[Trachyonychia: 20-nail dystrophy].

Trachyonychia (20-nail dystrophy) is described in four patients with clinical, histological, and scanning electron microscopy techniques. Trachyonychia is a polyetiological symptom: psoriasis vulgaris, lichen planus, alopecia areata, atopic dermatitis, 20-nail dystrophy of childhood, and severe nail dystrophy can all lead to trachyonychia-like nail changes. Nail bed biopsies are necessary for proper diagnosis. In our patients, lichen planus, or psoriasis were excluded clinically and histologically. Spongiotic dermatitis of nail matrix and nail bed with column-like parakeratosis within the nail

The nail dystrophy of psoriatic arthritis.

Nail abnormalities occur frequently in patients with psoriatic arthritis. This study of the finger nails of 46 patients with psoriatic arthritis, 100 nonpsoriatic rheumatism patients, and 100 nonpsoriatic general medical patients was designed to characterise these abnormalities with particular reference to the severity of nail pitting. The results of the study suggest: (1) Onycholysis alone in the absence of previous injury to the affected nail is in favour of a psoriatic origin for the nail dystrophy. (2) Two or all of onycholysis, horizontal ridging, and nail pitting in the same patient are …

Twenty-nail dystrophy of childhood.

Twenty-nail dystrophy is an idiopathic nail dystrophy that begins insidiously in early childhood. All 20 nails are uniformly affected with excessive longitudinal striations and loss of nail luster. The skin and its ectodermal derivatives are otherwise normal. Twenty-nail dystrophy is believed to be a self-limited abnormality that resolves slowly with age.

Nail Squamous Cell Carcinoma Involving the Nail Fold in a Skin of Color Patient: A Case Report.

INTRODUCTION: Nail squamous cell carcinomas (SCCs) in skin of color (SoC) patients are exceedingly rare, with only 29 cases previously reported. CASE PRESENTATION: We describe the case of a female Fitzpatrick skin-type V patient who presented with a 3-year history of a hyperpigmented plaque involving the proximal nail fold of her right thumbnail. The patient reported a positive Pap smear for human papillomavirus (HPV) 16 and 18 seventeen years ago and had not received the HPV vaccine. Biopsy showed SCC positive for high-risk HPV, and the patient underwent Mohs surgery. CONCLUSION: This case

Salvage of a Broken Magnetic Tibia Nail with Plate Fixation Without Nail Removal: A Case Report.

INTRODUCTION: Magnetic intramedullary nails are used for deformity correction and limb lengthening because of their minimally invasive nature and reduced complication rates compared to external fixators. However, complications such as non-union and nail breakage remain rare but significant clinical challenges, particularly in cases with patient-specific risk factors. This case demonstrates a successful union after nail fracture without the removal of the intramedullary nail, using lateral plate fixation, offering a salvage option in this uncommon scenario. CASE REPORT: A 22-year-old female wi…

The Typical Nail Lichen Planus Severity Index: An Outcome Instrument for Typical Nail Lichen Planus.

INTRODUCTION: Despite numerous treatment options for nail lichen planus (NLP), a validated method for measuring the severity of NLP and therapeutic response in clinical trials is absent. The aim of the study was to develop and validate a measurement instrument, Typical Nail Lichen Planus Severity Index (tNLPSI), for typical NLP that could be used in clinical trials. METHODS: A total of 48 patients pathologically confirmed with typical NLP were enrolled in this study. Five dermatologists were trained to use the tNLPSI activity scale and the Physician's Global Assessment (PGA) scale to score sa

Association Between Nail Type and Aseptic Revision Risk After Cephalomedullary Nailing for Hip Fracture.

BACKGROUND: Although the majority of intertrochanteric femoral fractures in the United States are now treated with cephalomedullary nailing, it remains uncertain whether differences in clinical performance by nail type exist. The purpose of this study was to compare the aseptic revision rates associated with the 3 most commonly utilized cephalomedullary nails in the United States today: the Gamma nail (Stryker), the INTERTAN (Smith+Nephew), and the Trochanteric Fixation Nail/Trochanteric Fixation Nail Advanced (TFN/TFNA; DePuy Synthes). METHODS: Using an integrated health-care system's hip fr…

A novel intramedullary nail design of intertrochanteric fracture fixation improved by proximal femoral nail antirotation.

A proper and reliable fracture fixation is important for fracture healing. The proximal femoral intramedullary nail (IN), such as proximal femoral nail anti-rotation (PFNA) or Gamma nail, is widely used for intertrochanteric fracture fixation. However, it still suffers considerable stress concentrations, especially at the junction between the nail and the blade or lag screw. In this study, we propose a novel intramedullary nail design to enhance the intramedullary nail integrity by introducing a bolt screw to form a stable triangular structure composed of the nail, the lag screw, and the bolt
