Phenytoin for motion sickness: clinical evaluation.

Phenytoin has previously been shown to protect against motion sickness induced by Coriolis stimulation. The purpose of our series of investigations was to investigate further the efficacy of phenytoin for motion sickness prophylaxis and to gain insight into its mechanism of action. We tested participants with electronystagmography, off-vertical rotation, sea travel, and parabolic flight after they received phenytoin or placebo. Blood levels of at least 9 micrograms/mL were found to protect against motion sickness. Electronystagmography showed significant decreases in the gain of the vestibuloo

Value of otoneurological tests in the staging of multiple sclerosis.

We looked for correlations between otoneurological findings and neurological signs in multiple sclerosis (MS). 41 patients were tested for: spontaneous and caloric nystagmus, horizontal pursuit eye tracking, full-field optokinetic nystagmus and eye movements. The findings were compared with the course of the disease (progressive or remitting), age at onset of MS and disability, in order to test the predictive value of this type of test. The correlation between otoneurological abnormalities and lesion site is very high for brainstem lesions. Subclinical brainstem lesions were detected in 66% of

Differentiating the Structural and Functional Instability of the Craniocervical Junction.

This paper presents the anatomical and biomechanical aspects of chronic instability of the craniocervical junction (CCJ) with a discussion on clinical diagnostics based on mobility tests and provocative tests related to ligamentous system injuries, as well as radiological criteria for CCJ instability. In addition to the structural instability of the CCJ, the hypothesis of its functional form resulting from cervical proprioceptive system (CPS) damage is discussed. Clinical and neurophysiological studies have shown that functional disorders or organic changes in the CPS cause symptoms similar to

Central paroxysmal positional nystagmus: Characteristics and possible mechanisms.

OBJECTIVE: The diagnosis of central paroxysmal positional nystagmus (CPPN) is challenging, and the mechanisms require further elucidation. This study aimed to determine the characteristics and mechanisms of CPPN. METHODS: Seventeen patients with CPPN were subjected to analyses of their clinical findings, MRI lesions, and oculographic data on spontaneous and positional nystagmus. RESULTS: The direction of CPPN was mostly aligned with that of the head motion during the positioning, and 3 types of CPPN were identified: downbeat nystagmus on straight-head hanging, upbeat nystagmus on uprighting,

Classification of human rotation test results using parametric modeling and multivariate statistics.

The usefulness of vestibular testing is directly related to the accuracy of the test interpretations. Two factors, subjective analysis of large test data sets and failure to make appropriate age corrections, tend to reduce test accuracy. Correction of these problems can be accomplished by application of physiologically based models of vestibular function and multivariate classification techniques to the test data, thereby creating a more objective test interpretation procedure. Herein we report our results on the use of this strategy for analysis of sinusoidal harmonic acceleration (SHA) test

Eye- and head movements in freely moving rabbits.

1. Eye- and head movements were recorded in unrestrained, spontaneously behaving rabbits with a new technique, based upon phase detection of signals induced in implanted coils by a rotating magnetic field. 2. Movements of the eye in space were exclusively saccadic. In the intersaccadic intervals the eyes were stabilized in space, even during vigorous head movements. Most of this stability was maintained in darkness, except for the occurrence of slow drift. 3. Many saccades were initiated while the head was stationary. They were accompanied by a similar, but slower head rotation with approximat

[A course of early Meniere's disease].

Clinical manifestations are described and examination data are provided for 120 patients with Meniere's disease (MD) running for one year and less (mean 5.2 months). The patients' age varied from 15 to 66 years (mean 39.8 years). Unilateral MD was diagnosed in 95% patients, cochlear MD - in 58.3%, classic one - in the rest patients. Minor or moderate hypoacusis was found in 85% examinees. Mean 18.6 dB bone-air interval at low frequency was detected in 75% patients. Vestibular reflex was reduced insignificantly in 52.5% patients, 45% patients had hypofunction of the third-fourth degree. Early M

Age-related High-frequency Hearing Loss Is Not Associated With Horizontal Semicircular Canal Function.

OBJECTIVE: Investigate the association between age-related hearing loss and reduced peripheral vestibular function using paired assessments of high-frequency hearing and horizontal semicircular canal (HSC) function. We hypothesized that age-related high-frequency hearing loss would be correlated with reduced HSC function and, therefore, useful to predict age-related vestibular hypofunction. DESIGN: We conducted a single center, retrospective cross-sectional study in a tertiary/academic referral hospital. This study included 185 patients who were diagnosed with a cerebellopontine angle (CPA) t

Ethovision™ analysis of open field behaviour in rats following bilateral vestibular loss.

Bilateral vestibular loss (BVL) causes a unique behavioural syndrome in rodents, with symptoms such as locomotor hyperactivity and changes in exploratory behaviour. Many of these symptoms appear to be indirect consequences of the loss of vestibular reflex function and are difficult to explain. Although such symptoms have been reported before, there have been few systematic studies of the effects of BVL using automated digital tracking systems in which many behavioural symptoms can be measured simultaneously with high precision. In this study, data were obtained from rats with BVL induced by in

Asymmetries and three-dimensional features of vestibular cross-coupled stimuli illuminated through modeling.

Head movements during sustained rotation can cause angular cross-coupling which leads to tumbling illusions. Even though angular vectors predict equal magnitude illusions for head movements in opposite directions, the magnitudes of the illusions are often surprisingly asymmetric, such as during leftward versus rightward yaw while horizontal in a centrifuge. This paper presents a comprehensive investigation of the angular-linear stimulus combinations from eight different published papers in which asymmetries were found. Interactions between all angular and linear vectors, including gravity, are

Predictive properties of the video head impulse test: measures of caloric symmetry and self-report dizziness handicap.

OBJECTIVES: The purpose of this investigation was to determine whether a predictable relationship existed between self-reported dizziness handicap and video Head Impulse Test (vHIT) results in a large sample of patients reporting to a dizziness clinic. Secondary objectives included describing the characteristics of the vHIT ipsilesional and contralesional vestibulo-ocular reflex slow-phase velocity in patients with varying levels of canal paresis. Finally, the authors calculated the sensitivity and specificity of the vHIT for detecting horizontal semicircular canal impairment using the caloric

Comparing neurootological complaints in patients at the end of their professional lives (51-60 years) with those during the first phase of retirement (61-70 years).

Geriatrics defines the branch of medicine that treats all problems peculiar to old age and the aging, including the clinical problems of senescence and senility. In the full chain of years of the human life, a special period is humans' last decade of professional life (i.e., 51-60 years) and their first decade of retirement (i.e., 61-70 years). For this study aimed at comparing neurootological complaints of persons in this period of their lives, we examined large samples of European neurootological patients: Group A consisted of 1,965 persons aged 51-60 years, and group B consisted of 1,032 pe

[Vestibular tests in infants with TORCH and after CNS infections].

Thorough evaluation of hearing and balance in infants with congenital and perinatal TORCH (ang. Toxoplasmosis, Other, Rubella, Cytomegalovirus, Herpes) infections and after infections of the central nervous system (CNS) is essential due to serious danger of inner ear damage. Caloric stimulation is one of the most reliable diagnostic methods of vestibular testing in infants. Recording of vestibular-evoked myogenic potentials (VEMPs) makes evaluation of the otolith organ and its pathways possible. Potentials resulting from sacculo-collic reflexes are recorded on the surface of the neck muscles.
