Imaging of acute lower limb muscle injury and potential gender differences.

OBJECTIVES: To assess the most common lower limb acute muscle injuries on MRI imaging in a national specialist centre for orthopaedics and sports medicine and to explore potential gender differences. METHODS: Over a 3-year time period, all MRI lower limb studies with acute muscle injury (AMI) were reviewed. A British Athletics muscle injury classification (BAMIC) was given and a statistical analysis was performed. RESULTS: A total of 195 AMIs were diagnosed: 177 (91%) male and 18 (9%) female injuries (M to F = 9.8:1). The most common lower limb AMIs were BAMIC grade 1a injuries (n = 48, 25%)

[Electromyographic diagnosis of root disorders and localization of lumbar disk herniations].

Electromyographic examination of the quadriceps muscle of the thigh, the anterior tibial muscle, the long peroneal muscle, and the medial head of the gastrocnemius muscle was conducted in 80 patients with lumbar osteochondrosis. Electromyographic signs of abnormalities were detected in 66 (97.1%) of 68 patients with radicular disorders caused by Schmorl's body. According to the results of electromyography, the localization of Schmorl's body was determined correctly in relation to the length of the spleen in 90.6% of cases and in relation to the diameter of the intervertebral disk in 71.9%. Muscle atrophy after ACL reconstruction involves molecular mechanisms beyond unloading.

Anterior cruciate ligament reconstruction (ACLR) leads to profound muscle atrophy and weakness that remain resistant to rehabilitation. Although early recovery typically involves a brief period of limb unloading, the degree to which disuse alone accounts for muscle pathology after ACLR remains unclear. Here, we leveraged publicly available RNA-seq datasets of muscle biopsies from vastus lateralis obtained 7 days after ACLR or 10 days after unilateral lower limb suspension (ULLS), each with matched control limbs, to directly compare disuse-driven and ACLR-specific early transcriptional response

[A woman with chronic infection of the upper legs].

A 58-year-old woman with myositis ossificans traumatica came to our trauma outpatient clinic because of cutaneous perforation of one of the ossifications. This patient developed myositis ossificans traumatica after liposuction of both upper legs 30 years ago, which condition was complicated by a chronic infection of the ossified right quadriceps muscle, a walking limitation, and a loss of range of motion. Myositis ossificans traumatica is a rare disorder associated with trauma- triggered ossification of otherwise normal muscle tissue.

Extensor mechanism reconstruction with peroneus longus tendon autograft for neglected patellar fracture, report of 2 cases.

Extensor mechanism deficiency in the knee may occur due to neglected patellar and quadriceps tendons rupture or may be caused by chronic fractures of the patella. Older patients can tolerate nonunion with impaired function including extension limitation or persistent muscle weakness. In young patients, performing rigid internal fixation with reoperation should be considered when a nonunion occurs. However, delayed and neglected nonunion in patella fractures require performing different surgical procedures. We report two cases, operated for a patella fracture, in whom nonunion occurred and acco

[Simultaneous bilateral rupture of the quadriceps tendon].

Bilateral rupture of the quadriceps tendon is a rare injury. It usually occurs in obese older patients. Other predisposing factors are long-term chronic renal failure, gout, rheumatoid arthritis, diabetes mellitus, hyperparathyroidism and abuse of anabolic steroids. The most common cause of bilateral rupture is a sudden violent contraction of the quadriceps muscles with the knees semiflexed and the feet fixed. Examination reveals bilateral joint effusion, palpable or visible suprapatellar gaps, and an inability to extend both knees and lift the straight legs. Often the diagnosis is missed, and…

Quadriceps Strength Does Not Associate With Gait Adaptation Ability in Individuals With Anterior Cruciate Ligament Reconstruction.

CONTEXT: Postoperative muscle weakness contributes to the development of aberrant gait biomechanics that persist after traditional anterior cruciate ligament reconstruction (ACLR). However, it is unknown if quadriceps weakness impedes the ability of ACLR patients to modify gait biomechanics using a real-time gait biofeedback (RTGBF) intervention. OBJECTIVE: The purpose was to determine if quadriceps strength is associated with the ability to modify vertical ground reaction force (vGRF) during a RTGBF intervention. DESIGN: Cross-sectional study. SETTING: Research laboratory. PATIENTS OR OTH

Acute compartment syndrome from anterior thigh muscle contusion: a report of eight cases.

Eight patients with an anterior thigh muscle contusion or rupture developed an acute anterior compartment syndrome. All patients had an increased pressure in the quadriceps muscle, ranging between 41 and 80 mm Hg. At fasciotomy, three cases had a hematoma in the rectus femoris and four in the vastus intermedius, and in one case only, edema of the anterior compartment was found. Postoperatively, there was immediate pain relief in all cases, and after about 4 weeks, all patients had normal function. We recommend broad indications for fasciotomy and evacuation of hematoma after thigh muscle contu…

Skeletal muscle myosin is the autoantigen for experimental autoimmune myositis.

Experimental autoimmune myositis (EAM) is a rodent model for human inflammatory muscle disease (IMD). It can be induced by immunization of rodents with skeletal muscle homogenate and adjuvant. The specific myositogenic autoantigen has not been clearly identified although some evidence points to skeletal muscle myosin. In this report we strengthen this evidence, showing that Lewis rats immunized with purified skeletal muscle myosin develop EAM with the same pattern and severity as EAM induced by whole rabbit skeletal muscle homogenate (WRM). Multiple inflammatory lesions are detected histopatho

Posterior Tibial Slope Characteristics and Their Relationships With Thigh Muscle Morphology, Activation Patterns, and Dynamic Knee Valgus in Male and Female Soccer Players Before and After Fatigue: A Descriptive Laboratory Study.

BACKGROUND: Soccer players are at high risk of anterior cruciate ligament injury as well as reinjury, with higher rates in women and in the supporting leg, and fatigue impairs the neuromuscular control of the knee joint. However, little is known about the bony anatomy of the knee joint and its relationship with neuromuscular measures related to knee stabilization, and studies investigating anatomic and biomechanical measures simultaneously are widely lacking. PURPOSE: To investigate (1) whether posterior tibial slope (PTS) characteristics and thigh muscle morphology differ between sexes; (2)

Ocular fixation with quadriceps tendon allograft.

Management of difficult strabismus, such as strabismus fixus and paralytic strabismus, in order to maintain the alignment is complicated. There are many surgical approaches described in the current literature, together with notes on the materials used to stabilize these deformities. We present a new surgical approach using quadriceps tendon allograft for the correction of difficult strabismus such as strabismus fixus and paralytic strabismus. Our idea for using deep frozen quadriceps tendons developed from the fact that this is the most stable and strongest tendon in the whole body. Six patien

The other half of the sprinting coin: the neuromuscular profile of a 91-yr-old female world record sprinter.

A 91-yr-old female set the W90+ world record in the 200 m sprint in 2024. We characterized her neuromuscular function, fatigability, denervation markers, and single-fiber contractile properties, and, where possible, compared these outcomes with published reference data. Knee extensor muscle architecture, force, power, fatigability, and motor unit (MU) behavior were assessed in vivo, and a vastus lateralis biopsy was analyzed for single-fiber contractile properties and markers of denervation. Quadriceps cross-sectional area was 36.5 cm2, with fascicle length, pennation angle, and muscle thickne

A comparative analysis of autograft choices of anterior cruciate ligament reconstruction and their effects on muscle strength and joint biomechanics.

INTRODUCTION: Anterior cruciate ligament reconstruction (ACLR) is crucial to restore knee stability and function after ACL injuries, especially in physically active individuals. Despite advances in surgical techniques and rehabilitation protocols, the choice of autograft has a significant impact on postoperative recovery, particularly on muscle strength and joint biomechanics. In this study, the effects of four autografts are investigated: Iliotibial band (ITB), combined ITB and hamstring tendon (ITB + HT), hamstring tendon (HT) and bone-tendon-bone (BTB) on quadriceps and hamstring peak torqu
