Many risk factors have been linked to retinal vein occlusions (RVOs) whether central or branch retinal vein occlusion. Ocular risk factors include glaucoma and hypermetropia. Controversy exists to whether short axial length is a risk factor for retinal vein occlusions. We report an extreme case that supports the latter hypothesis. A 33-year-old male presented with decreased visual acuity in the left eye. He turned out to have nanophthalmos with hemiretinal vein occlusion and macular edema with unremarkable systemic work up for retinal vein occlusion except for a glycated hemoglobin (HbA1c) lev…

Retinal vascular occlusions are the second most common retinal vascular diseases following diabetic retinopathy. Central retinal artery occl … ses are arteritis and vascular spasm. The pathogenesis of retinal branch vein occlusion and central retinal vein occlusion remains speculative. Two different mechanisms have been postulated, i.e. thrombosis in the vein due to a compression by atherosclerotic changes in the adjacent artery and a local alteration of the blood flow due to unfavourable physiologic …

Retinal vein occlusions and central retinal artery occlusions have been reported with coronavirus disease 2019 (COVID-19) vaccines. We aim to provide a descriptive analysis of cases reported in France until mid-2023, and recorded in the French pharmacovigilance database. An independent ophthalmologist reviewed all cases. We analyzed 290 cases (228 retinal vein occlusions, 58 central retinal artery occlusions, and four combinations). Retinal vein occlusions occurred with mRNA vaccines (68.0%) and adenovirus-vectored vaccines (32%), with an 11-day median onset delay. Almost half of the patients …

Retinal vein pulsation was first noted soon after the invention of the ophthalmoscope 170 years ago and was seen to change with cerebrospinal fluid pressure (CSFP) variation in the 1920s. The classical explanation for vein pulsation was that the cardiac cycle induced systolic peak in intraocular pressure (IOP) tended to intermittently collapse the retinal vein close to its exit in the central optic disk, causing pulsation to be counter-phase to IOP. Recently, improved ophthalmodynamometry and video recording techniques have allowed us to explore the fundamentals of retinal vein pulsation. This…

We found a central retinal vein occlusion in three of nine patients with spontaneous arteriovenous fistulas of the cavernous sinus region. The femoral angiogram displayed a "low-flow" fistula from the internal carotid artery and/or dural branches of the external carotid artery into the cavernous sinus in all three patients. Angiographically, occlusion of the superior ophthalmic vein was present in all three patients. The arteriovenous flow of the shunt was directed into the sphenoparietal sinus and into other cerebral veins, but not into the orbit. This is the first angiographic demonstration …

Previous studies have suggested an association between hyperlipidaemia and retinal venous and arterial occlusion. To investigate this association further, the retinal arterial vasculature was studied by fluorescein angiography in forty hyperlipidaemic subjects, and clinical examination and biochemical investigations, including lipid profile, were performed in 99 patients with retinal vein occlusion and forty patients without retinal vein occlusion as a comparison group. Retinal arterial abnormalities were found on fluorescein angiography in eight patients with combined hypercholesterolaemia an…

We reviewed retrospectively 20 cases of optociliary vein over the past 5 years. Fifteen of the 20 cases (75%) were over 40 years of age. There was no sex differences (9 males and 11 females). Causative ocular diseases were: central retinal vein occlusion (14 cases, 70%); optic disc drusen (2 cases, 10%); and optic nerve sheath meningioma, high myopia, glaucoma, congenital anomaly (1 case each, total 20%). The number of patients with an optociliary vein was high in central retinal vein occlusion (14/190 cases, 7.4%). We concluded that occurrence of optociliary vein is not restricted to cases wi…

Optociliary veins occur in three clinical settings: in chronic central retinal vein obstruction, with retro-orbital tumors, and as congenital anomalies. In the last setting, associated anomalies of the disc and retinal vessels may be present, which makes differentiation from the first two difficult. Fluorescein angiography may be of value in making this differentiation for, in some if not all congenital OC veins, the direction of flow is from the choroid into the central retinal vein. Two cases are presented in which a congenital optociliary vein was present in association with a fundus pictur…

Fifteen patients are described in whom the triad of blindness, optic disc swelling followed by optic atrophy, and optociliary shunt veins occurred. The causes of the syndrome included spheno-orbital meningioma, optic nerve glioma, meningocele of the optic nerve, and chronic papilloedema. It is postulated that chronic compression of the intraorbital portion of the optic nerve produces gradual obstruction of the central retinal vein, thus preventing the normal passage of venous blood from the retina through the central retinal vein to the cavernous sinus. Optociliary veins are a pre-existing shu…

The authors present two cases of neovascular glaucoma following branch retinal vein occlusion. The first case presented bilateral ischaemic branch retinal vein occlusions. In this case there was bilateral impairment of blood flow in the carotido-cerebrovascular system, more marked on the side where the neovascular glaucoma was present. The second case developed neovascular glaucoma following an ischaemic branch retinal vein occlusion of the right eye and had bilateral carotid stenosis that was more complete on the right: where the neovascular glaucoma occurred. The authors analyse the main pat…
