Evidence for Updating the Core Domain Set of Outcome Measures for Juvenile Idiopathic Arthritis: Report from a Special Interest Group at OMERACT 2016.

OBJECTIVE: The current Juvenile Idiopathic Arthritis (JIA) Core Set was developed in 1997 to identify the outcome measures to be used in JIA clinical trials using statistical and consensus-based techniques, but without patient involvement. The importance of patient/parent input into the research process has increasingly been recognized over the years. An Outcome Measures in Rheumatology (OMERACT) JIA Core Set Working Group was formed to determine whether the outcome domains of the current core set are relevant to those involved or whether the core set domains should be revised. METHODS: Twent

Re-attendance at biennial screening mammography following a repeated false positive recall.

We determined the re-attendance rate at screening mammography after a single or a repeated false positive recall and we assessed the effects of transition from screen-film mammography (SFM) to full-field digital mammography (FFDM) on screening outcome in women recalled twice for the same mammographic abnormality. The study population consisted of a consecutive series of 302,912 SFM and 90,288 FFDM screens. During a 2 years follow-up period (until the next biennial screen), we collected the breast imaging reports and biopsy results of all recalled women. Re-attendance at biennial screening mamm

Melting down the Ice Queen: an integrative treatment of anorexia nervosa.

A 23-year-old woman with anorexia nervosa (AN) and a strong need for control was offered an integrative treatment, empowering the patient to be an active participant and advocating shared decision-making. To emphasise this, both the therapist and patient describe their views on the therapy. The integrative treatment resulted in more psychological flexibility and behavioural improvements, as is evident from an increased weight, a decreased dietary restriction and an increased valued action. The strength of this integrative treatment is based on accepting and encouraging patient's self-chosen tr

Cognitive behavioral therapy for eating disorders.

Cognitive behavioral therapy (CBT) is the leading evidence-based treatment for bulimia nervosa. A new "enhanced" version of the treatment appears to be more potent and has the added advantage of being suitable for all eating disorders, including anorexia nervosa and eating disorder not otherwise specified. This article reviews the evidence supporting CBT in the treatment of eating disorders and provides an account of the "transdiagnostic" theory that underpins the enhanced form of the treatment. It ends with an outline of the treatment's main strategies and procedures.

Patient autonomy in alcohol rehabilitation. II: Program evaluation.

The alcoholism inpatient services at the U.S. Army Hospital Nürnberg are described and assessed with attention to current issues. The results compare favorably with contemporary findings, and include an almost zero dropout rate that relates to giving the patient more "say so" in treatment selection by providing a variety of treatments in a smorgasbord fashion. Abstinent and nonabstinent patients following treatment are contrasted on psychometric and demographic variables.

Shared decision making in Eosinophilic esophagitis: Integrating physician and patient perspectives.

BACKGROUND AND OBJECTIVES: Shared decision making (SDM) is a patient-centered approach for conditions where multiple, preference-sensitive treatment options exist and there is no single best choice. Eosinophilic esophagitis (EoE), an increasingly prevalent chronic immune-mediated disease, offers an example of how patients and physicians use SDM to navigate the challenging tradeoffs of weighing pharmacologic and dietary therapies. We aimed to identify communication challenges for SDM in EoE care from physician and patient perspectives and to explore how patient treatment preference archetypes i

Living in a Patient-Centric Universe.

Patients with head and neck cancer face a number of challenges in terms of treatment, cure of their underlying malignant condition, and quality of survivorship. This presentation will focus on the patient perception of both the quality of care and the empathy and availability of the head and neck oncology team. It has become evident that the quality of survivorship plays a central role in the patient's decision-making process. The process of decision making by the patient facing head and neck cancer and the impact on treatment choices will be explored. The ability of the patient and her or his

Patient engagement for quality improvement and knowledge translation in paediatric orthopaedic care - a patient partner led initiative.

BACKGROUND: In healthcare, a shift towards patient-centric models has gained traction, emphasizing patient empowerment. This is exemplified by initiatives such as the Canadian Institutes of Health Research (CIHR) Strategy for Patient-Oriented Research (SPOR), which aims to foster a more sustainable and equitable healthcare system, as well as the increased support provided by the Patient-Centered Outcomes Research Institute (PCORI), which supports research that provides useful and reliable healthcare information to users. Despite progress in various healthcare domains, patient-engagement initia

Expanding the scope of shared decision-making in vascular access planning for hemodialysis: a case for interprofessional collaboration.

Vascular access planning guidelines have shifted from a fistula-centered to a patient-centered approach. We advocate for expansion of the shared decision-making dyad between the nephrology team and patient to include other vascular access planning stakeholders. We propose earlier and consistent interprofessional collaboration facilitates more constructive discussions that could improve outcomes.

Patient Values, Preferences, and Expectations for Managing Symptoms Due to Benign Prostate Obstruction.

An exploration of a patient's values and preferences is an essential component of the shared decision-making (SDM) process. For the management of symptomatic benign prostate enlargement, for which there are numerous treatment options that differ in their risks, benefits, and trade-offs, it is important to empower patients so that they are able to play an active role in decisions on their treatment. The goal is to ensure that choices reflect an individual's needs, desires, and values, thereby promoting satisfaction and minimising later regret. Preferences for the management of male lower urinar

Enhancing interprofessionalism in shared decision-making training within homecare settings: a short report.

Training in shared decision-making (SDM) often focuses solely on dyadic relationships between one healthcare provider and one patient. However, many healthcare decisions often involve two or more health professionals. These decisions warrant utilizing an interprofessional shared decision-making (IP-SDM) approach which enables patients and their caregivers to face difficult decisions around care together. Most existing SDM training programs fall short when building interprofessional (IP) competencies and require an approach that integrates IP with SDM. This short report discusses the creation a

Beneficial Effects of Two Types of Personal Health Record Services Connected With Electronic Medical Records Within the Hospital Setting.

Healthcare consumers must be able to make decisions based on accurate health information. To assist with this, we designed and developed an integrated system connected with electronic medical records in hospitals to ensure delivery of accurate health information. The system-called the Consumer-centered Open Personal Health Record platform-is composed of two services: a portal for users with any disease and a mobile application for users with cleft lip/palate. To assess the benefits of these services, we used a quasi-experimental, pretest-posttest design, assigning participants to the portal (n

Predictors and Variation in Cardiac Rehabilitation Participation After Transcatheter Aortic Valve Replacement.

BACKGROUND: Cardiac rehabilitation (CR) is strongly recommended for a spectrum of cardiovascular conditions and procedures including aortic valve replacement. OBJECTIVES: The purpose of this study was to characterize patient and hospital factors associated with CR participation after transcatheter aortic valve replacement (TAVR) and determine which factors explain hospital-level variation in CR participation. METHODS: We linked clinical and administrative claims data from patients who underwent TAVR at 24 Michigan hospitals between January 1, 2016 and June 30, 2020 and obtained rates of CR e

An international cross-sectional study of dementia researchers' own perspectives on patient and public involvement.

Patient and Public Involvement (PPI) can enhance the quality and relevance of health research. However, its implementation remains uneven across regions and research fields. This study explores how dementia and aging researchers perceive PPI's impact on their work. A cross-sectional survey was distributed to 392 researchers in Europe, Latin America, the USA, and Canada. Following multiple expert reviews, native speaker input, and pilot testing, the final survey was administered. Quantitative data were analyzed using descriptive statistics, group comparisons, and logistic regression models. Of

Standardized metrics can reveal region-specific opportunities in community engagement to aid recruitment in HIV prevention trials.

Good Participatory Practice (GPP) guidelines support and direct community engagement practices in biomedical HIV prevention trials, however no standardized metrics define the implementation and evaluation of these practices. Collaboratively, the Community Program staff of the HIV Vaccine Trials Network (HVTN) and the HIV Prevention Trials Network (HPTN) created a metric to describe, monitor, and evaluate one component of GPP, recruitment practices, in two HIV monoclonal Antibody Mediated Prevention (AMP) clinical trials, HVTN 703/HPTN 081 and HVTN 704/HPTN 085. From participatory design to clinical routine: new concepts for visualizing patient-reported outcomes in breast cancer care.

BACKGROUND: Patient-reported outcomes (PROs) improve cancer care by enabling structured self-assessment of disease-related symptoms, overall functioning, and health-related quality of life. Despite proven benefits, routine use is still limited, in part due to suboptimal data visualization. METHODOLOGY: In a sub-study of the multicenter PRO B trial (52 centers, 924 patients with metastatic breast cancer), an interdisciplinary team of clinicians, researchers, patient representatives, and service designers co-developed user-centered PRO visualizations. Drawing on a literature review, an analysis
