The great imitator--still today! A case of meningovascular syphilis affecting the posterior circulation.

Cerebral ischemia due to meningovascular syphilis is rare and more frequently affects the anterior circulation than the posterior circulation. We describe clinical features and imaging studies of a 50-year-old patient with Parinaud syndrome and a syphilitic dorsal midbrain infarction. Brain magnetic resonance imaging indicated vasculitis of the posterior circulation. The diagnosis of meningovascular syphilis was established by serum and cerebrospinal fluid examinations. Although rare, because of the high impact on treatment, clinicians should always be aware of meningovascular syphilis in the

Pediatric internuclear ophthalmoplegia.

BACKGROUND: Internuclear ophthalmoplegia (INO) is a rare eye movement disorder in the pediatric population. METHODS: We performed a retrospective review at a university-based tertiary referral ophthalmology practice from 2004 to 2012 to identify pediatric patients with INO. RESULTS: Three patients with INO were identified. Etiologies included high-grade astrocytoma, perinantal hypoxia and neonatal intracerebral hemorrhage. One of our patients, a 2 year-old girl, is the youngest reported case of INO. CONCLUSION: While rare, INO in a pediatric patient requires a full neurologic evaluation and

Symptoms in children with convergence insufficiency: before and after treatment.

PURPOSE: To investigate symptom patterns and evaluate the relationship between patient characteristics and symptom severity before and after treatment for symptomatic children with convergence insufficiency (CI). METHODS: In a randomized clinical trial, the convergence insufficiency symptom survey was administered pre- and posttreatment to 221 children aged 9 to <18 years with symptomatic CI. Frequency of symptom type was determined at baseline, mean change in performance-related vs. eye-related symptoms for treatment responders was compared, and the relationship between patient characteristi

Modeling the mechanisms of Uhthoff's phenomenon in MS patients with internuclear ophthalmoparesis.

Internuclear ophthalmoparesis (INO) is the most common saccadic eye movement disorder observed in patients with multiple sclerosis (MS). It is characterized by slowing of the adducting eye during horizontal saccades, and most commonly results from a demyelinating lesion in the medial longitudinal fasciculus (MLF) within the midline tegmentum of the pons (ventral to the fourth ventricle) or midbrain (ventral to the cerebral aqueduct). Recent research has demonstrated that adduction velocity in MS-related INO, as measured by infrared eye movement recording techniques, is further reduced by a sys

Cortical activation during optokinetic stimulation - an fMRI study.

CONCLUSION: Activation of cortical areas related to visual motion processing and control of eye movement, and deactivation of parieto-insular vestibular cortices (PIVC) were revealed by functional magnetic resonance imaging (fMRI) with small-field optokinetic stimulation (OKS). The results agreed well with those of previous studies, which indicates that the current protocol is reliable enough to be used as a clinical examination. OBJECTIVES: To propose an fMRI set-up with OKS that is reliable and simple enough to be performed as a clinical test. SUBJECTS AND METHODS: Ten right-handed healthy

[Small cerebellar vascular lesion can produce isolated conjugate deviation of the eyes].

The cerebellum has almost never been considered responsible for conjugate deviation of the eyes (CDE). A few cases of CDE caused by cerebellar lesions without the involvement of the brainstem have been reported, but the lesions were too large to evaluate their localization in the cerebellum. In this report, we describe 2 cases of isolated CDE caused by small cerebellar vascular lesions and a case of CDE and staggering gait that occurred following cerebellar infarction. We further describe cases of head rotation without CDE and those of vertigo without CDE or head rotation due to a similar smal

Magnetoencephalographic findings of Panayiotopoulos syndrome with frontal epileptic discharges.

We previously reported the results of a magnetoencephalographic study in patients with Panayiotopoulos syndrome manifesting occipital epileptic discharges, in which the equivalent current dipoles of spike discharges were clustered alongside the major cortical sulci, such as parieto-occipital and calcarine. This report is the result of a magnetoencephalographic study of three patients with Panayiotopoulos syndrome exhibiting equivalent current dipoles clustering in the frontal area. Patient 1, a 13-year-old male, exhibited clustering equivalent current dipoles alongside right inferior frontal s

[Parallel processing of motion vision].

Several neuropsychological studies have reported dissociation between motion vision and object vision. One patient with motion blindness had a bilateral MT/V5 lesion and could see objects, but could not see the motion of the objects (Zihl et al, 1983). By contrast, some blindsight patients with primary visual cortex lesions cannot see objects but can see their movement (e.g. Riddoch, 1917). These results imply that movement vision and form vision rely on independent mechanisms. However one patient with motion blindness had controversial symptoms concerning motion vision. She could not perceive

Ectopic brain in the orbit with congenital adduction deficit and simultaneous abduction.

An 11-month-old girl was referred to us with enophthalmos and restriction of ocular movement in the right eye. There was restriction of supraduction and adduction with synergistic divergence on left gaze. MRI revealed a multiloculated cystic mass in the right inferior orbit. There was an atrophic and misinserted inferior rectus muscle and only a vestigial right medial rectus muscle. Excisional biopsy of the orbital cystic mass was performed. Microscopic examination disclosed that the mass was brain tissue composed of arachnoid tissue, brain parenchyma, choroid plexus, and dura mater. Our repor

Ocular myasthenia revisited: insights from pseudo-internuclear ophthalmoplegia.

Ocular myasthenia can mimic central disorders of eye movements. We compared horizontal saccades in two patients with myasthenia gravis who presented as pseudo-internuclear ophthalmoplegia (pseudo-INO), two patients with true INO due to multiple sclerosis (MS), and five healthy subjects. In myasthenics, peak velocity of horizontal saccades was similar to, or greater than, controls; in MS patients, adducting saccades were slower than controls. Differences between the peak velocity of abducting and adducting eyes for each saccade were similar to controls for myasthenic pseudo-INO, but greater tha

[Parinaud's oculoglandular syndrome. A rare differential diagnosis of "red eye"].

Two cases of Parinaud's oculoglandular syndrome, which represents an ocular manifestation of cat-scratch disease, are reported. The symptoms are subacute and include unilateral conjunctivitis and pre-auricular lymphadenopathy. Diagnosis primarily relies on the recognition of suggestive clinical signs in conjunction with positive serologic testing. In most cases, therapy is not necessary.

Graves' disease with moderate-severe endocrine ophthalmopathy-long term results of a prospective, randomized study of total or subtotal thyroid resection.

The optimal thyroid surgery to be performed in patients with Graves' disease and concomitant endocrine ophthalmopathy is a matter of debate. We randomly assigned 44 patients with moderate-severe eye signs in a trial of treatment with subtotal, leaving a small (approximately 2 g) thyroid remnant, or total thyroidectomy. At inclusion, the patients had been treated with antithyroid drugs, and corticosteroids been given to 12 (27%). All received postoperative thyroxine supplementation and were followed for 3 years at regular examinations. The eye disease improved in all cases, and throughout the s

Design of and normative data for a new computer based test of ocular torsion.

PURPOSE: To evaluate a new clinically practical and dynamic test for quantifying torsional binocular eye alignment changes which may occur in the change from monocular to binocular viewing conditions. METHODS: The test was developed using a computer with Lotus Freelance Software, binoculars with prisms and colored filters. The subject looks through binoculars at the computer screen two meters away. For monocular vision, six concentric blue circles, a blue horizontal line and a tilted red line were displayed on the screen. For binocular vision, white circles replaced blue circles. The subject

Ocular flutter associated with a localized lesion in the paramedian pontine reticular formation.

Ocular flutter is a rare horizontal eye movement disorder characterized by rapid saccadic oscillations. It has been hypothesized that it is caused by loss of "pause" neuronal inhibition of "burst" neuron function in the paramedian pontine reticular formation (PPRF); however, there have been no imaging studies confirming such anatomical localization. We report the case of a woman with an acute attack of multiple sclerosis associated both with ocular flutter and a circumscribed pontine lesion, mainly involving the PPRF on magnetic resonance imaging. As she recovered from the attack, both the mid

Major orbital complications of endoscopic sinus surgery.

BACKGROUND: The paranasal sinuses are intimately related to the orbit and consequently sinus disease or surgery may cause severe orbital complications. Complications are rare but can result in serious morbidity, the most devastating of which is severe visual loss. METHODS: A retrospective review was undertaken of four cases of severe orbital trauma during endoscopic sinus surgery. RESULTS: All the cases suffered medial rectus damage, one had additional injury to the inferior rectus and oblique, and two patients were blinded as a result of direct damage to the optic nerve or its blood supply.

[Paraneoplastic opsoclonus-myoclonus syndrome revealing T-cell lymphoma].

BACKGROUND: Nonrhythmic involuntary ocular oscillations and axial and segmentary myoclonia are associated in the opsoclonus-myoclonus syndrome. In adults, a paraneoplastic origin is generally found. We report the first of opsoclonus-myoclonus associated with non-Hodgkin's lymphoma. CASE REPORT: A 66-year-old woman rapidly developed a typical opsoclonus-myoclonus syndrome within a few hours, presenting vertigo, cerebellous ataxia, multidirectional involuntary ocular movements and non-rhythmic axial and segmentary myoclonia. Brain computed tomography and magnetic resonance imaging demonstrated

A family with autosomal dominant distal arthrogryposis multiplex congenita and brown syndrome.

Arthrogryposis multiplex congenita is a heterogeneous condition found in a number of different disorders and characterized by congenital joint contractures. We describe typical signs of congenital Brown syndrome (inability to elevate the affected eye actively or passively in full adduction) in three relatives with distal arthrogryposis multiplex congenita. We found a thickening of the superior oblique muscles in these patients with pain and increased intraocular pressure in upgaze. The pathogenesis of clinical and morphological findings is discussed. The association of Brown syndrome with dist
