Trigeminal nerve balloon compression (TNBC)1-3 can provide immediate therapeutic relief to patients suffering from trigeminal neuralgia. This is a particularly effective treatment option for patients who are not eligible for surgical procedures (i.e., elderly patients or patients with multiple comorbidities) or for patients who have had an insufficient response to microvascular decompression. TNBC can also be used as a bridge treatment before stereotactic radiosurgery. Use of intraoperative computed tomography-like images using a C-arm system (DYNA-CT) imaging facilitates the TNBC procedure.4,

The canalis sinuosus is an anatomical variation whereby the infraorbital canal sometimes generates a small, lateral branch (canal) close to its midpoint, to allow the passage of the anterior superior alveolar neurovascular bundle in the anterior maxilla. This article focuses on an incidental finding of this variant, in a 74-year-old Trinidadian female of Afro-Caribbean descent with an endodontic presenting complaint. The canalis sinuosus shadow on conventional radiography resulted in uncertainty as to the offending tooth until a 3-dimensional scan was undertaken in this region. This report wil

BACKGROUND: Sequelae related to implant placement/advanced bone grafting procedures are a result of injury to surrounding anatomic structures. Damage may not necessarily lead to implant failure; however, it is the most common cause of legal action against the practitioner. This study aimed to describe morphological aspects and variations of the anatomy directly related to implant treatment.
METHODS: Morphometric analyses were performed in 22 Caucasian skulls. Measurements of the mental foramen (MF) included height (MF-H), width (MF-W), and location in relation to other known anatomical landma

OBJECTIVE: The aim of this study was to describe the standard diagnostic procedure and the application of the Caldwell-Luc approach for the retrieval of a displaced root from the maxillary sinus and to share our experience in treating this complication.
STUDY DESIGN: Twenty-four patients with a fractured root accidentally displaced into the maxillary sinus were referred by general dentists to our department from 2005 to 2008. All were managed by a standardized diagnostic procedure and a Caldwell-Luc approach. We recorded the age of each patient, the gender, the tooth, the size of root fragmen

PURPOSE: Trigeminal schwannomas extending into the deep parapharyngeal space are relatively rare, and a surgical approach to such tumors has not been well established. We report four cases in which the tumors were completely removed by using a combination of three approaches.
METHODS: Four patients with trigeminal schwannomas extending into the extracranial space were surgically treated using a combination of the transcervical approach, anterolateral retromaxillar pathway via gingivobuccal sulcus, and epi and interdural middle fossa approach.
RESULTS: The maximum diameters of the tumors rang

UNLABELLED: Case description: A case of a 37-year-old female patient suffering from refractory bilateral trigeminal neuralgia is presented, who underwent various interventions such as acupuncture, block therapies and even microvascular decompression without effective pain relief.
CLINICAL FINDINGS: Paresthesias and shooting-like twinges of pain intensity 10/10 in bilateral maxillary and mandibular branches of the trigeminal nerve, with nasal and intraoral triggers that made eating impossible, becoming increasingly severe since refractoriness to microvascular decompression and carbamazepines,

Rhynocoris marginatus Fabricius (Hemiptera: Reduviidae) is an important biological control agent against more than 25 insect pests in India. For a better understanding of the feeding adaptation of this bug, the gross morphology and histology of its head and salivary apparatus were studied using both a light microscope and scanning electron microscope. The head is more or less elongate, mobile, and immersed into the eyes. R. marginatus has a three-segmented curved rostrum; the middle segment is longer than the other two segments. The terminal rostral segment bears spines and trichobothria exter

Myeloid sarcoma (MS) is a localized extramedullary mass of immature granulocytic cells that usually occurs in patients with acute myeloid leukemia (AML) or myeloproliferative disorders. It may rarely precede peripheral blood or bone marrow involvement, presenting a diagnostic challenge. Although MS may be found in any location, an intraoral occurrence is rare. In this report we describe a rare case of a patient with nonleukemic MS of the maxillary gingiva. The histologic specimen was first interpreted as non-Hodgkin's lymphoma. The correct diagnosis was reached after extensive immunohistologic

In contrast to the number of reports in the literature describing high velocity injuries of the maxillo-facial region, there are relatively few which deal with civilian type gunshot injuries. The purpose of this study was to undertake a retrospective analysis of 40 cases of low-velocity hand-gun injuries of the maxillo-facial region. The majority of cases were due to assault followed by accidental shootings and failed suicide attempts. Injuries ranged from mild soft tissue damage without fractures to severe, comminuted fractures of the facial bones. In some cases, soft tissue damage was severe

A 73-year-old male with a history of incidentally diagnosed Paget disease of bone affecting the skull and left orbit 2 years prior presented with 3 months of vision loss, proptosis, and periorbital swelling of the OS. Examination showed best-corrected Snellen visual acuity of 20/150 in the affected eye, intact motility, 7 mm of relative proptosis, significant dilated and tortuous "corkscrew" conjunctival vessels, serous choroidal and retinal detachments, optic nerve hyperemia, and venous tortuosity and dilation. Although the bony lesions in the left orbit were stable from 1 year prior on imagi

We report the results of an experiment in which electrophysiological activity was recorded from the human cerebellum and cerebrum in a sample of 14 healthy subjects before, during and after a classical eye blink conditioning procedure with an auditory tone as conditional stimulus and a maxillary nerve unconditional stimulus. The primary aim was to show changes in the cerebellum and cerebrum correlated with behavioral ocular responses. Electrodes recorded EMG and EOG at peri-ocular sites, EEG from over the frontal eye-fields and the electrocerebellogram (ECeG) from over the posterior fossa. Of …

Cavernous sinus syndrome is a combination of unilateral ophthalmoplegia, autonomic dysfunction and sensory loss of the ophthalmic or maxillary branch of the trigeminal nerve. In the case of cavernous sinus syndrome after a head injury, we should consider the possibility of a dissecting aneurysm. In addition to ruptured aneurysms, silent intracranial aneurysms must also be treated if they cause compression symptoms. In our case report, we present a 23-year-old female patient who lost consciousness during a fall, and immediately developed right-sided ptosis, ophthalmoplegia and loss of facial se

BACKGROUND: In an effort to avoid the damage and inconvenience associated with transcranial approaches, we developed an endoscopic transmaxillary transMüller's muscle approach for decompression of the superior orbital fissure (SOF).
METHODS: The endoscopic transmaxillary transMüller's muscle route was performed in ten cadaveric heads. We measured important anatomic landmarks, and angles radiographically. This approach was initially attempted in one patient with traumatic superior orbital fissure syndrome (tSOFS).
RESULTS: A maxillary antrostomy was carried out with a buccal sulcus incision.

The combination of Gillies elevation with 1-point percutaneous Kirschner wire fixation of isolated simple zygoma fractures was found to be effective in restoring preinjury appearance and function and avoiding soft tissue morbidity. The proximity of the infraorbital nerve, inferior orbital rim, and dental roots warrants care in the placement of the wire. The need for precise anatomic guidelines becomes apparent when considering these relationships. Eighteen adult skulls (36 sides) were examined, and specific points were determined that could be important while inserting Kirschner wire for zygom

AIM: To provide core information on cone beam computed tomography (CBCT) technology and its potential applications in endodontic practice.
SUMMARY: CBCT has been specifically designed to produce undistorted three-dimensional information of the maxillofacial skeleton as well as three-dimensional images of the teeth and their surrounding tissues. This is usually achieved with a substantially lower effective dose compared with conventional medical computed tomography (CT). Periapical disease may be detected sooner using CBCT compared with periapical views, and the true size, extent, nature and p

A case of nasal NK/T cell lymphoma with central nervous system (CNS) involvement is reported. A 56-year-old man presented with eyelid edema and transverse myelopathy. Cerebrospinal fluid examination revealed atypical lymphoid cells with azurophilic granules, which were positive for CD2, CD8, and CD56, and negative for CD3 and CD5 by flow cytometry. Because a tumor mass was found involving the ethmoid and maxillary sinuses, CNS involvement was considered to have resulted from local invasion by the nasal lymphoma. In spite of intensive chemotherapy including intrathecal infusion, the patient die

The nasopharynx, upper part of the parapharyngeal space, and skull base are relatively inaccessible to the surgeon without major postoperative complications. Operative fields reached by conventional approaches through the palate and maxillary sinus are too limited and narrow to remove extensive tumors. The authors applied a transmandibular transpterygoid approach for the removal of five residual nasopharyngeal carcinomas (NPCs) after full doses of irradiation, one pleomorphic adenoma of the nasopharynx, and one large parapharyngeal schwannoma extending into the jugular foramen. This approach o

Dental pains provoked by an altitude beyond 5.000 ft can occur with any kind of tooth, healthy or restored. It is considered as a consequence of a brutal pressure change. The incidence has not been decreasing in spite of flight conditions improvement. These kind of dental pain cause a problem for those who fly: civilian and military pilots as well as airlines passengers. The conditions in which this might happened are variable. There may be a definite correlation between the character of the symptoms of aerodontalgia and the underlying pulpal pathology. The proposed etiology concerning patholo

Facial wounds are frequently associated with bone fractures and multiple trauma; however, disruption of the parotid gland or Stensen's duct is rare and is often associated with facial nerve palsy. Lesions are often undiagnosed on first assessment of the multiple trauma patient. If microsurgical repair of Stensen's duct provides good functional results, ignoring such lesions often leads to later complications such as fistulae and sialocoeles. Although simple ligation of Stensen's duct has been reported to give satisfactory results, the authors prefer a more anatomic reconstruction of this struc
