Effects of Corrective Taping on Balance and Gait in Patients With Hallux Valgus.

BACKGROUND: Taping is an effective temporary therapy for improving hallux valgus (HV) in adults. Although HV has been demonstrated to impair postural balance, there is a lack of information about how corrective taping affects balance and gait patterns in adults with HV deformity. METHODS: Eighteen middle-aged female patients (average age, 53.5 years) with HV were included. Corrective tape was applied to correct HV angulation. A series of balance and gait stability tests were performed before applying tape and 1 hour after the tape was applied with a Balance Master computerized posturography d

Esophageal perforation after anterior cervical spine surgery: a systematic review of the literature.

OBJECTIVE Esophageal perforation is a rare but well-known complication of anterior cervical spine surgery. The authors performed a systematic review of the literature to evaluate symptomatology, direct causes, repair methods, and associated complications of esophageal injury. METHODS A PubMed search that adhered to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines included relevant clinical studies and case reports (articles written in the English language that included humans as subjects) that reported patients who underwent anterior spinal surgery and

Scaphoid Fracture--Overview and Conservative Treatment.

Scaphoid fracture is the most common fracture in carpal bones. Although the reported union rate of conservatively-treated fractures is more than 90%, there is a controversy over the most appropriate treatment. In recent years, many reports have compared the clinical outcomes of conservatively-treated scaphoid fractures with the results of operatively treated ones using randomized controlled trials and meta-analyses. However, there is no consensus regarding the optimal treatment. In this paper, based on literature review, we provide an overview of the management of the acute scaphoid fractures

[Spare-parts concept in pediatric hand: About one bilateral syndactyly].

INTRODUCTION: The spare-parts concept is well described in traumatology of the hand. Tissues of a scarified ray are used to reconstruct another damaged finger. We will detail an application in pediatric hand through a clinical case. MATERIALS AND METHOD: We describe a patient presenting a bilateral complete and non-complex syndactyly of the third web space associated with a bilateral hexadactyly classified as type B according to the Temtamy and Mc Kusick's classification. The treatment of the syndactyly was performed with a dorsal flap for the web space and with longitudinal zigzag incisions

Editorial Commentary: Operative Treatment of Primary Patellar Dislocation.

Systematic review of the literature does not support operative treatment of primary patellar dislocation, as the results of nonoperative treatment are similar. However, surgery is indicated for recurrent patellar dislocation, and in the future, we anticipate that contemporary medial patellofemoral ligament reconstruction outcomes for primary patellar dislocation will prove superior to nonoperative treatment.

Hallux metatarsophalangeal (MTP) joint arthroscopy for hallux rigidus.

UNLABELLED: Arthroscopy of the hallux metatarsophalangeal (MTP) joint is an evolving technique for hallux rigidus and other disorders of the hallux MTP joint. Purported advantages of this minimally invasive procedure include rapid rehabilitation and the ability to identify and address other intra-articular and sesamoid pathologies without additional dissection. Larger, prospective, and comparative outcomes studies are needed to optimize the treatment algorithm. LEVEL OF EVIDENCE: Level V, expert opinion.

Nonsurgical Management of Early-onset Scoliosis.

Early-onset scoliosis is potentially fatal if left untreated. Although surgical management with growing instrumentation may be necessary, this is not a panacea and is associated with high complication rates. Recent evidence has demonstrated that nonsurgical treatment can be an effective early management strategy in delaying or even precluding the need for surgery, especially surgery with growing instrumentation. The goal of both nonsurgical and surgical management is to control or correct the spinal curve to allow appropriate pulmonary development while delaying definitive fusion until an appr

Tarsal tunnel syndrome-A narrative literature review.

BACKGROUND: Tarsal tunnel syndrome is classified as a focal compressive neuropathy of the posterior tibial nerve or one of its associated branches individually or collectively. The tunnel courses deep to fascia, the flexor retinaculum and within the abductor hallucis muscle of the foot/ankle. The condition is rare and regularly under-diagnosed leading to a range of symptoms affecting the plantar margins of the foot. There are many intervention strategies for treating tarsal tunnel syndrome with limited robust evidence to guide the clinical management of this condition. The role of conservative

Thromboprophylaxis in haemophilia patients undergoing orthopaedic surgery.

The review of literature on thromboprophylaxis in nonhaemophilia patients undergoing orthopaedic surgery and on thromboprophylaxis in people with haemophilia undergoing orthopaedic surgery leads us to the conclusion that we cannot currently recommend thromboprophylaxis extensively in haemophilia patients undergoing orthopaedic surgery. This sharply contrasts with what the literature recommends for nonhaemophilia patients, that is to say mixed pharmacological and mechanical thromboprophylaxis. However, if we detect several risk factors for thromboembolism in a haemophiliac patient who is to und

Surgical management of metastatic disease of the proximal part of the femur.

➤ Pathologic fracture of the proximal part of the femur resulting from metastatic disease causes severe pain and an inability to walk. Surgical stabilization can be challenging because of bone loss resulting from the underlying metastatic lesions, the potential for major blood loss, and the poor health of the patient.➤ The goal of surgical treatment is the creation of a stable construct to allow early weight-bearing.➤ Pathologic femoral neck fractures secondary to metastases are best managed with arthroplasty.➤ The treatment of intertrochanteric or subtrochanteric fractures is more controversi

Arthroscopic and open management of posterolateral rotatory instability of the elbow.

Posterolateral rotatory instability (PLRI) is the most common cause of residual instability following a simple elbow dislocation. PLRI may result from trauma or iatrogenic injury to the radial ulnohumeral ligament during treatment for other conditions, such as lateral epicondylitis. PLRI can be identified through a combination of history and physical examination, and confirmed with magnetic resonance imaging arthrography. Once diagnosed, surgery is necessary to correct persistent instability. Instability can be confirmed arthroscopically through several findings, including subluxation of the r

Management of congenital radial longitudinal deficiency: controversies and current concepts.

Radial longitudinal deficiency is a spectrum of upper extremity dysplasia and hypoplasia affecting the proximal arm and the radial aspect of the forearm, wrist, and hand. Often, the hand surgeon is the first to evaluate a patient with radial longitudinal deficiency and thus must be aware of its common associated syndromes. Specific evaluation, including clinical examination and laboratory testing, is necessary. At this time, there are many surgical approaches that can be used for treatment of radial longitudinal deficiency. The procedures should be specifically tailored to the patient and fami

Surgical treatment of femoroacetabular impingement in patients with slipped capital femoral epiphysis: A review of current surgical techniques.

Slipped capital femoral epiphysis (SCFE) is relatively common in adolescents and results in a complex deformity of the hip that can lead to femoroacetabular impingement (FAI). FAI may be symptomatic and lead to the premature development of osteoarthritis (OA) of the hip. Current techniques for managing the deformity include arthroscopic femoral neck osteochondroplasty, an arthroscopically assisted limited anterior approach to the hip, surgical dislocation, and proximal femoral osteotomy. Although not a routine procedure to treat FAI secondary to SCFE deformity, peri-acetabular osteotomy has be

Breaking the rules: Dupuytren's disease under Skoog's fibres.

Skoog and others have concluded that the Skoog fibres, or transverse ligament of the palmar aponeurosis (TLPA), are never involved in the Dupuytren's disease process. Here we present a case in which this clearly occurs. Whilst for the vast majority of Dupuytren's cases the Skoog's fibres delineate the required deep extent of dissection for pretendinous cords, this case proves an exception, highlighting an occasional need to beware of accepting the conventional wisdom.

Complications after flexor tendon repair: a systematic review and meta-analysis.

PURPOSE: Although outcomes after flexor tendon repair have reportedly improved with modern treatment, complications are common. The purpose of this study was to determine the incidence of these complications and the potential contributory factors within the published literature. METHODS: We performed a systematic review of the available literature to identify publications in which patients with flexor tendon ruptures were surgically treated. We extracted demographics, zone of injury, core suture technique (only modified Kessler or a combination of techniques), use of epitendinous suture, and

The Latarjet coracoid process transfer procedure: alterations in the neurovascular structures.

BACKGROUND: The Latarjet coracoid process transfer procedure is an established, reliable treatment for glenoid deficiency associated with recurrent anterior shoulder instability, but changes in neurovascular anatomy resulting from the procedure are a concern. The purpose of our cadaveric study was to identify changes in the neurovascular anatomy after a Latarjet procedure. MATERIALS AND METHODS: We obtained 4 paired, fresh-frozen cadaveric forequarters (8 shoulders) from the Maryland State Anatomy Board. In each shoulder, we preoperatively measured the distances from the midanterior glenoid r
