Morphometric comparison of the pedicle rib unit to pedicles in the thoracic spine.

STUDY DESIGN: A radiographic study of the pedicle rib unit morphology as compared with measurements of the pedicle in cadaveric thoracic spines. OBJECTIVE: To evaluate the morphology of pedicle rib units in the thoracic spine in normal thoracic human spines, with particular attention to T4-T9, and to compare the dimensions of the pedicle rib unit with corresponding dimensions of the adjacent pedicles. SUMMARY OF BACKGROUND DATA: Despite the clinical successes reported with thoracic pedicle screw-rod constructs, controversy exists as to the safety of this technique in pedicles that are anatom

Isolated cryptococcal thoracic empyema with osteomyelitis of the rib in an immunocompetent host.

Cryptococcal thoracic empyema is extremely rare and has been described almost always in immunocompromised patients with pulmonary or disseminated cryptococcosis. To our knowledge, isolated cryptococcal thoracic empyema with rib osteomyelitis has never been reported in an immunocompetent patient. We herein present such a case. Computed tomographic (CT) scan suggested the diagnosis of thoracic empyema in association with adjacent rib osteomyelitis, confirmed by postoperative histological examination. Surgical debridement followed by appropriate antifungal therapy achieved a complete eradication

Rib fracture pain and disability: can we do better?

OBJECTIVE: The purpose of this study was to determine the magnitude and duration of pain and disability in patients with rib fractures treated using current standard therapy. This was a prospective case series. METHODS: Injured patients with a chest radiographic diagnosis of one or more rib fractures between June 1, 2001, and October 31, 2001, were asked to participate. Pain levels were assessed at days 1, 5, 30, and 120 after injury using a visual pain scale (0-10). Disability at 30 days was assessed using the SF-36 Health Status Survey, and the total number of days lost from work/usual acti

Solitary rib lesions in bone scans of patients with breast carcinoma.

The aetiology of solitary rib lesions detected on bone scans was evaluated retrospectively. Seventy-five patients with breast carcinoma, and each with a solitary hot spot on a bone scan, were included in the study. The aetiology of the solitary rib lesions was determined by using all available clinical, laboratory and radiological data, and was clarified in 65 of the 75 patients, and not clarified in the remaining 10. In 17 of those 65 (26.1%), the aetiology of increased uptake was malignant; while in 48 of the 65 (73.8%) it was benign in origin. Linear lesions were mainly metastatic in origin

The cervical rib. A predisposing factor for obstetric brachial plexus lesions.

Controversy surrounds the aetiology of obstetric brachial plexus lesions. Most authors consider that it is caused by traction or compression of the brachial plexus during delivery. Some patients, however, present without a history of major traction during delivery, and some delivered by Caesarean section also suffer the injury. In our series of 42 infants, 28 had an Erb's palsy, and the remaining 14 presented with a more extensive lesion, involving the lower roots. In five of these, a complete ossified cervical rib was found. We believe that anatomical variations, such as cervical ribs or fibr…

Hepatocellular carcinoma with metastasis to the rib complicated by hemothorax. An autopsy case.

A 64-year old man was admitted to our hospital with multiple hepatocellular carcinoma (HCC) lesions in the liver, lung and bone. Three weeks after admission, the patient became complicated with right upper chest pain. A chest radiograph showed a marked increase in right pleural effusion. Thoracentesis demonstrated a hemothorax. Despite treatment with a continuous pleural tap and blood transfusions, the patient's clinical status worsened and he developed severe dyspnea. His right pleural effusion might be considered to be caused by a rupture of the HCC metastasis in the right 2nd rib. The patie

Cervical rib and median sternotomy-related brachial plexopathies: a reassessment.

OBJECTIVE: The objective of this study was to identify electrodiagnostic and anatomic distinctions between true neurogenic thoracic outlet syndrome and median sternotomy-related brachial plexopathy, in reference to the pattern of abnormality of the medial antebrachial cutaneous sensory nerve conduction study (NCS) response. BACKGROUND: Neurogenic thoracic outlet syndrome and sternotomy-related brachial plexopathy are both lower trunk brachial plexopathies, but their clinical and electrodiagnostic presentations are distinct. The anatomic differences distinguishing these disorders from each oth

[Vertebral body replacement with a rib segment block in transthoracic intervention].

The aim of this clinical report is to describe the outcome of 7 patients regarding the efficiency of a vertebral body replacement with rib bone for destructive lesions in the thoracic spine. In these patients, an autologous bone block shaped from four rib segments, was used as a vertebral body replacement. In order to compose the bone block, the ribs were fused by means of one or two commercialized titanium screws. In a retrospective study the following parameters were analysed: neurological status, rate of bony fusion, primary correction gain, secondary correction loss. Mean time of follow-up…

[Removal of the 1st rib in thoracic outlet syndrome. Is it helpful? Is it safe?].

A thoracic outlet syndrome is only rarely caused by osseous anomalies. Instead, it is a soft-tissue disease, where the C8 and T1 roots or the proximal inferior trunk of the brachial plexus are compressed by fibromuscular anomalies between the inferior cervical spine and the first rib or pleura. Thoracic outlet syndrome is a clinical diagnosis. If surgery is recommended, it should be concentrated on the fibromuscular anomalies and not on the first rib. Transaxillary removal of the first rib carries an important risk of serious complications. Among these, brachial plexus injury is the most frequ

Rib-muscle pedicle flap for the repair of congenital tracheal stenosis.

Congenital tracheal stenosis (CTS) is an uncommon congenital anomaly that presents early in life with symptoms of biphasic stridor. Most cases require surgical correction. Techniques have included dilation, resection of the involved segment, and tracheoplasty. Today pericardium and costal cartilage are the most frequently used materials for tracheoplasty, but patients still often encounter problems with the graft, with the procedure, or with late complications. This preliminary study was undertaken to determine the feasibility of a rib-intercostal muscle pedicle flap for the treatment of CTS.

Short rib-polydactyly syndrome and pericentric inversion of chromosome 4.

We report on a newborn infant with clinical and radiological manifestations of some type of short rib-polydactyly syndrome who died soon after birth. Chromosomal studies on peripheral blood lymphocytes and chondrocytes demonstrated an apparently balanced pericentric inversion of chromosome 4 (present in the mother also). This association may have occurred by chance but, if not, the chromosomal breakpoints could interrupt the gene responsible for short rib-polydactyly syndromes, or else be related to the mechanism of short rib-polydactyly syndromes.

[Successful operation for two cases of an aneurysmal bone cyst of the rib].

An aneurysmal bone cyst involving the rib is very rare. A 43-year-old man and 49-year-old woman complained of pain and swelling on the chest. X-ray and CT revealed a local expansion and honeycomb lesion of the right 9th rib. Surgical excision of the rib were performed to remove the tumor completely. The histopathological examination revealed that the lesion consisted of many cavities filled with blood and trabecullae containing osteoid tissue and giant cells, showing it to be an aneurysmal bone cyst. Because of the potential for the rapid growth and difficulty in differentiation between benign
