A 5 year old girl suffering from chronic tonsillitis and bilateral tympanic effusion developed atrophy of the right half of the tongue. With the exception of the twelfth cranial nerve (right) no neurological signs could be detected. We are reporting a case of an isolated hypoglossal nerve paralysis.

Conventional electromyography (EMG) and single fiber EMG (SFEMG) were performed in a 64-year-old diabetic woman with mild type B botulism. Isometric contractions of single motor units in the first dorsal interosseous muscle were recorded with an intramuscular microstimulation technique in patients with short- or long-term hemiplegia and compared with controls. Recent reports suggest an increased incidence for the general population of tarsal tunnel syndrome (TTS) in rheumatoid arthritis (RA), similar to that previously reported for carpal tunnel syndrome. Proof of this supposition could be important in the diagnosis and management of pain in the feet of patients with RA. Forty cases of 'wasted leg syndrome' were studied clinically and electrophysiologically. Muscle biopsy was examined in nine cases, majority of patients were adults engaged in heavy manual work. The illness was noticed incidently with a strictly unilateral wasting of the whole lower limb (in 65% of cases), of all muscles below the knee (in 22.5% of cases) or of quadriceps muscles only (in 12.5% of cases). A nine-year-old child presented with congenital insensitivity to pain and anhidrosis. Quantitative studies and electron microscopy of the cutaneous branch of the radial nerve revealed almost complete absence of small myelinated and unmyelinated fibers and a disproportionate number of nerve fibers with a diameter of 6-10 micrometers. A grouping of both type 1 and type 2 muscle fibers was also seen. We suggest that this disease entity is not caused by a hereditary sensory neuropathy, but rather that it derives from a developmental defect.

Palmar stimulation was used to assess median nerve conduction across the carpal tunnel. In 50 hands from 25 control subjects, motor and sensory latencies in the wrist-to-palm segment (mean +/- SD: 1.15 +/- 0.21 msec and 1.12 +/- 0.21 msec respectively) were less than half the conventional terminal latencies in the wrist-to-muscle and wrist-to-digit segment (3.01 +/- 0.44 msec and 2.47 +/- 0.39 msec). A teased fibre technique was used to study 19 biopsies of the index finger branch of the radial cutaneous nerve of leprosy patients and contacts. These were compared with four normal nerves. Five nerves were from patients with preclinical nerve lesions, five from leprosy patients with minimal sensory nerve impairment, and five from contacts of lepromatous leprosy. The extent of demyelination in preclinical nerve lesions in leprosy and in contacts of leprosy is recorded. The usefulness of nerve conduction velocity studies in early leprosy patients and in contacts is discussed.

In order to study the clinico-electroneurographical correlations in early brachial plexitis of the newborn the authors studied 118 children from the moment of birth up to 1.5-4 years. 16 patients suffering from syringomyelia were checked for additional peripheral nerve lesions. On voluntary effort, often in conformity with the decrease of muscle strength, a moderate rarefication of the discharge patterns was apparent. Fasciculation potentials were common. Motor conduction velocities of the median and ulnar nerve were in the normal range in cases without additional peripheral nervous lesion at the elbow or the carpal. Depending on the severity of the peripheral nervous lesion, we found the duration of the muscle action potentials increased.

A 50-year-old man presented with acute-onset numbness and tingling in his extremities, which progressively worsened, leading to severe gait instability and near-inability to walk. Two weeks earlier, he had experienced an upper respiratory tract infection.
