Non-specific effects of childhood vaccinations - A case control study nested into a Health and Demographic Surveillance System in rural Burkina Faso.

INTRODUCTION: Previous studies in African countries have been suggestive of non-specific effects (NSE) of vaccination on child survival. Live vaccines (e.g. measles, MV) have been found to reduce child mortality while inactivated vaccines (e.g. diphtheria-tetanus-pertussis, DTP) have been associated with increased mortality; NSE were often found to be sex-specific. METHODS: A case-control study nested into the Health and Demographic Surveillance System (HDSS) cohort of the Centre de Recherche en Santé de Nouna (CRSN) was conducted in northwestern Burkina Faso. A total of 3,010 children born i

Following the World Health Organization's Recommendation of Exclusive Breastfeeding to 6 Months of Age Does Not Impact the Growth of Rural Gambian Infants.

BACKGROUND: The WHO recommends exclusive breastfeeding (EBF) for the first 6 mo of life. OBJECTIVE: The objective of this study was to assess the benefit of EBF to age 6 mo on growth in a large sample of rural Gambian infants at high risk of undernutrition. METHODS: Infants with growth monitoring from birth to 2 y of age (n = 756) from the ENID (Early Nutrition and Immune Development) trial were categorized as exclusively breastfed if only breast milk and no other liquids or foods were given. EBF status was entered into confounder-adjusted multilevel models to test associations with growth t

Cost-effectiveness of community-based strategies to strengthen the continuum of HIV care in rural South Africa: a health economic modelling analysis.

BACKGROUND: Home HIV counselling and testing (HTC) achieves high coverage of testing and linkage to care compared with existing facility-based approaches, particularly among asymptomatic individuals. In a modelling analysis we aimed to assess the effect on population-level health and cost-effectiveness of a community-based package of home HTC in KwaZulu-Natal, South Africa. METHODS: We parameterised an individual-based model with data from home HTC and linkage field studies that achieved high coverage (91%) and linkage to antiretroviral therapy (80%) in rural KwaZulu-Natal, South Africa. Cost

The influence of orphan care and other household shocks on health status over time: a longitudinal study of children's caregivers in rural Malawi.

In the context of rising rates of orphanhood in AIDS-affected settings, very little is understood about implications for caregiver well-being given increasing and intensifying responsibilities for the care of orphaned children. Emotional distress and self-reported health status as well as shifts in household orphan care, wealth, food security and recent illness and death among household members were measured among a panel of 1219 caregivers in rural Malawi between 2007 and 2009. Logistic regression was used to identify predictors of improved and diminished caregiver health and emotional distre

[Survey on the current status of drinking water supply, latrines and the health knowledge of residents in the remote rural areas of several provinces of China].

A survey on the current status of drinking water supply, latrines and the health knowledge of residents in the remote rural areas of the southwest and northwest of China was reported. A stratified cluster sampling method was adopted in the survey. 83 poverty countries and 74,900 families were involved in the survey and four hundred thousands data were obtained by home visiting. The results showed that the coverage of rural latrines, sanitary latrines and safe night-soil disposal in the surveyed regions had reached 90.0%, 6.0% and 25.0% of populations respectively. The coverage of safe-drinking

Facilitating Personal Recovery in the Community: a Qualitative Evaluation of a Peer Support Group for Adults with Severe Mental Health Challenges in Rural Australia.

Individuals with severe mental health challenges [SMHCs] often have unmet needs that are beyond the scope of traditional mental health services. These needs are typically met by community agencies, charities and peer support groups. Happy Chat [HC] is one such peer-support group whose goal is to address social isolation, and loneliness by creating a safe space to facilitate the development of informal friendships among individuals with SMHCs in the community. Nineteen participants were interviewed as part of a qualitative evaluation of HC. Data were subjected to thematic analysis. Four categor

Telemedicine as a "digital umbilical" for maternal and reproductive healthcare: a qualitative study from health and wellness centers in rural West Bengal, India.

BACKGROUND: Rural women in India continue to face major geographical, financial, and sociocultural barriers to get timely maternal and reproductive healthcare, despite recent investments in primary care infrastructure and digital health initiatives. The West Bengal state telemedicine program, where patients are delivered healthcare through Health and Wellness Centres (HWCs), offers a hybrid model that combines on-site Community Health Officers (CHOs) with remote physicians, but its functionality for reproductive health remains underexplored. METHODS: This descriptive qualitative study explore

Pregnancy loss in rural Bangladesh: an analysis of rates, proportions, timing, and determinants based on data from a Health and Demographic Surveillance System.

BACKGROUND: Pregnancy loss, encompassing miscarriage/spontaneous and induced abortion, has substantial physical and mental health consequences for women. However, population-level evidence on its occurrence in Bangladesh remains limited, with most data derived from reports and small-scale community-based studies. Here we provide population-level estimates of pregnancy loss and examine associated factors in rural Bangladesh. METHODS: We analysed data from 61 428 women aged 15-49 years and 7612 pregnancies, extracted from the Baliakandi Health and Demographic Surveillance System of icddr,b and

Alcohol Use in a Tribal Rural Population of India: Prevalence, Social Determinants, and Impact of a Community-Based Health System-Integrated Intervention.

BACKGROUND: Alcohol consumption is an overlooked public health concern in tribal populations of India. Culturally and systemically rooted factors contribute to neglect in engagement and treatment, even with prohibition policies in place. Therefore, this study analyzed the prevalence and sociodemographic factors associated with alcohol consumption in a tribal rural district of India and evaluated the effects of a health system-integrated, community-based intervention on knowledge, attitudes, and practices (KAP) concerning abstinence. METHOD: A total of 269 community members and 81 healthcare w

Low Earth Orbit Communication Satellites: A Positively Disruptive Technology That Could Change the Delivery of Health Care in Rural and Northern Canada.

Canada is a progressive nation that endeavors to provide comprehensive, universal, and portable health care to all its citizens. This is a challenge for a country with a population of 40 million living within a land expanse of 10 million km2 and where 18% live in rural or highly remote locations. The combined population of Yukon, Northwest Territories, and Nunavut is only 128,959 (0.32% of the population), living within 3.92 million km2, and many of these citizens live in isolated communities with unique health needs and social issues. The current solution to providing health care in the most
