The correction of leg-length discrepancy after treatment in developmental dysplasia of the hip by using a percutaneous epiphysiodesis.

The aims of this study were to determine factors that cause a leg-length discrepancy (LLD) to the extent that correction is indicated after treatment in developmental dysplasia of the hip and determine whether the LLD may be treated with a percutaneous epiphysiodesis. Twelve children were included in this study. The factors which caused LLD were avascular necrosis, femoral shortening and varus at reduction, and septic arthritis. Percutaneous epiphysiodesis of the contralateral limb reduced the mean predicted LLD from 3.8 to 1.2 cm at maturity. We concluded that LLD (>2.5 cm) might result from

Treatment of rigid hypertrophic posttraumatic pseudarthrosis of the tibia in children using distraction osteogenesis.

Posttraumatic tibial pseudarthrosis is a relatively uncommon complication of tibia fractures in children. Although reported as a successful method of treating tibial nonunions in adults, distraction osteogenesis through a nonunion site via ring external fixation has not been described in children. The authors report three consecutive cases of distraction through an angulated, shortened, hypertrophic, posttraumatic nonunion to achieve successful union and concurrent correction of deformity. Distraction was carried out using a ring fixator with computer-guided correction. Union was achieved in e

[3D-navigated high energy shockwave therapy and axis correction after failed distraction treatment of congenital tibial pseudarthrosis].

The treatment of congenital tibial pseudarthrosis using a distraction procedure as described by Ilizarov is a standard surgical intervention. Nevertheless, there are problems in achieving bony stability in about 10% of cases even after repeated surgery as reported by Lammens et al. (2000). Traub et al. (1999) found a rate of 50% amputations in 33 cases treated since 1927. To prevent an Ilizarov procedure from resulting in a delayed union or nonunion, Paley et al. (1992) recommended autografting immediately after distraction. Based on the good results in the stimulation of osteogenesis in adult

Update on leg fractures in paediatric patients.

Leg fractures are common and further increasing in prevalence in paediatric patients. The diagnosis is readily made in most cases. Choosing the best treatment is the main issue. Non-operative treatment is the reference standard for non-displaced or reducible and stable fractures but requires considerable expertise and close monitoring, as well as an immobilisation period that far exceeds 3 months in many cases. Some surgical teams therefore offer elastic stable intra-medullary nailing (ESIN) as an alternative to children who do not want to be immobilised for several months. Internal fixation i

Vascular complications in orthopaedic patients treated by orthopaedic microsurgeons.

Over a 20 year period, five patients experienced serious vascular complications involving major arterial insult during various routine orthopaedic procedures. The nature of the vascular damage necessitated immediate surgical repair which was performed successfully by an orthopaedic microsurgeon who was either a member of the initial surgical team or the hospital's Orthopaedic Unit. The purpose of this study is to illustrate not only the severity of these limb and/or life-threatening complications, but also to focus attention on the importance of the orthopaedic surgeon's ability to manage thes

Accuracy of leg length changes in total hip arthroplasty using a computed tomography-based augmented reality navigation system.

INTRODUCTION: Leg length is an important consideration in total hip arthroplasty (THA) as leg length discrepancies (LLD) after THA cause poor outcomes and medical litigation. This study aimed to investigate the accuracy of computed tomography (CT)-based navigation with augmented reality (AR) technology for measuring intra-operative leg length change using anteroposterior radiography (two-dimensional, 2D) and CT (three-dimensional, 3D). MATERIALS AND METHODS: This study included 71 patients (75 hips) who underwent primary cementless THA in the supine position between June 2022 and November 202

Optimization of electronic prescription for parallel external fixator based on genetic algorithm.

PURPOSE: For the parallel external fixator, there are some defects, such as uneven distraction rate, unbearable pain and uncontrollable movement trajectory in practical clinical applications. In order to solve the problems, a new deformity correction algorithm, which is used to calculate the elongation of the six struts, is developed. Meanwhile, the corresponding computer software for getting the electronic prescription is developed. METHODS: First, the trajectory of the moving bone is planned by Cartesian coordinate path control. Next, the prescription is obtained from the trajectory by the

Does fully-threaded Headless Compression Screw provide a length-stable fixation in undisplaced femoral neck fractures?

OBJECTIVE: Femoral neck shortening is a common complication after surgical treatment for intracapsular femoral neck fractures. This study investigated whether fully-threaded Headless Compression Screw (FTHCS) can be a more length-stable implant than partially-threaded cannulated screw (PTCS) in reducing femoral neck shortening. METHODS: A total of 50 patients with undisplaced femoral neck fractures (17 treated by internal fixation with three FTHCS, and 33 treated by three PTCS) from 2011 to 2014 were enrolled in this study. The radiography of the hips and medical records were reviewed for pro

Gait patterns in children with limb length discrepancy.

BACKGROUND: Very few articles describe the compensations in gait caused by limb-length discrepancy (LLD). Song and colleagues explored kinematic and kinetic variables utilizing work equalization as a marker of successful compensation for LLD. They found no difference in strategies based on the location of pathology. The purpose of this study was to define the various gait patterns in patients with LLD and the impact of these compensations on gait kinetics. METHODS: Forty-three children (mean age 12.9±3.7 y) with LLD >2 cm were evaluated in the motion lab using a VICON motion system with 2 AMT

Spinal and extraspinal deformities in a patient with dysspondyloenchondromatosis.

Anisospondyly (irregularly shaped vertebral bodies) and enchondroma-like lesions in the metaphyseal and diaphyseal portions of the long tubular bones are the characteristic features in patients with dysspondyloenchondromatosis (DSC). Thoraco-lumbar scoliosis and windswept deformity of the lower limbs were the major abnormalities encountered in this patient. To the best of our knowledge this is the first case report delineating the spine pathology via reformatted CT scan and the correction of a windswept deformity by means of temporary hemiepiphysiodesis in a patient with (DSC).

Revision for recurrent instability: what are the predictors of failure?

Dislocation is a common complication following total hip arthroplasty (THA). In this study, we evaluated treatment strategies in patients undergoing revision THA for instability. A total of 156 hips in 154 patients underwent revision THA for instability between 2000 and 2007 at our institution. Demographic data and surgical treatment used were analyzed to determine risk factors for failure. Revision treatments included acetabular components in 100 hips, liner exchange in 56 hips, and femoral and acetabular components in 13 hips. Thirty-three (21.2%) had further dislocation. Isolated liner revi

The management of the orthopaedic sequelae of meningococcal septicaemia: patients treated to skeletal maturity.

Septicaemia resulting from meningococcal infection is a devastating illness affecting children. Those who survive can develop late orthopaedic sequelae from growth plate arrests, with resultant complex deformities. Our aim in this study was to review the case histories of a series of patients with late orthopaedic sequelae, all treated by the senior author (CFB). We also describe a treatment strategy to address the multiple deformities that may occur in these patients. Between 1997 and 2009, ten patients (seven girls and three boys) were treated for late orthopaedic sequelae following meningoc
