Managing Respiratory Failure in Obstructive Lung Disease.

Exacerbations of obstructive lung disease are common causes of acute respiratory failure. Short-acting bronchodilators and systemic glucocorticoids are the foundation of pharmacologic management. For patients requiring ventilator support, use of noninvasive ventilation reduces the risk of mortality and progression to invasive mechanical ventilation. Challenges associated with invasive ventilation include ventilator dyssynchrony, air trapping, and dynamic hyperinflation. Careful monitoring and adjustment of ventilatory support parameters helps to optimize the patient-ventilator interaction and

The management of acute respiratory failure.

The mortality rate of severe respiratory failure in most centers remains 60% to 90% with conventional pulmonary support techniques. Recent advances in the physiologic management of patients with acute respiratory failure include optimizing systemic oxygen delivery as reflected by continuous mixed-venous saturation monitoring, avoidance of the damaging effects of high inflation pressures and volumes during mechanical ventilation, and the increasing application of extracorporeal life support techniques for refractory respiratory failure. The future promises the routine clinical application of no

Effect of ZnO nanoparticles on biofilm formation and gene expression of the toxin-antitoxin system in clinical isolates of Pseudomonas aeruginosa.

BACKGROUND: Biofilm formation by Pseudomonas aeruginosa (P. aeruginosa) is known to be characteristic of this organism. This bacterium is considered one of the most life-threatening bacteria and has been identified as a priority pathogen for research by WHO. Biofilm-producing P. aeruginosa is a concern in many parts of the world due to antibiotic resistance. Alginate also plays an important role in the biofilm formation of P. aeruginosa as well as the emergence of antibiotic resistance in biofilms. In addition, the systems of toxin-antitoxin( TA) play an important role in biofilm formation. Me

Use of Health and Welfare Technology in Palliative Care: State-of-the-Art Review.

BACKGROUND: As more individuals live longer with complex conditions, the need for effective palliative care (PC) grows. It has been stated that access to PC should be integrated early and delivered in a timely manner to patients with life-threatening illnesses. Health and welfare technologies (HWTs) offer tools to enhance care delivery, particularly in home and rural settings. Although there is a profound lack of evidence regarding the impact when used in PC, it is necessary to critically assess the current state of knowledge regarding impacts and consequences of technologies, ensuring that th

Indwelling peritoneal and pleural catheters in the management of malignant and non-malignant ascites and pleural effusion: experiences from a Danish tertiary center.

Recurrent pleural effusions and ascites significantly impair quality of life, particularly in patients with advanced malignant and non-malignant disease. Traditional management often relies on repeated hospital-based procedures, which provide temporary symptom relief but place a considerable burden on patients and healthcare systems. This retrospective cohort study evaluates the safety, effectiveness, and clinical outcomes of indwelling pleural and peritoneal catheters (IPCs and IPeCs) in 63 patients treated at Lillebaelt Hospital Vejle between October 2019 and October 2024. A total of 30 pati

Patients' and caregivers' perception of multidimensional and palliative care in amyotrophic lateral sclerosis - protocol of a German multicentre study.

INTRODUCTION: Amyotrophic lateral sclerosis (ALS) is an inevitably fatal condition that leads to a progressive loss of physical functioning, which results in a high psychosocial burden and organizational challenges related to medical care. Multidimensional and multiprofessional care is advised to meet the complex needs of patients and their families. Many healthcare systems, including Germany, may not be able to meet these needs because non-medical services such as psychological support or social counselling are not regularly included in the care of patients with ALS (pwALS). Specialised neuro

A Shared Decision-making Tool for Drug Interactions Between Warfarin and Nonsteroidal Anti-inflammatory Drugs: Design and Usability Study.

BACKGROUND: Exposure to life-threatening drug-drug interactions (DDIs) occurs despite the widespread use of clinical decision support. The DDI between warfarin and nonsteroidal anti-inflammatory drugs is common and potentially life-threatening. Patients can play a substantial role in preventing harm from DDIs; however, the current model for DDI decision-making is clinician centric. OBJECTIVE: This study aims to design and study the usability of DDInteract, a tool to support shared decision-making (SDM) between a patient and provider for the DDI between warfarin and nonsteroidal anti-inflammat

Lived Experiences of Kidney Transplant Patients at Selected Referral Hospital in Rwanda: A Phenomenological Study.

INTRODUCTION: Kidney transplantation is the preferred treatment for end-stage renal disease, offering better survival and quality of life than dialysis. However, patients must manage lifelong medication, follow-up, and lifestyle adjustments. While post-transplant experiences are well documented in high-income countries, little is known about kidney transplant patients in Rwanda. Understanding the experiences of patients who received transplants abroad is important. This study explored the lived experiences of kidney transplant patients attending follow-up care at a referral hospital in Rwanda.

Understanding mental health and help-seeking among adolescents in Europe: a focus group-based participatory study to inform preventive interventions.

This multi-centre, qualitative study aimed to identify dimensions of mental health and possible determining factors of help-seeking among young people in Europe. Based on adolescent experiences, key risk factors for their mental health should be detected to inform preventive interventions. Focus group sessions were conducted in Cyprus, Germany, Italy, Poland, Slovenia, Spain, and the UK with adolescents aged 10 to 24 years, and their stakeholders (parents, school professionals and clinicians). The discussions followed a broad rationale asking about stressors, coping strategies, and barriers an

Understanding public perspectives of prehospital trauma research: A qualitative community engagement study on consent and blood transfusion.

BACKGROUND: Unintentional injuries are a major cause of death worldwide. Modern trauma systems have reduced morbidity and mortality through rapid prehospital care, yet trauma research faces challenges obtaining informed consent during emergencies. Because patients are often incapacitated and substitute decision makers are unavailable, studies commonly rely on deferred or waived consent. Despite their use, little is known about public perspectives on these models or on prehospital interventions such as paramedic-administered blood transfusion. OBJECTIVE: To explore Canadian public perspectives

Turning pain into purpose: a qualitative investigation into the psycho-social impacts associated with ectopic pregnancy loss among women in the United Kingdom.

Despite the profound effects ectopic pregnancy has been demonstrated to have on women's mental health, this type of pregnancy loss has received little attention compared to other forms. Ectopic pregnancies are the leading cause of maternal death in early pregnancy and often require urgent medical intervention, which overshadows the psycho-social impacts associated with pregnancy loss. We aimed to undertake a detailed investigation into women's experiences and associated psychosocial impacts of ectopic pregnancy. Using a qualitative research design, semi-structured interviews (N = 20) were cond

Neurobiological insights into central auditory processing dysfunction following early-life otitis media.

Otitis media with effusion (OME) is one of the most common pediatric disorders and a leading cause of transient conductive hearing loss during critical periods of brain development. Beyond peripheral effects, growing evidence indicates that OME may influence central auditory processing through a combination of molecular, cellular, and systems-level mechanisms. Pro-inflammatory cytokines, including interleukin-1β and tumor necrosis factor-α, together with reduced auditory input, can activate microglia and astrocytes, limit brain-derived neurotrophic factor (BDNF) signaling, and disrupt MAPK pat
