POEMS (polyneuropathy, organomegaly [hepatosplenomegaly or lymphadenopathy], endocrinopathy, M protein, and skin changes) syndrome is an uncommon plasma cell dyscrasia with diverse manifestations, including lymphadenopathy as well as those that comprise the acronym. Dermatologic changes may include hyperpigmentation, thickened skin, hypertrichosis, and papular angiomas. A 64-year-old woman had an 11-month history of a highly inflamed linear eruption that affected the left arm, including the dorsal aspect of the hand. It was characterized histologically by an intense plasma cell infiltrate extending throughout the dermis and subcutaneous fat. During the subsequent 2 years, the area evolved into a linear sclerotic plaque in keeping with the diagnosis of linear scleroderma. This case represents a clinical and pathologic presentation of linear scleroderma not previously emphasized in the literature.

A patient with IgA lambda multiple myeloma (MM) developed plasma cell leukemia (PCL), presenting as oculomotor nerve palsy. The cerebrospinal fluid (CSF) contained plasma cells, which double stained with fluoresceinated anti-IgA and anti-lambda antisera. The palsy was most probably due to meningeal myelomatosis. The neurologic disorder appeared to be refractory to the therapy used, although plasma cells disappeared from the peripheral blood. Selenium concentrations have been measured in plasma and in blood leucocytes from 29 haemodialysis patients and from 25 healthy men. The selenium contents of the plasma and white blood cells of the dialysis patients were significantly reduced (p less than 0.001). Selenium deficiency in humans results in a congestive cardiomyopathy and is associated with increased risks of accelerated atherosclerosis and cancer. Each of these is found with abnormal frequency in haemodialysis patients.

A 57-year-old man was evaluated for infiltrative sclerotic nodules and plaques of the skin, the oral cavity, and the eye. Skin biopsy revealed a proliferation of mature histiocytes and extensive hemosiderin deposition, findings most consistent with xanthoma disseminatum and its variant, disseminated xanthosiderohistiocytosis. The subsequent development of a plasma cell dyscrasia was consistent with previous reports and suggests that xanthoma disseminatum is a marker for plasma cell disorders.

The acid phosphatase activity has been studied cytochemically in bone marrow plasma cells of 12 patients with multiple myeloma and 15 with non-myeloma plasmocytosis. The acid phosphatase activity has been significantly higher in the myeloma group. The utilization and usefulness of this cytochemical reaction for the diagnosis of multiple myeloma are proposed.

We have demonstrated that patients with sickle cell disease greater than 2 years of age have a marked decrease in their plasma PGI2 or prostacyclin regenerating ability (0.05 +/- 0.08 ng/mg of vascular tissue) when compared to normal controls (0.44 +/- 0.12). This abnormality was not present in other chronic haemolytic states, and was not related to concomitant hyposplenism. 11 patients with plasma cell leukaemia (PCL) are reported. We examined the phospholipid composition of the plasma, of whole erythrocytes, and of "ghosts" of the erythrocytes from healthy volunteers, patients with sickle cell disease without crisis, and such patients in crisis, and found that phosphatidylglycerol in the plasma and "ghosts" was very significantly increased in sickle cell crisis but not in the absence of crisis. The significance of these findings is discussed.

A 73-year-old man developed conjunctivitis, epiphora, pancytopenia, dermatitis, and a polyclonal gammopathy. Diffuse infiltration and thickening of the conjunctiva and lacrimal canaliculi by plasma cells was present. Histopathologic examination suggested a reactive, chronic inflammatory response rather than neoplasia. The patient's ocular manifestations appeared to be related to his pancytopenia, dermatitis, and polyclonal gammopathy.
