The Significance of Retinal Arterial Anastomosis in the New Classification System of Prepapillary Vascular Loops.

OBJECTIVE: To analyze the ophthalmic characteristics of congenital prepapillary vascular loops (PVL), propose the significance of retinal ar … ranching vessels, trifurcation of superior retinal vein, smaller angle of vascular arcade, and cilioretinal artery. CONCLUSIONS: PVLs are usually arterial loops, however, venous loops may not be as rare as previously thought. Both arterial and venous loops can cause vitreous hemorrhage, and venous loops can cause retinal vein occlusion. PVL could be divided int …

Unusual case of retinal arterial branch occlusion: possible variant of Sneddon syndrome.

Sneddon's syndrome (SS) manifests through multiple strokes and livedo reticularis. Livedoid vasculopathy (VL) is characterized by a long history of foot and leg ulceration and histopathology indicating a thrombotic process. Arterial retinal branch occlusion is described in a 52-year-old male with VL. He did not present noticeable laboratory abnormalities, such as antiphospholipid antibodies, or a history of strokes. Retinal artery occlusion accompanied by VL could be a variant of Sneddon's syndrome. Optical coherence tomography angiography revealed a reduction in the macula's vascular layers i…

Retinal arterial macroaneurysm rupture by Valsalva maneuver: a case report and literature review.

BACKGROUND: Retinal artery macroaneurysms (RAMs) are focal dilations of the large retinal arteries. Most RAMs are asymptomatic, however, when hemorrhage or exudation caused by a RAM involves the macula, patients can experience marked vision loss. This article reported a rare case of a ruptured RAM due to the Valsalva maneuver in an elderly female with constipation and offered a review of the relevant literature. CASE PRESENTATION: A 78-year-old woman with several risk factors presented with multi-level retinal hemorrhages following a Valsalva maneuver during constipation. Due to the poor coag…

Endothelial dysfunction and impaired arterial wall properties in patients with retinal vein occlusion.

Retinal vein occlusion (RVO) is a common retinal vascular lesion, and a leading cause of visual impairment. Patients with RVO have an increased risk for cardiovascular disease and share multiple common risk factors. In this study, we investigated the endothelial function and arterial stiffness of patients with RVO compared to healthy-control (CL) subjects. We enrolled 40 consecutive patients with RVO and 40 CL subjects. RVO was diagnosed by an ophthalmologist, endothelial function was evaluated by flow mediated dilation (FMD) in the brachial artery, and carotid-femoral pulse wave velocity (PWV

Optical coherence tomography in retinal arterial occlusions: case series and review of the literature.

Retinal arterial occlusions (RAOs) are an uncommon source of monocular vision loss, typically occurring in patients over the age of 60. Diagnosis is typically made by history and clinical examination, while ancillary testing may include fluorescein angiography. Optical coherence tomography (OCT) is not routinely utilized in the diagnostic assessment of RAO, and its role in the diagnosis and management of patients with RAO is still evolving. In this series, we review the literature on OCT findings in RAOs, particularly noting the role of OCT in delineating anatomic findings, chronicity and natu

[Analysis of risk factors and comparison of heparin and hemodilution therapies for retinal artery occlusion].

No standards are available for the treatment of central retinal artery occlusion (CRAO). The aim of this study was to compare data about visual acuity development under heparin treatment with those from hemodilution. Data on 139 patients suffering from CRAO between 1991 and 2000 were examined retrospectively. Risk factors were listed. Therapies were compared in 38 of these patients, 23 of whom received full heparinization and 15 of whom received hemodilution. Many risk factors were shown. Neither therapy showed serious complications. Visual acuity improved significantly, by three or more lines…

Acute occlusion of the retinal arteries: current concepts and recent advances in diagnosis and management.

PURPOSE/BACKGROUND: Central retinal artery occlusion (CRAO) is usually a blinding event, and is not an infrequent presentation to the accident and emergency (A&E) department. The evidence-base in support of current treatment options is weak. METHODS: This paper reviewed the literature germane to the diagnostic, therapeutic and prognostic aspects of retinal arterial occlusive disease. RESULTS: The visual prognosis associated with CRAO remains poor, and current therapeutic practices are of unproven benefit. The non-ophthalmologist in the A&E department should lie the patient flat and give a st…
