Social Determinants of Health in Patients Undergoing Osteocutaneous Free Flap Reconstruction for Malignant Disease of the Mandible.

BACKGROUND: Social determinants of health (SDH), such as race, language, and insurance status, may impact access to surgical care and postoperative outcomes. This study investigates the role of SDH in patients undergoing osteocutaneous free flap reconstruction for mandibular defects due to oral cavity malignancy. METHODS: A retrospective cohort study of 131 patients at a single tertiary academic center was conducted. Demographic variables and clinical outcomes were extracted from records. The CDC Social Vulnerability Index (SVI) assessed county-level SDH. Outcomes included readmission, reoper

Priority of fibular reconstruction in patients with oral cavity cancer undergoing segmental mandibulectomy.

BACKGROUND: The fibula osteoseptocutaneous free flap is generally used for segmental mandibular reconstructions following resection of oral cavity squamous cell carcinoma (OSCC). However, less complex reconstructions may be feasible for patients with predicted poor survival. Herein, we sought to identify the main risk factors (RFs) associated with poor prognosis in OSCC patients undergoing segmental mandibulectomy to help decide between fibular and non-fibular reconstructions. METHODS: Between 1996 and 2011, we examined the 5-year control, distant metastases, and survival rates in 310 consecu

Osteochondroma of the temporomandibular joint: report of 2 cases emphasizing the importance of personalizing the surgical treatment.

BACKGROUND: Osteochondromas (OCs) are rare in the craneofacial area (0.6%). We present 2 cases of OC of the mandibular condyle, emphasizing the surgical decision of each case. CASE 1: In a 48-year-old woman with facial asymmetry, left cross-bite, and mandible deviation to the left, a computerized tomographic (CT) scan confirmed the presence of a bony expansion of the right condyle. The clinical diagnosis was osteochondroma. The patient underwent condylectomy with costochondral reconstruction. CASE 2: In a 76-year-old woman with a 1-month history of right preauricular pain, CT showed a deformed

Uncommon clinical manifestations in a case of thyroid carcinoma.

A case of multiple, functional bone metastases from a previously operated follicular thyroid carcinoma is reported in an apparently euthyroid patient. Additional uncommon clinical manifestations encountered in this case were: 1) mandibular metastasis mimicking an intraoral arteriovenous malformation, 2) extensive, destructive, and expansive intraspinal metastases in the thoracic vertebral column protruding into the posterior mediastinum and causing severe spinal lesions, and 3) the precipitation of a fatal thyroid crisis following radioactive iodine treatment.

Oral metastasis of the mediastinal germ cell tumor (yolk sac).

A yolk sac tumor is a rare malignant tumor of germ cell origin. It most commonly arises from the testes and ovaries in young adults, but extragonadal sites of origin are reported in 10% to 15% of the cases. Yolk sac tumors are malignant, tend to recur locally, and may present with widespread metastases at the time of diagnosis. Involvement of the head and neck is uncommon. In this study, we present the case of a 23-year-old man presenting with mandibular and adjacent gingival metastasis of a mediasatinal yolk sac tumor. Thus, the patient has already undergone chemotherapy; no additional treatm

Tracheostomal recurrence in oral squamous cell carcinoma: a case report and literature review.

Tracheostomal recurrence is a rare but serious complication following temporary tracheostomy in oral cancer patients, with an estimated incidence of ∼1%. It is typically associated with poor prognosis, as most patients succumb to disease within months of recurrence. This report concerns a 61-year-old male with advanced squamous cell carcinoma of the right retromolar trigone (pT4bN3b, American Joint Committee on Cancer, 8th edn). Following segmental mandibulectomy, modified radical neck dissection, and adjuvant chemoradiotherapy, the patient developed a squamous cell carcinoma recurrence at the

Transdifferentiation of Hairy Cell Leukemia to Lytic and Mass-Producing Histiocytic Sarcoma: A Novel Head and Neck Example and Review of the Literature.

BACKGROUND: Transdifferentiation of hairy cell leukemia to histiocytic sarcoma (HS) has not been previously reported in the head and neck. Given the rarity of HS, it can pose a challenge to diagnose this aggressive malignancy. CASE PRESENTATION: A 93-year-old male with a complex medical history presented with symptomatic and mobile lower anterior teeth. No intraoral growth mass was present initially and radiographic examination showed a radiolucent lesion in the anterior mandible. An incisional biopsy under local anesthesia was performed and histopathologic examination revealed a malignant sa

[A Case of Lower Gingival Carcinoma Treated Surgically Using Computer-Assisted Patient-Specific Cutting Guide and Mandibular Reconstruction Plate].

We report a case of advanced lower gingival carcinoma treated with a computer-assisted custom-made surgical guide and reconstruction plate system(TruMatch®)in combination with a pectoralis major myocutaneous flap. In a 74-year-old male patient, a neoplastic lesion with a 30×30 mm periapical induration was observed around an extraction socket of left mandibular third molar. After various examinations, we diagnosed lower gingival carcinoma(cT4aN2bM0, Stage ⅣA). The patient was a paraplegic with a spinal cord injury, and reconstruction of mandible with free flap was not indicated, requiring minim

[A Case of Recurrent Squamous Cell Carcinoma of the Mandibular Gingiva Around a Mandibular Dental Implant].

Extensive bimaxillary bone support devices(hereafter referred to as a dental implant)are useful for morphological and functional reconstruction after resection of oral cancer. However, tumor recurrence due to peri-implantitis may occur. In this report, we describe a case of squamous cell carcinoma(SqCC)recurrence around a mandibular dental implant. An 80-year- old female patient underwent tumor resection and stratification for SqCC extending from the left lingual margin to the gingiva of the left lower molar in 2013. In 2014, 2 dental implants were placed in the bilateral mandibular canine reg

Marsupialisation for the treatment of unicystic ameloblastoma of the mandible: a long-term follow up of 116 cases.

Unicystic ameloblastoma is a unique histopathological type of ameloblastoma, and treatment is controversial. Marsupialisation is effective in reducing the size of cystic lesions and their complications. We have retrospectively analysed the clinical, histopathological, and prognostic data of affected patients who were treated by marsupialisation between 2003 and 2013 in three Chinese hospitals. Our aim was to evaluate the effects and prognosis, and the factors associated with outcome. A total of 116 patients with mandibular unicystic ameloblastomas were included, and 74, 26, and 16 patients wer

The importance of early diagnosis of gardner's syndrome in dental examination.

Gardner syndrome (GS) is a rare genetic disorder. Dentists play an important role in diagnosis considering that craniomaxillofacial osteomas are a major criteria for GS. We report a 26-year-old male patient who referred to our department with toothache. On routine panoramic radiographic examination, multiple radiopaque masses were detected incidentally. In addition, on extraoral examination, a soft tissue tumor was detected on his shoulder. The patient was referred to the gastroenterology department and intestinal polyps were detected in the colon. Histopathology report revealed malignant chan

Does invasion of oral squamous cell carcinoma occur into a non-pathological fracture of the mandible?

Although the route of invasion of oral squamous cell carcinoma into the intact mandible is well-documented we know of no reports about whether malignant cells invade through non-pathological fractures. A 68-year-old patient presented with a traumatic fracture of the mandible at the site of an existing carcinoma. He had asegmental resection but histological examination showed no evidence of invasion into the bone.

Mandibular metastases from an ileum stromal tumor.

INTRODUCTION: Metastatic disease of the jaws is unusual and accounts for 1 to 4% of oral cavity malignancies. Jaw metastases from the gastrointestinal (GI) tract usually evolve from adenocarcinoma of the esophagus, colon, and rectum. Gastrointestinal stromal tumors (GIST) are the most common mesenchymal tumors of the GI tract defined by a positive C-Kit (CD117). These tumors are thought to arise from Cajal cells in GI tract walls, essential for intestine motor function. The small intestine harbors only 30% of GIST. After reviewing the literature, no case of jaw metastases from GIST was found.

[Kahler's disease. Multiple myeloma].

Kahler's disease, multiple myeloma, is a malignant condition of unbridled multiplication of plasma cells in bone marrow. Clinical features are anaemia, pain in the affected bones, spontaneous bone fractures and increased infection susceptibility. In the final stage of the disease severe renal failure can occur. With the present chemotherapy a good response is seen in 50-70% of patients, but complete response occurs only in a minority of patients. Radiotherapy is often used in addition to chemotherapy. Bisphosphonates are used to inhibit osteolysis and to prevent complications associated with o

Salivary duct carcinoma in the mandible: a case report.

Salivary duct carcinoma (SDC) is a distinctive and aggressive neoplasm. The most frequent site of origin is the parotid gland, followed by the submandibular gland. SDC originating in the minor salivary glands, particularly in the ectopic glands within the mandible, is extremely rare. We describe a 62-year-old man with SDC in the mandible, who presented with a painless lump in the right submandibular region (later identified as lymph node metastasis) and ipsilateral mental nerve palsy. Histologic examination after ablative surgery revealed SDC originating in the mandible and cervical nodal meta

Combined anterior thigh flaps and vascularised fibular graft for reconstruction of massive composite oromandibular defects.

Six massive, composite oromandibular defects were reconstructed using combined anterior (anterolateral and anteromedial) thigh flaps and vascularised fibular graft in bridge or chimeric fashion. Except for minor dehiscence in one case and infection in another, all flaps survived without complication. Anterior thigh flaps provide large-calibre, long vascular pedicles while derivative branches from the lateral circumflex femoral system facilitate simultaneous transplantation of multiple components. Because the pedicle of the vascularised fibular graft is of insufficient length, the lateral circu

Fine needle aspiration cytology in the diagnosis of non-neoplastic diseases of head and neck masses.

In this study, 3 patients with head and neck masses, which were suspicious of neoplastic compromise, were submitted to FNAC investigation. Two cases showed specific infection by Paracoccidioidomycosis and one by Actinomycosis. The cytologic findings were in agreement with histopathologic diagnosis and clinical follow-up; that results showes that FNAC importance in the diagnosis of non-neoplastic masses could be explored in ambulatorial routine.
