A multicentre case-control study about effectiveness of pneumococcal vaccination against pneumonia hospitalization in at-risk adults.

BACKGROUND: Community-acquired pneumonia (CAP) is a leading cause of morbidity in adults with chronic conditions. Although pneumococcal vaccine effectiveness (VE) has been demonstrated in older adults, evidence is scant regarding adults < 65 years. We evaluated pneumococcal VE against CAP-related hospitalization in at-risk adults aged 18–64 years. METHODS: We conducted a multicentre case-control study in Spain (2020–2024). Cases were adults aged 18–64 years with risk conditions, hospitalized for ≥ 24 h, with radiological and clinical patterns compatible with CAP. Controls were hospitalized pat

Scalable production and characterization of recombinant pneumococcal histidine triad protein D in Escherichia coli for vaccine development.

Streptococcus pneumoniae causes 1.5 million deaths annually, primarily in low-income countries. Pneumococcal histidine triad protein D (PhtD), a conserved 92-110 kDa surface protein, is a promising serotype-independent vaccine candidate due to its roles in adherence, zinc homeostasis, and complement evasion. This study optimized scalable production of His-tagged PhtD in Escherichia coli BL21(DE3). The PhtD gene (serotype 2, strain D39) was cloned into pET30a(+), and expression was optimized using a full factorial design of experiments, achieving up to 19.8 mg/L soluble PhtD (>90 % purity) with

Projected epidemiological impact of lowering the adult pneumococcal vaccination age from 60 to 50 years in Germany.

The epidemiological impact of lowering the vaccination age of German adults from 60 to 50 y was evaluated. An age- and serotype-specific dynamic transmission model was used to evaluate the impact of vaccination with either the 23-valent pneumococcal polysaccharide vaccine (PPSV23) or the 20-valent or 21-valent pneumococcal conjugate vaccines (PCV20 and V116, respectively) on pneumococcal disease incidence, including invasive pneumococcal disease (IPD) and inpatient and outpatient non-bacteremic pneumococcal pneumonia, over 10 y. The base case (PPSV23 of ages ≥60 at a 30% vaccination coverage r

The impact of pneumococcal vaccination and nasopharyngeal colonization on the performance of a serotype-specific urine antigen detection (SSUAD) assay.

To better inform pneumococcal immunization policies, ongoing surveillance for pneumococcal community-acquired pneumonia (CAP) is crucial. To estimate the serotype-specific CAP burden of pneumococcal disease following the introduction of a new 15-valent pneumococcal conjugate vaccine (PCV), V114, a 15-plex serotype-specific urine antigen detection (SSUAD) assay was developed as a tool for surveillance of Streptococcuspneumoniae serotypes. V114-017 (NCT03547167; EudraCT 2017-004915-38) was a phase 3 randomized controlled trial in which participants (18-49 years) received V114 or 13-valent PCV (P

Effectiveness of pneumococcal vaccination in adults with common immune-mediated inflammatory diseases in the UK: a case-control study.

BACKGROUND: People with immune-mediated inflammatory disease are at increased risk of pneumococcal pneumonia. The effectiveness of pneumococcal vaccination in people with immune-mediated inflammatory diseases has not been evaluated. We investigated the effectiveness of pneumococcal vaccination in preventing morbidity and mortality associated with pneumonia in patients with immune-mediated inflammatory diseases. METHODS: In this matched case-control study, we used primary-care electronic health record data from the Clinical Practice Research Datalink Gold database in the UK, with linked hospit

PCV13 vaccine prevents pneumococcal biofilms without affecting Staphylococcus aureus population within the polymicrobial biofilm.

In respiratory pathogens such as Streptococcus pneumoniae, biofilm formation is associated with the colonization of the nasopharynx and chronic respiratory infection. Previous data have shown that pneumococcal conjugate vaccines (PCVs) had an impact on S. pneumoniae colonization and a potential replacement by other respiratory pathogens such as Staphylococcus aureus. The objective of this work was to evaluate the evasion of the immune system by monospecific biofilms and by S. aureus-S. pneumoniae mixed biofilms. We performed opsonophagocytosis assays (OPA) using human HL-60 against previously

Cost-Effectiveness Analysis of Routine Use of 15-Valent Pneumococcal Conjugate Vaccine in the US Pediatric Population.

This study evaluated the clinical and economic impact of routine pediatric vaccination with the 15-valent pneumococcal conjugate vaccine (PCV15, V114) compared with the 13-valent PCV (PCV13) from a societal perspective in the United States (US). A Markov decision-analytic model was constructed to estimate the outcomes for the entire US population over a 100-year time horizon. The model estimated the impact of V114 versus PCV13 on pneumococcal disease (PD) incidence, post meningitis sequalae, and deaths, taking herd immunity effects into account. V114 effectiveness was extrapolated from the obs

Oral Self-Care, Pneumococcal Vaccination, and Pneumonia Among Japanese Older People, Assessed With Machine Learning.

BACKGROUND: Pneumonia is a leading cause of death worldwide. It is a particularly serious burden in older people, as they tend to have a weakened immune response. Identifying the role of oral self-care and pneumococcal vaccination in healthy, independent older people can aid pneumonia prevention among them. This study investigated the associations between oral self-care, pneumococcal vaccination, and pneumonia experience among independent older people. METHODS: This cross-sectional study used data from the 2016 Japan Gerontological Evaluation Study. We used machine learning to examine the ass

23-valent pneumococcal polysaccharide vaccine and the risk of renal progression in older patients with chronic kidney disease.

BACKGROUND: The 23-valent pneumococcal polysaccharide vaccine (PPSV23) ensures favorable outcomes and reduces the risk of cardiac events in patients on dialysis. However, the effect of PPSV23 vaccination on renal function remains unknown, particularly in patients with chronic kidney disease (CKD). Therefore, we investigated the association between PPSV23 efficacy and renal progression in older patients (age ≥ 75 years) with CKD. METHODS: This multicenter, longitudinal cohort study was conducted using data (2008-2016) from the Epidemiology and Risk Factors Surveillance of CKD database. This da

Development and qualification of an automated capillary Western method for the identification of polysaccharide serotypes in pneumococcal conjugate vaccine (PCV).

Streptococcus pneumoniae bacterial infection causes mortality in both adults and infants. To mitigate the impact of the disease, several Pneumococcal conjugate vaccines (PCVs) have been manufactured for the U.S. market, including the recent approval of the 15-valent PCV Vaxneuvance™ from MSD. These vaccines demonstrate high efficacy for both the adult and pediatric dose. These PCVs contain multiple unique serotypes in the final, formulated vaccine product, and identifying a specific polysaccharide, in the presence of other serotypes, is a critical quality attribute that must be demonstrated th
