We report a case of carcinoid syndrome requiring an emergency operation for an upper gastrointestinal perforation. A 46-year-old man had undergone left lower lobectomy for a lung carcinoid tumor seven years previously, and liver metastasis was found five years previously. He developed cutaneous flushing and watery diarrhea, and was diagnosed with carcinoid syndrome one year previously. Although he was treated with octreotid, his symptoms became worse and he was admitted to our hospital. Diffuse idiopathic endocrine neoplasia of the lung (DIPNECH) is a rare disease characterized by proliferation of neuroendocrine cells in the bronchial wall. Less than 20 cases of DIPNECH have been reported in imaging literature. We present here a case of histopathologically diagnosed DIPNECH with diffusely increased In-octreotide uptake in both lungs.

The treatment of small bowel fistulas remains a difficult problem. Spontaneous closure rates of 30% are currently being achieved after 4 to 6 weeks of conservative therapy with bowel rest, TPN, octreotide, and control of sepsis. This article reports progress in the field of endocrinopathies and focuses on the molecular aspects of these diseases. Implications for genetics and metabolic study are presented. Although limitations of earlier approaches are confirmed, progress is noted, particularly with regard to the contribution of octreotide scintigraphy. Integrated with the evolving applications of molecular insights, significant clinical progress has been recorded.

Bleeding from esophageal varices is the most severe complication of portal hypertension, and should be managed in specially trained centers. Vasoactive drugs, mainly octreotide, plus endoscopic treatment are able to control bleeding in 90% of the cases. Rescue treatments like TIPS and surgery should be immediately available for those who do not stop bleeding or have varices difficult to manage.

A 40-year-old man presented with abdominal pain and odynophagia. CT scan revealed a pseudocyst in the posterior mediastinum and pleural effusion complicating pancreatitis. He was managed with transpapillary pancreatic duct stenting along with other supportive measures including octreotide, and he improved symptomatically. The stent was removed after four months; there was no pseudocyst on follow up CT scan.

We report a patient with persistent congenital hyperinsulinism (CHI) who developed recurrent hypoglycaemia following fluoxetine therapy.
