"Jail isn't the answer for these inmates": how to respond to methamphetamine use in prisons.

PURPOSE: The correctional system continues to face challenges with responding to and managing methamphetamine use among incarcerated individuals. This study aims to uncover what resources and policies could better help correctional workers deal with these challenges. The authors also examined methamphetamine's impact on correctional work and staff well-being. DESIGN/METHODOLOGY/APPROACH: An online survey was distributed to correctional workers (n = 269) in Manitoba, Canada, featuring questions about their experiences related to methamphetamine use in populations under their care, what support

Tuberculosis in Prisons: Importance of Considering the Clustering in the Analysis of Cross-Sectional Studies.

The level of clustering and the adjustment by cluster-robust standard errors have yet to be widely considered and reported in cross-sectional studies of tuberculosis (TB) in prisons. In two cross-sectional studies of people deprived of liberty (PDL) in Medellin, we evaluated the impact of adjustment versus failure to adjust by clustering on prevalence ratio (PR) and 95% confidence interval (CI). We used log-binomial regression, Poisson regression, generalized estimating equations (GEE), and mixed-effects regression models. We used cluster-robust standard errors and bias-corrected standard erro…

The impact of COVID-19 vaccination in prisons in England and Wales: a metapopulation model.

BACKGROUND: High incidence of cases and deaths due to coronavirus disease 2019 (COVID-19) have been reported in prisons worldwide. This study aimed to evaluate the impact of different COVID-19 vaccination strategies in epidemiologically semi-enclosed settings such as prisons, where staff interact regularly with those incarcerated and the wider community. METHODS: We used a metapopulation transmission-dynamic model of a local prison in England and Wales. Two-dose vaccination strategies included no vaccination, vaccination of all individuals who are incarcerated and/or staff, and an age-based a…

COVID-19 vaccine for people who live and work in prisons worldwide: A scoping review.

Overcrowding, poor conditions, and high population turnover make prisons highly susceptible to COVID-19. Vaccination is key to controlling COVID-19, yet there is disagreement regarding whether people who live and work in prisons should be prioritised in national vaccination programmes. To help resolve this, we critically examine the extent, nature, and quality of extant literature regarding prioritisation of COVID-19 vaccinations for people who live and work in prisons. Using a scoping review as our methodological framework, we conducted a systematic literature search of 17 databases. From 2,3…

A cautionary tale for health education initiatives in vulnerable populations: Improving nutrition in Haiti prisons.

INTRODUCTION: Prisons in low-income countries have barriers to providing adequate nutrition to the incarcerated. This perspective discusses a quality improvement program with health education to improve nutrition provided to men in two prisons in Haiti. METHODS: Incarcerated men in the National Penitentiary in Port Au Prince and the prison in Mirebalais were the focus of the program. A culturally competent educational intervention was delivered to the prison cooks. Program evaluation included a baseline and a follow-up assessment in 2021 and 2022 in both prisons. Calories, body composition, a…

The Model for Understanding Inpatient Aggression: A Version for Mental Health Nurses Working in Prisons.

Preventing and managing aggression remains an important and challenging task for mental health nurses. Despite the concern, there is a dearth of frameworks to assist practice and inform assessment and intervention related to aggression, for forensic mental health nurses working in prisons. This paper presents a model for understanding aggression within prison mental health units. The model elucidates various personal and situational determinants of aggression. It offers a systematic framework for mental health nurses to investigate and understand aggression, to ultimately assist nurses to prov…

Injecting substance use in prisons in the United States: a case for needle exchange programs.

Despite efforts to prevent illicit substance use in prisons, inmates in many countries still have access to them, along with associated paraphernalia. In the United States, a large proportion of people in prison are there due to drug-related charges. The secretive nature of illicit substance use in prison and the opposition to implementing harm reduction interventions such as needle exchange programs have put prisoners who engage in such practices at high risk for infectious bloodborne diseases. While states have their reasons for opposing the implementation of needle exchange programs in pris…

'Keeping a lid on it': Exploring 'problematisations' of prescribed medication in prisons in the UK.

BACKGROUND: The non-medical use of prescription medication and risk of diversion have become policy and practice concerns within prison settings in the UK. These issues have been highlighted by the Advisory Council on the Misuse of Drugs, Her Majesty's Inspectorate of Prisons and Her Majesty's Prison and Probation Service (2019) prison drugs strategy. In 2019, new prescribing guidance was issued by the Royal College of General Practitioners for clinicians working within prison settings. METHODS: Informed by Bacchi's (2009) What's the problem represented to be? framework, the ways in which the…

Disparities in hepatitis C care across Canadian provincial prisons: Implications for hepatitis C micro-elimination.

BACKGROUND: Delivery of hepatitis C virus (HCV) care to people in prison is essential to HCV elimination. We aimed to describe current HCV care practices across Canada's adult provincial prisons. METHODS: One representative per provincial prison health care team (except Ontario) was invited to participate in a web-based survey from January to June 2020. The outcomes of interest were HCV screening and treatment, treatment restrictions, and harm reduction services. The government ministry responsible for health care was determined. Non-nominal data were aggregated by province and ministry; desc…

A cost-effectiveness study of tuberculosis and latent tuberculosis infection screening in prisons in Japan.

BACKGROUND: Prison inmates are considered a high-risk population for tuberculosis (TB) in Japan. However, they are currently only screened for active TB using chest X-ray (CXR) as part of an annual health check.OBJECTIVE: To evaluate the cost-effectiveness of screening for latent TB infection (LTBI) using interferon-gamma release assay (IGRA) on entry into prison institutions.METHOD: We developed a transmission model of TB, taking into consideration the TB and LTBI epidemiology among inmates in Japan. Using a decision tree, we compared and evaluated no screening, screening using CXR, and IGRA

Availability, Accessibility, and Coverage of Needle and Syringe Programs in Prisons in the European Union.

Needle and syringe programs (NSPs) are among the most effective interventions for controlling the transmission of infection among people who inject drugs in prisons. We evaluated the availability, accessibility, and coverage of NSPs in prisons in European Union (EU) countries. In line with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) criteria, we systematically searched 4 databases of peer-reviewed publications (MEDLINE (PubMed), ISI Web of Science, EBSCO, and ScienceDirect) and 53 databases containing gray literature to collect data published from January 20…

Challenges and opportunities for the management of infectious diseases in Europes' prisons: evidence-based guidance.

People in prison have multiple complex health and social care needs. These are likely to be the result of a combination of overlapping, and sometimes interlinked, risk factors for infection, ill-health, and incarceration, such as problem drug use. Incarceration can represent a unique opportunity to make high-quality health care available to people in prison and to target socially deprived groups who are often medically underserved when living in the community they originate from. In recent years, international and European institutions have increasingly acknowledged the importance of treating

Food refusal as a protest tool. Hunger strikes in Belgian prisons during the interwar period.

In studies on resistance and protest, empirical research tends to concentrate on collective forms of action, those organised within social movements or by resistance groups. As such, research into the protest tactic of hunger strikes suffers from the same ailment. While there is ample historical study into hunger strikes by the suffragettes or the IRA, the actions of individuals without any connection to political movements have mostly been left unnoticed - although they tended to outnumber the former, especially in prisons where hunger strikes are in a way a measure of last resort. This paper…

High rate of hepatitis C reinfection following antiviral treatment in the North East England Prisons.

To achieve elimination of hepatitis C (HCV), a critical group to prioritise for diagnosis and treatment is the prison population, where HCV prevalence is high. A universal offer of blood-borne virus testing (UOBBVT) programme and a new treatment pathway were introduced to seven North East England (NEE) Prisons. Our aim was to assess: (a) the proportion of individuals with active HCV commencing direct-acting antivirals (DAAs); (b) the outcomes following DAA treatment; (3) the reinfection rate following sustained virological response (SVR). Data were collected prospectively on BBVT uptake, HCV p…

Poor continuity of care for TB diagnosis and treatment in Zambian Prisons: a situation analysis.

OBJECTIVES: Prisons act as infectious disease reservoirs. We aimed to explore the challenges of TB control and continuity of care in prisons in Zambia. METHODS: We evaluated treatment outcomes for a cohort of inmates diagnosed with TB during a TB REACH funded screening programme initiated by the Zambia Prisons Service and the Centre for Infectious Disease Research in Zambia. RESULTS: Between October 2010 and September 2011, 6282 inmates from six prisons were screened for TB, of whom 374 (6.0%) were diagnosed. TB treatment was initiated in 345 of 374 (92%) inmates. Of those, 66% were cured or…
