Bilateral congenital torticollis: a case report with 25 years of follow-up.

Unilateral congenital sternocleidomastoid muscle (SCM) contracture causing torticollis is well known. Although the unilateral muscular torticollis is quite often recognized, a bilateral contracture of SCM muscle is very rare. A review of the literature showed only three cases of bilateral congenital torticollis reported over the last two decades. We present a case report of a boy with congenital bilateral torticollis with 25 years of follow-up. Bilateral tenotomies of the right SCM were performed and the child was immobilized in Schanz's cervical orthosis. Three months after operative procedur

Management of restricted mouth opening caused by radiation: A clinical report.

Trismus, a commonly observed sequela in patients who have undergone radiation therapy to treat malignancies of the head and neck, causes a loss of function that reduces patients' overall quality of life. Radiation can cause intense fibrosis in the masticatory muscles, and this fibrosis may lead to trismus. This clinical report describes the management of a patient with radiation-induced trismus who was treated with a custom-made mouth-opening device. The device had maxillary and mandibular sections, was made of a thermoactive acrylic resin material, and was connected with 2 vertical screws in

Modification of hemiplegic compensatory gait pattern by symmetry-based motion controller of HAL.

As one of several characteristics of hemiplegic patients after stroke, compensatory gait caused by affected limb is often seen. The purpose of this research is to apply a symmetry-based controller of a wearable type lower limb robot, Hybrid Assistive Limb (HAL) to hemiplegic patients with compensatory gait, and to investigate improvement of gait symmetry. The controller is designed respectively for swing phase and support phase according to characteristics of hemiplegic gait pattern. The controller during swing phase stores the motion of the unaffected limb and then provides motion support on

Long-term outcomes in treatment of deformational plagiocephaly and brachycephaly using helmet therapy and repositioning: a longitudinal cohort study.

OBJECTIVES: Deformational plagiocephaly and/or brachycephaly (DPB) is a misshapen head presenting at birth or shortly thereafter, caused by extrinsic forces on an infant's malleable cranium. There are two treatment methods available for DPB: helmeting and repositioning. Little is known about the long-term outcomes of these two treatment options. The purpose of this study was to examine children who received helmeting or repositioning therapy for DPB as infants and compare the long-term head shape outcomes of the two groups. METHODS: A longitudinal cohort study design was used to evaluate chan

Efficacy of a trunk orthosis with joints providing resistive force on low-back load in elderly persons during static standing.

PURPOSE: Postural alignment of elderly people becomes poor due to aging, possibly leading to low-back pain and spinal deformity. Although there are several interventions for treating these conditions, no previous study has reported the effectiveness of a spinal orthosis or lumbosacral orthosis (LSO) in healthy elderly people without specific spinal deformity. We therefore developed a trunk orthosis to decrease low-back muscle activity while training good postural alignment through resistive force provided by joints with springs (here, called the ORF, which stands for orthosis with joints provi

Does routine pin re-torquing of patients wearing a halo-thoracic orthosis reduce the need for pin replacement?

BACKGROUND: The halo orthosis is a treatment option currently used in Australia for cervical spine immobilisation following trauma, fracture and post surgical stabilisation. In a previous study, the authors reported halo pin replacement to be a common complication. The aim of this study was to investigate the potential correlation between routine halo pin re-torquing and the incidence of pin replacement. CASE DESCRIPTION AND METHODS: A retrospective case series study was undertaken. A total of 258 charts were reviewed, with 170 patients included in the study. Patients were fitted with a Breme

Performance of spinal cord injury individuals while standing with the Mohammad Taghi Karimi reciprocal gait orthosis (MTK-RGO).

Most patients with spinal cord injury use a wheelchair to transfer from place to place, however they need to stand and walk with orthosis to improve their health status. Although many orthoses have been designed for paraplegic patients, they have experienced various problems while in use. A new type of reciprocal gait orthosis was designed in the Bioengineering Unit of Strathclyde University to solve the problems of the available orthoses. Since there was no research undertaken regarding testing of the new orthosis on paraplegic subjects, this study was aimed to evaluate the new orthosis durin

The influence of ankle joint mobility when using an orthosis on stability in patients with spinal cord injury: a pilot study.

OBJECTIVES: Perceived risk of falling is an important factor for people with spinal cord injury (SCI). This study investigated the influence of ankle joint motion on postural stability and walking in people with SCI when using an orthosis. METHODS: Volunteer subjects with SCI (n=5) participated in this study. Each subject was fitted with an advanced reciprocating gait orthosis (ARGO) equipped with either solid or dorsiflexion-assist type ankle-foot orthosis (AFOs) and walked at their self-selected speed along a flat walkway to enable the comparison of walking speed, cadence and endurance. Metatarsus adductus: development of a non-surgical treatment pathway.

Metatarsus adductus (MA) occurs in one to two cases per 1000 births and is the most common congenital foot deformity in newborns. The appearance is that of a curved or adducted forefoot with a normal hindfoot. A systematic literature review was conducted to answer the following question: For a child who presents with MA, what is the most evidence-based conservative treatment option? Thirteen articles were reviewed using the National Health and Medical Research Council levels of evidence and guidelines for clinical practice. Conservative treatment options reported on included the following: no

The effect of shoulder immobilization on balance in community-dwelling older adults.

PURPOSE: Shoulder immobilization devices are commonly used in the treatment of older adults following proximal humeral fractures. Immobilization of the shoulder may have a negative effect on balance, which could increase risk for falls. The purpose of this study was to examine the effect of shoulder immobilization on balance in the community-dwelling older adult population as measured by the Berg Balance Scale (BBS). METHODS: Fifty-three subjects (14 men and 39 women, mean age = 75.4 years) participated in the study. The BBS was administered twice to each participant. Subjects were tested onc

Efficacy of a multifaceted podiatry intervention to improve balance and prevent falls in older people: study protocol for a randomised trial.

BACKGROUND: Falls in older people are a major public health problem, with at least one in three people aged over 65 years falling each year. There is increasing evidence that foot problems and inappropriate footwear increase the risk of falls, however no studies have been undertaken to determine whether modifying these risk factors decreases the risk of falling. This article describes the design of a randomised trial to evaluate the efficacy of a multifaceted podiatry intervention to reduce foot pain, improve balance, and reduce falls in older people. METHODS: Three hundred community-dwelling

Biomechanical effect of electromechanical knee-ankle-foot-orthosis on knee joint control in patients with poliomyelitis.

In this study, an ideal electromechanical KAFO, satisfying stability in the stance and knee flexion in the swing phase during walking, was developed. Biomechanical evaluations were performed on four polio patients by means of three-dimensional gait analyses and energy consumption studies. From the three-dimensional gait analysis on poliomyelitis patients, a considerable amount of knee flexion during the swing phase was observed in controlled-knee gait, which resulted in approximately 33% less energy consumption than in locked-knee gait. The developed electromechanical KAFO in this study was he

Improvement in functional ambulation as a therapeutic effect of peroneal nerve stimulation in hemiplegia: two case reports.

Two chronic stroke survivors who utilized an ankle foot orthosis (AFO) prior to study entry were evaluated at baseline and after 4 weeks of daily use of a surface peroneal nerve stimulator. Participants were assessed without their dorsiflexor assistive device, using the modified Emory Functional Ambulation Profile (mEFAP). The participants demonstrated improvement in all 5 components of the mEFAP relative to baseline. These case reports indicate that enhanced functional ambulation may be an important therapeutic effect of peroneal nerve stimulation. Potential mechanisms are discussed. Controll

A new hip-knee-ankle-foot sling: kinematic comparison with a traditional ankle-foot orthosis.

In this study, we performed a kinematic analysis of a new, low-cost sling for the lower limb, compared to a common ankle-foot orthosis (AFO). Gait with no orthosis, with the AFO, and with the new sling was analyzed in one hemiplegic subject. Both the AFO and the sling reduced the mean angle and ROM (range of movement) of the ankle and the vertical displacement of the center of mass. The sling, but not the AFO, restored the normal sequence heel-strike, forefoot contact of the affected side. The sling, but not the AFO, reduced the affected limb stance and stride duration, increased stride length
