Phrenicus palsy in progressive systemic sclerosis.

We report the course of a patient with progressive systemic sclerosis (PSS), who died of respiratory failure. Her condition was aggravated by the terminal development of unilateral Phrenicus palsy (PP), which has not previously been reported among the multiple presentations of scleroderma. PP should be borne in mind as one of the potential causes of decreasing lung function in scleroderma.

[A case of respiratory dyskinesia due to clebopride malate].

Clebopride malate is therapeutically used for the treatment of peptic ulcer. This drug has potent antidopaminergic activity that causes acute dystonic reaction, parkinsonism and tardive dyskinesia as adverse effects. Here, we have reported an 86-year-old man who developed abnormal involuntary movement of respiratory muscles and lower limb muscles after this drug had been given for four months. This involuntary movement appeared spontaneously at resting state and disappeared during sleep. Surface EMG demonstrated a synchronous grouping discharge in m. orbicularis oris, m. sternocleidomastoideus

[Lung resection in the principle disease of mucoviscidosis and status following poliomyelitis--2 case reports].

Two cases of destroyed lung after repeated pneumonia are reported; one in a patient suffering from cystic fibrosis in the age of 8 years, the other in a 15 year old girl with a phrenic nerve paralysis. In both children physiotherapy as well as retreatments with appropriate antibiotics did not achieve any long-term curative effect. Therefore a pneumonectomy was indicated. In the patient with cystic fibrosis a small air leak was observed on the first postoperative day at the bronchial suture line, which was treated with an immediate rethoracotomy and correction. In the other child a pericardial

Critical swallowing functions contributing to dysphagia in patients with recurrent laryngeal nerve paralysis after esophagectomy.

BACKGROUND: Recurrent laryngeal nerve paralysis (RLNP) after esophagectomy can cause aspiration because of incomplete glottis closure, leading to pneumonia. However, patients with RLNP often have preserved swallowing function. This study investigated factors that determine swallowing function in patients with RLNP. METHODS: Patients with esophageal cancer who underwent esophagectomy and cervical esophagogastric anastomosis were enrolled between 2017 and 2020. Videofluoroscopic examination of swallowing study (VFSS) and acoustic voice analysis were performed on patients with suspected dysphagi

Case Report: Neonate With Stridor and Subcutaneous Emphysema as the Only Signs of Physical Abuse.

A stridulous, dysphonic cry with no external signs of trauma is a unique and unusual presenting sign for physical abuse. We report a previously healthy neonate with unremarkable birth history and medical history who presented with stridor and hypopharyngeal perforation due to physical abuse. This case highlights the need for further evaluation for traumatic injuries in the setting of unexplained new-onset stridor and consideration of physical abuse in the differential diagnosis.

Inflammatory nodule mimicking a phrenic neoplasm.

BACKGROUND AND AIMS: Isolated phrenic nerve nodule is usually a primitive tumour. Surgery is diagnostic and therapeutic at the same time. We report the case of a completely serum-negative Caucasian male with a right diaphragmatic relaxation associated to an isolated small nodule of the phrenic nerve. METHODS: The patient was referred to our unit complaining shortness of breath and progressive fatigue. A standard chest X-ray showed right diaphragmatic palsy; chest scanning revealed a nodular lesion belonging to the right phrenic nerve. Positron emission tomography was negative for glucose upta

Retrospective study investigating causes of abnormal respiratory noise in horses following prosthetic laryngoplasty.

REASONS FOR PERFORMING THE STUDY: To investigate causes of respiratory noises in horses following prosthetic laryngoplasty (with or without a ventriculocordectomy) and to examine potential associations between degree of arytenoid abduction and the presence of other upper respiratory tract (URT) abnormalities, including right-sided collapse. METHODS: Clinical records and dynamic videoendoscopic recordings were examined from horses presented between 1995 and 2010 for investigation of respiratory noise during exercise, following a prosthetic laryngoplasty (+/- a ventriculocordectomy). Relationsh

Unilateral phrenic nerve paralysis in a patient with Wegener's granulomatosis.

A 35-year-old male diagnosed with Wegener's granulomatosis (WG) in 1989 recently came to our hospital with the complaint of left lumbar pain. He was found to have left nephrolithiasis and left diaphragmatic elevation. Fluoroscopic study and electromyographic examination revealed findings compatible with unilateral phrenic paralysis. We could not detect any cause other than WG which could be responsible for the unilateral diaphragmatic paralysis. Although different types of lung involvement have been reported in WG, to our knowledge this is the first in which unilateral phrenic nerve paralysis

Assessment of the results of glottis-dilating operations using lung function tests.

Our aim was to obtain an objective evaluation of the airway before and after glottis-dilating operations utilizing lung function tests. The charts of 109 patients who underwent either reversible or irreversible glottis-dilating operations by Lichtenberger were reviewed. 64 nonselected cases of these patients, all with irreversible glottis-dilating operations, were studied. Lung function tests that were performed were body-pletysmography, forced inspiratory volume (FIV1), forced expiratory volume (FEV1), peak inspiratory flow rate (PIF), peak expiratory flow rate (PEF) and resistance of the air

Bilateral vocal cord dysfunction complicating short-term intubation and the utility of heliox.

Axial spondyloarthritis (SpA) is a chronic inflammatory rheumatic disorder that primary affects axial skeleton. It comprises wide spectrum of patients with immune mediated spine inflammation, from early, so called non-radiographic axial spondyloarthritis to clinically evident ankylosing spondylitis. Conventional radiography is still the cornerstone of diagnosis, evaluation and classification of SpA. However, it has limitations in early disease, because it can only depict the consequences of inflammation for its inability to visualize soft tissue abnormalities within bone marrow. Magnetic reson

Hemidiaphragmatic paralysis following subclavian vein catheterization.

The right subclavian artery was inadvertently punctured during attempted preoperative insertion of a right subclavian venous catheter in a 59-yr-old woman undergoing radical hysterectomy. Large supraclavicular swelling became apparent soon after the arterial puncture. The postoperative chest X-ray obtained approximately 24 h after the catheterization revealed significant elevation of the right hemidiaphragm, which was further augmented on the 2nd to 4th postoperative days; oxygenation was concurrently impaired during these days. It was clinically judged that the hemidiaphragmatic paralysis was

Hemidiaphragmatic paralysis caused by cervical herpes zoster.

Although herpes zoster virus usually affects sensory nerves, it can also damage motoneurons. Injury to the phrenic nerve has been described previously, but only anecdotally. We report on a case of left hemidiaphragmatic paralysis with severe axonal degeneration secondary to cervical herpes zoster, and describe its clinical, radiological, pulmonary function and electromyographic evolution during an 18-month follow-up.

Anatomical interrelation between the phrenic nerve and the internal mammary artery as seen by the surgeon.

Paresis of the diaphragm (especially left-side paresis) is a relatively frequent finding following cardiac surgery. While, usually, it is a rather benign condition, in exceptional cases it may lead to severe impairment to death of the patient. The supposed causes of damage to the phrenic nerve include: local myocardial cooling by ice slush; opening of the pleural cavity in connection with local cooling; cross clamp length; total hypothermia; central venous cannulation; traction-related damage; mammary artery harvesting. Perhaps the commonest cause of damage to the phrenic nerve, i.e., the effe

Vocal cord dysfunction presenting as bronchial asthma: the association with abnormal thoraco-abdominal wall motion.

A 23 year old man with glottic dysfunction presented as recurrent bronchial asthma. His symptoms were aggravated by application of a noseclip and associated with asynchronous thoraco-abdominal wall motion. The glottic dysfunction was corrected by sedation but not continuous positive airway pressure. This is the first report of vocal cord dysfunction triggered by application of a noseclip and associated with asynchronous thoraco-abdominal wall motion.
