Antimicrobial therapy for bronchiectasis.

Antibiotics have a role in the management of acute exacerbations of bronchiectasis and may also benefit selected subsets of patients with bronchiectasis as a part of a long-term maintenance strategy. At present, there are no Food and Drug Administration-approved antibiotics for acute or chronic management of bronchiectasis. Clinical trials are underway to determine the efficacy and safety of various inhaled antibiotics for chronic therapy for bronchiectasis. Until those results are available, clinicians need to tailor their therapies to individual patients based on their best clinical judgment

Meta-analysis of Effects of Nutritional Therapies on Nutritional Status of Gastric Cancer Patients.

This protocol provides a methodological framework for conducting a meta-analysis to evaluate the effects of nutritional therapies on the nutritional status of gastric cancer patients. By following this protocol, researchers will be able to systematically identify, appraise, and synthesize evidence from randomized controlled trials and comparative studies. The steps include: formulating a comprehensive search strategy across multiple databases; dual-independent screening and selection of studies using predefined PICOS criteria (population, interference, comparison, outcome, and study design); s

Acupuncture for smoking cessation: an overview of systematic reviews.

BACKGROUND: An increasing number of randomized controlled trials (RCTs) investigated various forms of acupuncture for smoking cessation; however, their findings remain inconsistent, and substantial controversy persists regarding both its efficacy and safety. OBJECTIVES: This overview aimed to synthesize the outcome evidence presented in existing systematic reviews (SRs) on acupuncture for smoking cessation, appraise the methodological quality of the included SRs, and re-evaluate the primary outcomes. METHODS: A comprehensive literature search was conducted in eight databases from inception t

Role of industry sponsorship and research outcomes of myopia control interventions.

AIMS: To investigate whether industry-sponsored randomised control trials (RCT) on myopia control have more favourable outcomes and differ in risk of bias, compared with studies having other sources of sponsorship. METHODS: PubMed, Embase and MEDLINE were searched until 6 October 2023. RCTs of myopia control interventions were included if they reported spherical equivalent (SE) and/or axial length (AL) changes. We pooled the mean differences and 95% CIs in SE and AL changes using a random-effects model. We calculated both the risk ratio (RR) and adjusted OR of having favourable research outco

A survey of experts to identify methods to detect problematic studies: stage 1 of the INveStigating ProblEmatic Clinical Trials in Systematic Reviews project.

BACKGROUND AND OBJECTIVE: Randomized controlled trials (RCTs) inform health-care decisions. Unfortunately, some published RCTs contain false data, and some appear to have been entirely fabricated. Systematic reviews are performed to identify and synthesize all RCTs which have been conducted on a given topic. This means that any of these 'problematic studies' are likely to be included, but there are no agreed methods for identifying them. The INveStigating ProblEmatic Clinical Trials in Systematic Reviews (INSPECT-SR) project is developing a tool to identify problematic RCTs in systematic revie

A process for prioritising systematic reviews in tinnitus.

Objective: To develop an innovative prioritisation process to identify topics for new or updated systematic reviews of tinnitus research.Design: A two-stage prioritisation process was devised. First, a scoping review assessed the amount of randomized controlled trial-level evidence available. This enabled development of selection criteria for future reviews, aided the design of template protocol and suggested the scale of work that would be required to conduct these reviews. Second, using the pre-defined primary and secondary criteria, interventions were prioritised for systematic review.Study

Efficacy and safety of hormonal treatment of cryptorchidism: current state of the art.

UNLABELLED: Meta-analyses of randomised trials using hCG or GnRH for treatment on testicular descent show in most studies overall efficacy of about 20%, less if retractile testes were excluded. In recent years a number of potentially serious side effects have been reported. CONCLUSION: Considering the efficacy and the possible side effects of the hormonal treatment, the general use of hCG and GnRH in the treatment of cryptorchidism cannot be further recommended.

[A survey on trials focussing on the treatment of cannabis-dependence].

Epidemiological studies show that the prevalence of cannabis use in Germany is approaching that of countries known for their high consumption rate, such as Australia and some parts of the USA. In these countries, the resulting cannabis-related psychosocial damage has led to mainly psychotherapeutic treatment studies (conducted in the past ten years) that will be introduced here. According to these studies, the optimal treatment of cannabis-dependent adults would be a short intervention which consists of a combination of motivational-enhancement and cognitive-behavioural elements as well as ind

Is neoadjuvant therapy necessary for resectable pancreatic cancer? A review of randomized controlled trials to date: a narrative review.

Perioperative systemic therapy, administered either preoperatively (neoadjuvant therapy) or postoperatively (adjuvant therapy), has been considered a strategy to improve the long-term prognosis of pancreatic cancer. While adjuvant therapy following resection has demonstrated a clear long-term prognostic benefit, the significance of neoadjuvant therapy remains uncertain. This manuscript reviews previously published randomized controlled trials and provides a scientific discussion on the value of neoadjuvant therapy. To date, eight phase II or phase III randomized controlled trials have been con
