Atraumatic suction tip for microneurosurgery--clinical experience.

Our two-year experience with a new atraumatic tip for the neurosurgical suction tube is described. During microneurosurgical procedures the tip reduces injury to fragile vital brain structures and protects them from being aspirated into the suction tube. The perforated membrane of the appended suction tip does not impede evacuation of blood in any situation and can be easily cleaned off without demounting. The device can be immediately attached to the tube and disconnected. The neurosurgeon need not concentrate on its use, in contrast to those systems with active suction pressure control. The

[Dumbbell type schwannoma of the posterior mediastinum: a report of two cases with different surgical approaches].

Two cases of dumbbell type schwannoma of the posterior mediastinum are reported. Case 1 involved a 16-year-old man and case 2, a 48-year-old woman. They were asymptomatic, and in both cases an abnormal mass shadow of the left mediastinum was incidentally detected on a routine chest roentogenogram. In these cases, CT and MRI revealed that dumbbell type tumors extended to the spinal canal. The operations were performed with thoracic approach cooperated with a neurosurgeon in case 1 and with combined posterior and thoracic approach cooperated with an orthopedic surgeon in case 2. Histopathologic

A newly designed nerve monitor for microneurosurgery: bipolar constant current nerve stimulator and movement detector with a pressure sensor.

The authors describe a newly designed nerve monitor which is useful for numerous microneurosurgical procedures. Standard bipolar forceps are used to apply constant current stimulation. Muscle contraction evoked by the stimulation is detected by a small disc-shaped pressure sensor taped to the overlying skin. The responses are monitored both quantitatively on a liquid crystal display and qualitatively through an on-off auditory signal. Surgery can proceed without interruption. This apparatus can safely and reliably monitor the facial nerve, nerves involved in eye movements, lower cranial nerves

Arnold-Chiari II malformation: intraoperative real-time US.

Nineteen cases in which high-resolution, intraoperative, real-time sonography was performed during decompression of an Arnold-Chiari II malformation were reviewed. The variations of the often complicated hindbrain anatomic features, well known from autopsy series, were shown in detail with intraoperative sonography. The extent of herniation of the cerebellar vermis, the medulla, and the fourth ventricle into the cervical spinal canal, as well as fourth ventricular dilatation, arachnoid cysts, and cervical hydromyelia, were also demonstrated sonographically. Real-time imaging was useful in guid

[Clinical aspects and pathology of traumatic lesions of the optic nerve].

The cause of the unilateral complete blindness (amaurosis traumatica) occurring in the case of closed cranial injury has been sought for in the optic nerve canal (canalis opticus syndrome). For the decompression of the optic nerve surgical approach has been performed in several institutes and by various neurosurgeons. These interventions lead to no result, irrespective of the time of the operation or the surgical technique. On the basis of the data of the literature and their material the authors came to the conclusion that since the alteration of the optic nerve has been histologically observ…

Real-Time Remote Telerobotic Magnetic Navigation for Endovascular Simulated Stroke Thrombectomy Across 5700 Miles.

BACKGROUND AND OBJECTIVES: While many trials of the last decade have shifted the paradigm of stroke care toward endovascular therapy, a lack of timely access to centers with neurointerventional capabilities for many patients has remained a significant barrier to care. Endovascular robotic systems are poised to address this need for expanded access through telerobotic procedures, while also benefiting clinical team safety and raising the possibility of improved procedural precision. METHODS: A telerobotic setup consisting of a magnetic field generator and a mechanical advancer was placed with

Evaluation of Retrospective Patient Preference on Extent of Resection of Nonfunctioning Pituitary Adenomas.

BACKGROUND: Neurosurgeons treat nonfunctioning pituitary adenomas by surgical resection. Based on the adherence of the tumor to the normal pituitary gland, operative risks may include hormone replacement therapy for postoperative hypopituitarism with gross total resection that injures the gland or recurrent tumor with subtotal resection and purposeful avoidance of gland manipulation. None of the patients presented in this article had a preoperative preference regarding extent of resection. This study aimed to evaluate postoperative patient preferences regarding extent of resection. METHODS: A…

Cascade of decisions meet personal preferences in sciatica treatment decisions.

STUDY DESIGN: An in-depth interview study including patients, general practitioners, neurologists and neurosurgeons. OBJECTIVE: To gain insight in decision-making in sciatica care, by identifying patients' and physicians' preferences for treatment options, and the differences between and within both groups. SUMMARY OF BACKGROUND DATA: Sciatica is a self-limiting condition, which can be treated both conservatively and surgically. The value of both options has been disputed, and the care pathway is known for a substantial amount of practice variation. Most Dutch patients are taken care of by g…

Endoscopic vs. microscopic transsphenoidal surgery for Cushing's disease: a systematic review and meta-analysis.

PURPOSE: Systematic review and meta-analysis comparing endoscopic and microscopic transsphenoidal surgery for Cushing's disease regarding su … d of care. Microscopic surgery can be used based on neurosurgeon's preference. Endocrinologists and neurosurgeons in pituitary centers performing the microscopic technique should at least consider referring Cushing's disease patients with a macroadenoma.

The bioethics movement in India and the legacy of Sunil Pandya.

The passing of Sunil K Pandya (SKP) on December 17, 2024, evoked memories of his remarkable life both as an individual and a medical professional in many physicians, patients and colleagues. In her tribute, Lopa Mehta has described him as "Medical Ethics personified, an astute clinician, a dexterous neurosurgeon, an ardent historian, a philosopher who stood high above worldly gains and fame" [1]. Early in January 2025, we published an anthology of SKP's select articles in IJME as a tribute to his contribution to bioethics and to the journal. The reminiscences of his friends and colleagues in t

Third Ventricle Craniopharyngioma Intraventricular Tumor: A Case Report and a Brief Literature Review.

Craniopharyngioma is a rare embryonic malformation, usually benign, of the sellar or parasellar regions. In this study, an uncommon case of third ventricle craniopharyngioma and a succinct review of its prevalence are presented. Even with low mortality rates, these injuries involve a high degree of endocrinological, visual, and neuropsychological morbidities, which have a huge impact on the patient's quality of life. Thus, surgical resection is the preferred therapy for tumors that compromise the flow of cerebrospinal fluid. However, due to the location of the craniopharyngioma, surgical manag

The relationship between spontaneous cerebrospinal fluid leak and idiopathic intracranial hypertension.

Surgical treatment of spontaneous cerebrospinal fluid (CSF) leak is now performed by ENT surgeons, endonasal endoscopy being preferred to craniotomy as less invasive. However, it is often the symptom of underlying idiopathic intracranial hypertension, which lies outside the traditional sphere of ENT competence. Surgery is a necessary step, but should not obscure the need to treat the underlying pathology. This treatment is complex, and requires multidisciplinary team-work between otorhinolaryngologist, ophthalmologist, neurologist, neurosurgeon, radiologist, dietician, endocrinologist and psyc

The Benefits of a Guideline on Safe Termination of Pregnancy for Legal Indications: An Illustrative Case Report of a Hydranencephaly.

After years of the worsening burden of unsafe abortion and attendant morbidities and mortalities in Nigeria, a National Guideline on the Safe termination of pregnancy for legal indications was enunciated. This report presents and discusses an illustrative case of a hydranencephaly that benefited from it. A 43-year old multipara was informed during routine ultrasonography at booking for antenatal care, at 16 weeks of gestation, of a major defect in her baby and advised to meet her physician. Following a repeat high- resolution ultrasonography and discussions between the Obstetricians, Neurosurg

Schwannoma of the Recurrent Laryngeal Nerve: A Rare Entity.

Neurogenic tumors are the most common posterior mediastinal tumors in adults. Schwannomas originating from the recurrent laryngeal nerve are rare. The present study describes a 46-year-old man with a tumor in the left superior mediastinum. Because of the narrow relationship with the aorta and the left pulmonary artery, the tumor was excised by left-sided minithoracotomy. The tumor, a schwannoma, originated from and encased the left recurrent laryngeal nerve. Six months after surgery, the patient was free of recurrence without symptoms other than hoarseness. "Additional imaging by magnetic reso

Neuro-orthopaedic conditions in spina bifida: natural course and their management and long-term outcomes.

INTRODUCTION: Management of spina bifida is a complex issue with various systems of the body involved either directly or indirectly. DISCUSSION: Deformities of the bone and joints (especially spine and lower limbs) play a significant role in causation of deficits in the child. Management of these deficits require a focused, specialised and multidisciplinary approach with orthopaedician, physiotherapist, intensivist and psychologist apart from neurosurgeon playing an important part. To manage these deformities, one needs to understand the pathophysiology of causation of these defects. The corr

Endoscopic treatment of intranasal glioma in an infant presenting with dyspnea.

Nasal gliomas are uncommon congenital lesions arising from abnormal embryonic development. Clinically, these masses are firm and uncompressible. Histologically, they are made up of astrocytes and neuroglial cells, embedded in fibrous and vascular connective tissue. Proper management of a nasal glioma requires a multidisciplinary approach including an otorhinolaryngologist, radiologist, and neurosurgeon. Radiological investigations such as computed tomography or magnetic resonance imaging should be performed to exclude intracranial extension. The mainstay of treatment is complete surgical extir
