellation precludes complete pTNM tumor staging, in particular, tumor size, margins, and renal vein involvement. Based on our initial experience with 23 laparoscopic nephrectomies/nep

Associations Between Physical Activity in Pregnancy and Maternal, Perinatal, and Neonatal Parameters: A Single-Center Prospective Cohort Study.

Background: Physical activity during pregnancy plays an important role in influencing the course of pregnancy, the health of the mother, and neonatal outcomes. Regular exercise can positively affect maternal well-being, reduce the risk of pregnancy-related complications, and support optimal fetal development. Additionally, physical activity may contribute to a reduced need for C-sections and better postpartum recovery. Despite these benefits, global trends indicate a decline in physical activity levels, exacerbated by lifestyle changes such as remote work. This highlights the importance of pro
