Thirty-two cases with clostridial gas gangrene were treated during the years 1971-87 in the Department of Surgery, Turku University Central Hospital. The presumptive diagnosis was made on the basis of the clinical appearance of the patient and presence of gram-positive bacilli on a smear. Each patient underwent surgical debridement, antibiotic therapy and hyperbaric oxygen treatment. Seventeen cases had diffuse spreading myonecrosis, 11 of whom survived. Fifteen patients developed clostridial cellulitis with toxicity, 12 survived. Thus the over-all mortality was 28.1%. All those patients who d

Intestinal Ileus is Gut Shock caused by Bowel Hypoxia. The morbidity and mortality of Intestinal Ileus has puzzled more than two generations of investigators because they have overlooked the fact that the gas which collects in obstructed small intestine is mostly (90+%) Nitrogen. For some strange reason a gut full of nitrogen has not been looked on as comparable to a lung full of nitrogen, even though the lung and gut have a common embryological origin. My proposal is that intestinal epithelium lining a nitrogen filled lumen becomes as oxygen starved as alveolar lining in a similar circumstanc

Four patients in status asthmaticus and with alveolar hypoventilation were given a helium-oxygen (He-O2) mixture to breathe during 4 hours. Clinical improvement and return to normal of arterial blood gas levels were rapid and dramatic in all patients. The He-O2 mixture, which is a low density gas, causes a reduction in pulmonary resistance and an improvement in overall alveolar ventilation.

In traumatic injuries to the extremities, with a circulatory insufficiency, the resultant ischemia leads to decreasing levels of the energy-rich compounds adenosine triphosphate (ATP) and phosphocreatine (PCr) and increasing levels of lactate in muscle. A tourniquet model for temporary ischemia was used to determine if hyperbaric oxygen treatment could enhance the cellular metabolic restitution when the circulation was restored. The circulation of the rat hindlimb was interrupted for 1.5 and 3 hours. After 1.5 hours of ischemia, the levels of adenosine triphosphate, phosphocreatine, and lactat

A portable blood oxygenator is developed on the basis of the conventional sorption column. The prime volume of the oxygenator is up to 100 ml thus providing 100% saturation of the blood with oxygen at the perfusion rate of up to 300 ml/min. The device has been used in 54 hemosorption procedures for acute peritonitis, the improvement of the detoxication effect being observed.

A polarographic conjunctival oxygen sensor was used to measure oxygen tension in a tissue bed supplied by the internal carotid artery. The shared vascular source of the conjunctiva and brain suggests that conjunctival PO2 monitoring may provide an index of cerebral perfusion. We studied the effects of hyperventilation, a known stimulus of cerebral vasoconstriction, on conjunctival oxygen tension (PcjO2) in six normal, healthy adults; arterial blood gases were simultaneously measured in four of these subjects. A 5-min period of hyperventilation to a PaCO2 near 20 torr resulted in a rapid and si

Two premature neonates who suffered severe perioperative methemoglobinemia are reported. Simultaneous transcutaneous oxygen monitoring revealed adequate blood oxygen tension. Continuous transcutaneous monitoring may give a false sense of security by reflecting normal blood oxygen tension in the absence of adequate oxygen unloading capacity.

The relative antioxidant efficacy, in vitro, of several antibiotics was examined by studying their effects on the generation of reactive oxygen species (ROS) using zymosan-stimulated polymorphonuclear leukocytes (PMNL) and the cell-free, xanthine-xanthine oxidase system. The species investigated are superoxide radical anion (O2-.), hydrogen peroxide (H2O2), and hydroxyl radical (OH.). Three tetracyclines (tetracycline HCl, oxytetracycline HCl, and minocycline HCl), erythromycin, cephalexin, penicillin G, chloramphenicol, and streptomycin were used as test drugs. At concentrations comparable to

Between January, 1981, and May, 1985, 33 infants suffering from acute cardiorespiratory failure were treated with extracorporeal membrane oxygenation (ECMO) when all other forms of conventional management had failed. Only the patients with respiratory failure that was thought to be reversible were treated. Prolonged conventional respiratory management (more than five days) was considered a contraindication to ECMO support because of irreversible damage to the lungs caused by the barotrauma associated with conventional ventilation. Eighteen of the 33 patients (54%) survived and were discharged

Jugular vein-carotid artery extracorporeal membrane oxygenation (ECMO) was utilized in 22 newborns (16 male and 6 female) 1 to 12 days old with respiratory failure due to meconium aspiration (12 patients), diaphragmatic hernia (4), persistent fetal circulation (3), hyaline membrane disease (2), and Rh incompatibility (1). Prior to ECMO, all patients had alveolar-arterial O2 pressure gradients greater than 580 mm Hg (predicted mortality greater than 90%), weighed more than 1,800 gm, had a gestation period of longer than 35 weeks, and had no cerebral hemorrhage. The duration of ECMO was 41 to 31

Complex biochemical consequences are the result of a series of secondary biochemical changes caused by oxygen depletion. Hypoxia in the fetus and neonate results in decreased GI blood flow, especially to the GI mucosa. Although severe O2 deprivation cannot be entirely compensated for, an increase in tissue O2 extraction does occur in cases of moderate hypoxemia. In the neonate increased demands for O2 during feedings result in increased blood flow. The occurrence of hypoxia during feedings causes a decrease in intestinal motility, suggesting a clinical correlate to feeding intolerance and incr

Chemoreceptors for oxygen reside within the carotid body, but it is not known which cells actually sense hypoxia and by what mechanisms they transduce this information into afferent signals in the carotid sinus nerve. We have developed systems for the growth of glomus cells of the carotid body in dissociated cell culture. Here we demonstrate that, as in vivo, these cells contain the putative neurotransmitters dopamine, serotonin, and norepinephrine. Oxygen tension regulates the rate of dopamine secretion from the glomus cells. Similar to chemically stimulated catecholamine secretion from other

Increased 2-3 Diphosphoglycerate levels in cirrhotic patients have been reported. Previous studies did not show significant changes in 2-3 DPG in anaemic cirrhotic patients when compared to non anaemic cirrhotic patients, but the role played by alkalosis and/or hypoxia has not been investigated. To study this question, haematic 2-3 DPG was measured in 8 male patients with liver cirrhosis (histologically diagnosed) together with PO2, PCO2, pH and Hct. 2-3 DPG was also measured in 6 healthy male volunteers. We found a significant increase in blood 2-3 DPG of cirrhotic patients compared to contro

Using a multi-wire surface electrode, oxygen pressure fields were determined in leg ulcers of 11 patients with chronic venous insufficiency (cvi). In three of them, cvi was combined with arterial occlusive disease. The histograms of all patients with very low PO2 values revealed microcirculatory disturbances of the ulcer tissue. The influence of leg position is reflected in the histograms. Patients with cvi showed higher PO2 values when the leg was raised in comparison with when it was in the hanging position. The patients with mixed ulcerations showed a significant increase in tissue PO2 when

Recent developments in alcoholism : an official publication of the American Medical Society on Alcoholism, the Research Society on Alcoholism, and the National Council on Alcoholism1984eng
Alcoholism is a major health problem, and one of its primary manifestations is alcoholic liver disease. The mechanisms responsible for the various forms of alcoholic liver disease--fatty liver, alcoholic hepatitis, and cirrhosis--are at present poorly understood. Knowledge of these mechanisms is needed to provide a sound framework for the therapy and prevention of liver disease due to alcohol and for the identification of those individuals most susceptible to develop liver disease from alcohol abuse. These experiments were designed specifically to evaluate the postulate that ethanol-induced pe

Peripheral O2 uptake is mandated to subserve the energy requirements of tissue and organ systems in an obligatory aerobe such as the human. In addition to this basic need, local O2 demand may also be increased by more general bodily conditions such as fever, increased catecholamine levels, or alkalosis. Blood flow, which brings O2 to the periphery, is closely regulated at the local level to meet O2 demand except where other functional requirements, such as filtering by the kidney, may intervene. In addition to regulation of total flow at the tissue level, the distribution of flow at the microc
