Monitoring abdominal near-infrared spectroscopy during feeds in neonates with CHD recovering from surgery: a feasibility study.

OBJECTIVE: Monitoring cerebral and renal near-infrared spectroscopy for regional venous oxygenation is a common practice in the postoperative care of neonates recovering from surgery for CHD. In this study, we aimed to test the feasibility of using this technology for monitoring changes in splanchnic perfusion during feeds in infants recovering from cardiac surgery. METHODS: We monitored renal and splanchnic near-infrared spectroscopy in 29 neonates once recovered from the critical postoperative state and tolerating full enteral nutrition. Infants were tested over 3 feeds for splanchnic regio

Lessons Learned From the First Pilot Study of the Compensatory Reserve Index After Congenital Heart Surgery Requiring Cardiopulmonary Bypass.

BACKGROUND: Early warning systems that utilize dense physiologic data and machine learning may aid prediction of decompensation after congenital heart surgery (CHS). The Compensatory Reserve Index (CRI) analyzes changing features of the pulse waveform to predict hemodynamic decompensation in adults, but it has never been studied after CHS. This study sought to understand the feasibility, safety, and potential utility of CRI monitoring after CHS with cardiopulmonary bypass (CPB). METHODS: A single-center prospective pilot cohort of patients undergoing pulmonary valve replacement was studied. C

Laparoscopic surgery during pregnancy.

Important factors in laparoscopic surgery during pregnancy are listed here: There is a risk of aspiration because of a hormonally induced decrease in lower esophageal sphincter tone and mechanical effects of a gravid uterus. Supine hypotensive syndrome because of aortocaval compression can be a major problem. Pneumoperitoneum during pregnancy results in more pronounced restrictive lung physiology. Avoid hypoxemia, hypotension, acidosis, hypoventilation, and hyperventilation. No anesthetic drugs have been proven to be teratogenic in humans. Surgery during pregnancy is associated with the delive

Benefits and methods of achieving strict glycemic control in the ICU.

Hyperglycemia, a frequent complication in critically ill patients, has been shown to have a negative influence on morbidity and mortality. Many factors contribute to hyperglycemia, including the stress response, diabetes, obesity, advanced age, corticosteroids, sepsis, pancreatitis, and the use of nutrition support. Application of intensive insulin therapy, when compared with conventional glycemic control measures, seems to improve outcomes in the critically ill patient. Therefore, effective insulin therapy along with appropriate nutrition support prescriptions provide a means for the critical

Remote Patient Monitoring for Global Emergencies: Case Study in Patients With COVID-19.

BACKGROUND: The COVID-19 pandemic has highlighted the critical need for telehealth and remote patient monitoring in health care delivery. Despite the growing use of on-body wearable sensors for continuous monitoring and predicting adverse events, their widespread adoption remains a significant challenge. While the pandemic has accelerated the acceptance of these technologies, achieving widespread integration requires their sustained incorporation into routine health care practices beyond emergencies. In this study, we extend the application of our previously developed remote patient monitoring

Unlocking the potential of high-resolution multimodality neuromonitoring for traumatic brain injury management: lessons and insights from cases, events, and patterns.

Multimodality neuromonitoring represents a crucial cornerstone for patient management after acute brain injury. Despite the potential of multimodality neuromonitoring (particularly high-resolution neuromonitoring data) to transform care, its full benefits are not yet universally realized. There remains a critical need to integrate the interpretation of complex patterns and indices into the real-time clinical decision-making processes. This requires a multidisciplinary approach, to evaluate and discuss the implications of observed patterns in a timely manner, ideally in close temporal proximity

In vivo Testing of the Pressio Intracranial Pressure Monitor.

BACKGROUND: Measuring reliable intracranial pressure (ICP) is critical for patients with acute brain injuries. The aim of this study was to evaluate zero drift of the intraparenchymal strain gauge Pressio transducer (Sophysa, Orsay, France) in clinical conditions. METHODS: A prospective observational multicenter study was conducted in four French intensive care units of university hospitals. Patients with acute brain injuries were included if they needed ICP measurement using the Pressio transducer. The zero drift was measured at the explantation of the sensor. ICP-related adverse events were

Transcutaneous Carbon Dioxide Monitoring During Apnea Testing for Determination of Neurologic Death in Children: A Retrospective Case Series.

OBJECTIVES: Determination of neurologic death in children is a clinical diagnosis based on absence of neurologic function with irreversible coma and apnea. Apnea testing during determination of neurologic death assesses spontaneous respiration when PaCO2 increases to greater than or equal to 60 and greater than or equal to 20 mm Hg above pre-apneic baseline. The utility of transcutaneous carbon dioxide measurements during apnea testing in children is unknown. We seek to determine the degree of correlation between paired transcutaneous carbon dioxide and PaCO2 values during apnea testing for de

Evaluation of monitoring methods in asymptomatic dogs with high serum cystatin C concentrations.

This study evaluated the monitoring methods in asymptomatic dogs with high serum cystatin C (Cys-C) concentrations. Ten dogs with high serum Cys-C were divided into two groups based on the owner's choice; one receiving clinical pathology-based monitoring at an animal hospital specialised in chronic kidney disease, and the other receiving symptom-based monitoring at home, partly because they showed no clinical symptoms. The dogs that received the clinical pathology-based monitoring led to an early treatment intervention, resulted in a longer survival period than dogs received the symptom-based

[Prescription and follow-up of antithrombotic treatment with vitamin K antagonists].

The indications for antithrombotic treatment with vitamin K antagonists are now relatively precise, but management of this treatment is often difficult in clinical practice and be set by problems such as unstable hypocoagulability, an increased bleeding risk, interactions with other therapies and pathologies, and high-level vitamin K intake in the diet. Rigorous and accurate information of the patient and family, along with regular and frequent control of the international normalized ratio (INR), are essential for the safety and efficacy of this treatment. Some physicians cite an excessive ble

Continuous oximetry/capnometry monitoring reveals frequent desaturation and bradypnea during patient-controlled analgesia.

BACKGROUND: The most serious complication of patient-controlled analgesia (PCA) is respiratory depression (RD). The incidence of RD in the literature is derived from intermittent sampling of pulse oximetry (Spo(2)) and respiratory rate and defined as a deviation below an arbitrary threshold. METHODS: We monitored postsurgical patients in a hospital ward receiving morphine or meperidine PCA with continuous oximetry and capnography. Nurses responding to audible monitor bedside alarms documented respiratory status and interventions. RESULTS: A total of 178 patients were included in the analysis

Anesthetic management of cerebral aneurysm clipping during pregnancy: a case report.

A 35-week pregnant female presented emergently for clipping of a cerebral aneurysm under general anesthesia. The patient was neurologically stable with an active fetus. Anesthetic goals for this patient included maintenance of uteroplacental perfusion, fetal well-being, and maternal well-being. Maternal monitoring consisted of invasive arterial blood pressure, central venous pressure, and urine output, in addition to the standard monitors for anesthesia. Fetal monitoring consisted of fetal heart rate by external Doppler and uterine activity by external tocometer. Anesthesia care was directed a

Hyperventilation: is it a cause of panic attacks?

During transcutaneous PCO2 (PtcCO2) monitoring of 15 freely ambulant patients suffering from panic attacks, all the patients experienced a typical attack. Seven were identified as hyperventilators, as PtcCO2 fell to abnormally low levels during the attack. This group could not be distinguished on the basis of either their usual symptoms of panic or the hyperventilation provocation test. There was no apparent association between absolute levels of PCO2 and the nature of symptoms. The anxiety ratings of hyperventilators were lower than those of non-hyperventilators. These data do not support the

Integrated machine-learning-assisted wearable platform for stress state assessment based on sweat cortisol and multimodal physiological biosensing.

External stimuli can induce stress-related physiological states characterized by rapid autonomic and cardiovascular fluctuations and slower endocrine-related biochemical responses. To address this challenge, we developed an integrated wearable biosensor platform coupled with machine-learning-assisted interpretation for rest versus stress assessment, with labels defined by baseline and task phases. At the sensing level, a stepwise-modified electrochemical interface enabled sensitive detection of cortisol, thereby providing low-frequency endocrine-related biochemical context. Simultaneously, the

Nonerosive reflux disease.

Nonerosive reflux disease (NERD) is the most common phenotype of gastroesophageal reflux disease. By definition, patients with NERD have typical reflux symptoms caused by the intraesophageal reflux of gastric contents but have no visible esophageal mucosal injury. This is in contrast to patients with erosive reflux disease (ERD) or Barrett's esophagus (BE) who have obvious esophageal mucosal injury on endosco-py. Only 50% of patients with NERD have pathologic esophageal acid exposure as detected on 24-h pH monitoring. NERD patients with physiologic esophageal acid exposure and good temporal co

Passive Smart Home Monitoring for Delirium-Relevant Anomaly Detection in People Living With Dementia: Proof-of-Concept Study.

BACKGROUND: Delirium superimposed on dementia is associated with poor outcomes yet remains underdetected in home settings. Current detection relies on face-to-face clinical assessment (eg, the Confusion Assessment Method criteria), which is rarely applied outside hospitals. OBJECTIVE: This proof-of-concept study developed a theory-driven framework for detecting delirium-consistent anomalous patterns in home-dwelling people with dementia, using passive smart home sensor data. METHODS: The Technology Integrated Health Management dataset, an open access resource comprising a clinically derived

The State of Remote Patient Monitoring for Chronic Disease Management in the United States.

Remote patient monitoring (RPM) increased exponentially during the COVID-19 pandemic. RPM programs commonly incorporate tools to capture and transmit health-relevant data from the home to the clinical space to augment the clinical decision-making process of health care providers. Given the potential to improve patient health outcomes, health care systems around the world are actively engaged in fashioning, implementing, and exploring the outcomes of various RPM program models. However, new challenges to health care systems include increasing RPM program enrollment, optimizing condition-specifi
