Postcoital vaginal rupture or tear is a well-known entity to the gynecologist, albeit unusual; however, such cases are rarely encountered by the general surgeon. The index case is reported to highlight the rare situation wherein a middle-aged woman underwent laparotomy for a suspected small bowel perforation, which revealed a vaginal tear as the cause of pneumoperitoneum. This case emphasizes the importance of taking a gynecological history and performing a gynecological examination when the clinical diagnosis is uncertain.

Thermal ablative techniques are used increasingly to treat renal cell carcinoma (RCC). Percutaneous cryoablation of tumors at the upper pole of the kidney may result in pulmonary damage due to the intervening lung parenchyma. We treated two patients with RCC in the upper pole of the kidney by inducing pneumothorax with a pneumoperitoneum needle before proceeding to percutaneous cryoablation. The procedures, performed under computed tomography (CT) fluoroscopy guidance, resulted in complete tumor necrosis. There was no pulmonary damage.

A pneumoscrotum due to long-term ventilation is a rare entity. We present the case of a 59-year-old man with adult respiratory distress syndrome (ARDS) and long-term ventilation, who required intensive care and had severe subcutaneous emphysema and a pneumoscrotum. Computed tomography revealed subcutaneous emphysema of the neck, chest, and abdomen, emphysema of the mediastinum, a pneumoperitoneum, pneumoretroperitoneum, and pneumoperineum. A puncture of the scrotum confirmed the diagnosis of a pneumoscrotum. The patient died in the course of progressive ARDS.

Spontaneous intestinal intramural haematomas are uncommon in the absence of trauma and may occur in those on anticoagulation. A young girl presented with generalised abdominal pain and was found to have pneumoperitoneum. She underwent an exploratory laparotomy and was found to have multiple ileal intramural haematomata with an area of possible perforation with a collection of pus. We believe that this is the first reported case of a rupture of spontaneous intestinal intramural haematoma in a young girl with no history of anticoagulation use or coagulation disorders.

Pneumatosis cystoides intestinalis is generally benign in course and sometimes, if cysts ruptured, behave as the not uncommon cause of free air in acute abdomen. In our case, we illustrate ruptured isolated cysts of pneumatosis cystoides intestinalis are responsible for pneumoperitoneum in a 94-year-old male patient. Laparotomy with gastrotomy for decompression of intraluminal aeropressure was performed, with an uneventful recovery. This paper presents with preoperative and intraoperative images of high educational value for this, often underdiagnosed, clinical entity.

Rupture of the oesophagus or stomach at the time of cardiopulmonary resuscitation can occur with accidental oesophageal intubation. The common site of rupture is the lesser curvature of the stomach, but can also occur at the oesophagogastric junction. The patient presented with a massive pneumoperitoneum after an out of hospital ventricular fibrillation arrest. CT scanning was helpful in making the diagnosis. In out of hospital resuscitation, current JRCALC (Joint Royal Colleges Ambulance Liaison Committee) recommendations may not avoid this complication.

Anesthesiologists currently view laparoscopic cholecystectomy resemblant to other laparoscopic procedures with respect to the necessity of inducing a pneumoperitoneum via abdominal insufflation of carbon dioxide (CO2). The present case report describes a healthy 63-year-old man who while undergoing elective laparoscopic cholecystectomy under general anesthesia, developed hypoxemia in the course in consequence of pneumothorax. This complication, although rare, can be catastrophic if prompt diagnosis and rapid intervention and management do not come in the nick of time.

After laparoscopic appendectomy complications and injury to the intraabdominal organs and major vessels are rare but well documented. The majority of complications occur during establishment of pneumoperitoneum. Frequently used entries are open trocar (Hasson), closed using Veress needle and direct trocar insertion. In this case we describe an incident where the towel clip, used to get hold of the supraumbilical fascia to minimize the risk of injury, was pressed down too hard. The retroperitoneum was accidentally punctured and a haematoma above vena cava was created.

A 27-year-old woman sustained a trauma to her perineal area when she was ejected from a jet ski while riding on water at high speed. The patient presented to the emergency department with blood streaking from her anal canal. Imaging revealed pneumoperitoneum. Surgical intervention showed complex anal canal and rectal injuries. Primary repair of the injuries was performed. Postoperatively the patient did well and was followed up with no evidence of residual symptoms and with a continent anal sphincter.
