[Electromyographic study of the striated urethral sphincter by using the bulbocavernosus reflex: study on change of sacral reflex activity caused by bladder filling].

PURPOSE: The change of sacral reflex activity of the striated urethral sphincter in the urine storage phase is investigated using evoked potential reaction of the bulbocavernosus reflex (BCR). METHODS: Eleven normal male subjects and 13 male patients with neurogenic bladder due to suprasacral (C3-C7) spinal cord injury (SCI patients) were investigated. Within the SCI patients, five were complete SCI and 8 were incomplete SCI. BCR was elicited by electrical stimulation of dorsal nerve of the penis, and the evoked potential of the BCR was recorded with a concentric needle electrode from the per…

A Robust Approach to Quantify Nocifensive Blink Reflex Responsiveness.

The modulation of the hand-blink reflex (HBR), a prototypical nocifensive response, is increasingly used to investigate defensive behaviour, related to the notion of peripersonal space. However, HBR responsiveness is highly variable across participants. This variability has led researchers to use several seemingly arbitrary criteria to determine whether a subject should be included as a participant in a study. But are these criteria justified? Can better and more rigorous criteria be formulated? Does the traditional division into responders and non-responders reflect a practical decision to ex…

Oculocardiac reflex during endoscopic brow lift: A case report.

BACKGROUND: The oculocardiac reflex (OCR) is a trigeminovagal reflex most commonly associated with ophthalmic surgery. Its occurrence in aesthetic facial procedures is rarely reported. CASE PRESENTATION: A 44-year-old woman with no underlying medical conditions underwent elective endoscopic brow lift under general anesthesia using volatile anesthetic agents. Baseline heart rate was 90 beats per minute. During traction near the right supraorbital rim, severe bradycardia occurred with heart rate dropping to 20 beats per minute. The event resolved after cessation of traction and administration o…

The sneezing reflex: neurophysiology, neuroimmune pathways and clinical disorders.

OBJECTIVE: To review the physiological mechanisms, neural and molecular mediators, and clinical pathologies associated with the sneezing reflex. METHODS: A comprehensive narrative review was conducted utilizing established databases (PubMed/Medline, Scopus, Web of Science, Embase), covering articles published up to 2025. A structured search strategy applied keywords such as "sneezing reflex," "neuromedin B (NMB)," "trigeminal nucleus," "allergic rhinitis," "chronic rhinosinusitis," and "neuromodulation." RESULTS: The sneeze reflex is a complex protective mechanism divided into an initial sen…

Vestibulo-ocular reflex suppression: A practical guide for neurologists.

Vestibulo-ocular reflex suppression (VORS) testing remains an underutilized yet useful bedside tool for differentiating central and peripheral vestibular disorders in patients with dizziness. While most clinicians focus on the head impulse test (HIT) for detecting vestibular hypofunction, assessing the VORS can reveal central dysfunction, by observing compensatory saccades when turning their head while simultaneously following a moving object. Modern clinical practice incorporates visual-interactive smartphone-based methods for VORS assessment. This sign does not have a topographical value per…

[Secretion management in neurological diseases and impaired cough reflex].

Insufficient cough in patients with neurological disorders leads to secretion stagnation in the airways, increasing the risk of recurrent and potentially life-threatening respiratory tract infections (RTIs). Respiratory failure, including RTI, is the leading cause of death among these patients. This review explains and systemises the interventions to manage secretion stagnation, and schematically presents a recommended course of action. Interventions should always consider the patient's co-morbidities. Due to sparse data, further research regarding selected interventions is needed.

The near reflex evaluation in diabetic patients without retinopathy.

PURPOSE: To evaluate accommodative amplitude (AA) and fusional vergence amplitudes (FVA) in type 2 diabetes mellitus (DM) patients without diabetic retinopathy (DR). MATERIALS AND METHODS: This study of 30 type 2 diabetic patients aged 35-50 years and 31 healthy subjects analyzed FVA, objective AA measurement and statistical evaluation. The study also evaluated the effects of mean HbA1c, age, and DM duration on AA. RESULTS: The groups were similar in age (P=0.535). There were statistically significant differences between the groups in terms of mean convergence amplitude at distance (P=0.003)

Importance of Preoperative Pupillary Reflex in Traumatic Optic Neuropathy.

OBJECTIVE: Traumatic optic neuropathy (TON) refers to a pathological condition caused by direct or indirect injury to the optic nerves. In the case of patients with traumatic brain injury, adequate vision evaluation is difficult in many cases due to altered mentality. In order to address this problem, we investigated preoperative pupillary light reflex in TON patients as a predictive factor of surgical outcomes after optic nerve decompression. METHODS: From April 2020 to September 2022, we enrolled patients who were diagnosed with TON and underwent endoscopic optic nerve decompression at our …

Neurophysiological Assessment of H-Reflex Alterations in Compressive Radiculopathy.

This study aimed to investigate changes in the H-reflex recruitment curve in compressive radiculopathy, specifically assessing differences between symptomatic and asymptomatic limbs in patients with unilateral S1 radiculopathy through derived parameters. A total of 24 volunteers (15 male and 9 female, aged between 22 and 60 years) with confirmed nerve root compression in the L5/S1 segment participated. Nerve root compression was verified through clinical MRI examination and attributed to disc protrusion, spinal canal stenosis, or isthmic spondylolisthesis of L5/S1. Analysis revealed no differe…

Intraoperative bulbocavernosus reflex monitoring in posterior lumbar fusion surgery.

OBJECTIVE: To assess the utility of intraoperative bulbocavernosus reflex (BCR) monitoring in posterior lumbar fusion surgery. METHODS: We retrospectively evaluated 153 patients undergoing posterior lumbar fusion with intraoperative BCR monitoring. Voiding function was assessed at discharge and two follow-ups. RESULTS: BCR was preserved in 151 patients and completely disappeared in two patients at the end of surgery. For patients in whom BCR was preserved, voiding difficulties at discharge and 1-month and 6-month follow-ups were noted in 16 (10.6%), 9 (6.0%), and 0 (0.0%) patients, respectiv…

Blink reflex monitoring in microvascular decompression for trigeminal neuralgia.

Aim: To determine the value of the blink reflex in evaluating trigeminal sensory function during microvascular decompression for trigeminal neuralgia. Methods: The blink reflex (BR) in 103 patients with primary typical trigeminal neuralgia treated by microvascular decompression (MVD) was tested pre- and intraoperatively. The changes in BR were recorded. All patients underwent general anesthesia with intravenous propofol and fentanyl. Surgical efficacy and complications were evaluated after surgery. The relationship between intraoperative changes in the BR and postoperative trigeminal sensory fu…
