Functional Temporomandibular Joint Reconstruction in Costochondral Grafting of Micrognathia.

Rib bone and costochondral complex grafting has been used to treat micrognathia classified as Pruzansky type III. To acquire more physiological joint movement, we reconstructed a temporomandibular joint with the glenoid fossa in addition to the mandibular ramus. The patient underwent a tracheostomy to correct her airway obstruction at 2 months of age. After that, no further surgical treatments were performed on the micrognathia. When she was 6 years of age and during consultation at our department, micrognathia caused by Goldenhar syndrome was confirmed. A head and neck computed tomography sca

Progressive Overgrowth of Costochondral Graft in a Case of Hemifacial Microsomia: A Rare and Severe Case.

Costochondral grafts (CCGs) are widely used for mandibular condyle reconstruction in children due to their growth potential, yet their unpredictable behavior remains a challenge. We present a rare case of progressive CCG overgrowth in a 17-year-old male with hemifacial microsomia, resulting in severe facial asymmetry despite prior corrective surgeries. Clinical assessment revealed a 2.5 cm ramus discrepancy and marked mandibular deviation. Management required staged surgical correction, including graft excision, osteotomies, and genioplasty. This case illustrates the potential for extreme CCG

Treacher Collins Mandibular Distraction.

Treacher Collins syndrome (TCS) is a genetic disorder that presents with a variety of craniofacial deformities. One classic feature of TCS is a steep, counterclockwise rotation of the occlusal plane, and microretrognathia with bony deficiencies in both the body and ramus of the mandible. This morphology commonly necessitates reconstruction by the craniofacial surgeon. This article discusses strategies and considerations for surgically correcting the mandibular deformity associated with TCS using mandibular distraction osteogenesis and other related techniques. The proper implementation of thes

Ameloblastic Carcinoma In Situ: Review of Literature and a Case Presentation in a Pediatric Patient.

Ameloblastic carcinomas are rare malignant lesions with 3 mandibular pediatric cases reported in the literature. We present a case of ameloblastic carcinoma in situ in a 15-year-old male with a right mandibular cystic lesion on computed tomography. The incisional biopsy revealed plexiform ameloblastoma. Due to the infiltrating and aggressive nature of the tumor, the patient underwent hemimandibulectomy and immediate reconstruction with a vascularized osteocutaneous fibula free flap. The final pathology was read as ameloblastic carcinoma in situ. Given the rarity of this disease in the pediatri

Treatment of traumatic dislocation of the mandibular condyle into the cranial fossa: development of a probable treatment algorithm.

This study summarizes our experience of treating three rare cases of traumatic superior dislocation of the mandibular condyle into the cranial fossa and provides a potential treatment algorithm. Between the years 2002 and 2012, three patients with traumatic superior dislocation of the mandibular condyle into the cranial fossa were admitted to our department. After evaluating the interval from injury to treatment, the associated facial injuries including neurological complications, and the computed tomography imaging findings, an individualized treatment plan was developed for each patient. Multidetector row computed tomography with multiplanar and 3D images in the evaluation of posttreatment mandibular fractures.

Multidetector row computed tomography with multiplanar (MPR) and 3-dimensional (3D) computed tomographic reconstructions is the method of choice in condylar fractures and in the presence of complications for all types of mandibular fracture. MPR and 3D images are the best diagnostic tools to evaluate mandibular fractures after surgical treatment, both after surgery and during follow-up. The conventional radiography is imprecise in the condylar region due to the complicated anatomical bone structures in the area, the lack of sharpness, and image distortion. Computed tomographic imaging enables

Restoration of the atrophied posterior mandible with transverse alveolar maxillary/mandibular implants: technical note and case report.

Restoration of the atrophied posterior mandibular alveolus has been a surgical challenge in the past. Many treatment options have been published, each with unique shortcomings. This study will review and compare these techniques to a new type of implant, the transverse alveolar maxillary/mandibular implant (TAMMI). Using modified Nobel Biocare Brånemark System zygomatic implants that were shortened to 11.5, 13, 15, 18, 20, 22.5, or 25 mm, the authors reconstructed atrophied posterior mandibles. These TAMMIs were placed at a 45-degree angle, engaging both the crest of the ridge and the buccal c

Perception of professionals in the assessment of coronoid hyperplasia by computed tomography.

Coronoid Hyperplasia (CH) is a non-neoplastic and relatively rare enlargement of the coronoid process that may limit mandibular movement as a consequence of the close association between the hyperplastic coronoid process and the anterior region of the zygomatic bone. Computed tomography (CT) is extremely useful for the observation of this association and plays an important role in diagnosing and planning surgical treatment. Once the CT scan is performed, the data can be viewed in many different arrangements, including multiplanar (MPR) and 3D rendering, although the resolution of the latter ma

Orthodontic and surgical treatment of a patient with hemifacial microsomia.

This article describes the surgical and orthodontic treatment of a 12-year-old boy with a significant deformity and functional involvement caused by hemifacial microsomia. The left mandibular ramus and condyle were hypoplastic and abnormal in form and location. The lower third of the face was increased, with mandibular retrusion and significant facial asymmetry. He had difficulties in speaking and chewing and problems related to his facial appearance, which caused severe psychosocial disturbances. The patient received orthodontic treatment and temporomandibular joint reconstruction with a cost

Secondary reconstruction of the mandible with an aluminum oxide prosthesis.

A case is presented where secondary mandibular reconstruction with an aluminum oxide ceramic prosthesis was performed for a patient who had had an earlier hemimandibulectomy and primary bone graft for management of an ameloblastoma. Various factors that were considered in the construction of the prosthesis and problems encountered during operation are discussed. Although the prosthesis was found to be quite useful for the restoration of facial contour, the results of a long-term follow-up are needed to obtain a final evaluation of the reconstruction.

Temporomandibular joint reconstruction after condylar fracture complication related to osteosynthesis material.

Total condylar resorption and the failure of bone synthesis material as postoperative complications of condyle fracture are seldom reported in the literature. When these occur, they may severely limit temporomandibular joint (TMJ) function. In such cases, a reconstruction of the joint is indicated, which can be achieved by means of an alloplastic prosthesis. This article reports a rare case of impaction of osteosynthesis material in the region of the base of the skull associated with a mandibular condyle fracture treated with rigid internal fixation, which resulted in complications. There was

[Microsurgical reanastomosis of scapula transplants for maxillofacial bone reconstruction].

Between 1989 and April 1993 29 grafts from the scapula region have been harvested for reconstruction in the head and neck area. 7 out of 20 osteocutaneous grafts have been used for reconstructions in the maxillary/midface area. In this area, one graft was completely lost due to venous congestion and another loss of the soft tissue component of one graft occurred. The complication rate in the maxilla seems to be higher than in application of the flap for mandibular reconstruction. Reconstruction in the maxillary/midface area most often requires several surgical steps. In special situations, as …

Double osseous flaps for simultaneous midfacial and mandible reconstruction: Automation in surgical complexity within an entirely computerized workflow.

INTRODUCTION: Broad maxillofacial surgical resections involving both the midface and the mandible represent a challenge in terms of reconstruction. Although several papers have explored the possibility of simultaneously using two microsurgical flaps, reports on the implementation of a dual osseous flap strategy are limited, and mainly addressed to static anatomical reconstruction, regardless of functional implications. In particular, there is a lack in the literature of a unifying protocol which illustrates how technology including virtual planning, statistical shape modeling, virtual occlusio

Reconstruction of mandible with fibula free flap.

The fibula can be used as a donor for a free flap for mandible reconstruction. It has the advantages of low donor site morbidity, consistent shape, ample length, and distant location to enable a two-team approach, allowing multiple osteotomies because of its periosteal circulation. It can be raised with a skin island for composite tissue reconstruction. Eight segmental mandibular defects (average 11.62 cm) were reconstructed following resection for tumour. Six defects consisted of bone alone and the other two had only a small amount of associated intraoral soft-tissue loss. Two patients underw

Reconstruction of the Severe Cervical Scar Contracture Using a Combination of the Pre-expanded Bipedicled Forehead Flap and Lower Trapezius Musculocutaneous Flap.

Reconstructing severe cervical scar contractures (SCSC) remains a considerable challenge. This study presents a novel approach to SCSC reconstruction using a combination of pre-expanded bipedicled forehead and lower trapezius musculocutaneous flaps. A retrospective analysis was conducted on 25 patients who underwent this procedure between April 2004 and July 2020. Sixteen patients received a forehead flap combined with a unilateral lower trapezius musculocutaneous flap; in contrast, 9 patients underwent reconstruction using bilateral lower trapezius musculocutaneous flaps. The surgery was perf
