Electroencephalographic and seizure manifestations of pyridoxal 5'-phosphate-dependent epilepsy.

We describe the electroencephalographic and clinical seizure manifestations of pyridoxal 5'-phosphate-dependent epilepsy (PLP-DE) in two patients [diagnosis confirmed by low cerebrospinal fluid (CSF) PLP, complete resolution of previously intractable seizures with PLP supplementation, negative pyridoxine-dependent epilepsy CSF biomarkers, and/or positive disease causing pyridox(am)ine 5'-phosphate oxidase gene mutation] along with a comprehensive review of the literature. One patient presented with neonatal tonic status epilepticus with subsequent generalized tonic-clonic seizures, and the sec

Non-REM Sleep Characteristics Predict Early Cognitive Impairment in an Aging Population.

Objective: Recent research suggests that sleep disorders or changes in sleep stages or EEG waveform precede over time the onset of the clinical signs of pathological cognitive impairment (e.g., Alzheimer's disease). The aim of this study was to identify biomarkers based on EEG power values and spindle characteristics during sleep that occur in the early stages of mild cognitive impairment (MCI) in older adults. Methods: This study was a case-control cross-sectional study with 1-year follow-up of cases. Patients with isolated subjective cognitive complaints (SCC) or MCI were recruited in the Bo

Alcohol-induced sleepwalking or confusional arousal as a defense to criminal behavior: a review of scientific evidence, methods and forensic considerations.

An increasing number of criminal cases have claimed the defendant to be in a state of sleepwalking or related disorders induced by high quantities of alcohol. Sleepwalkers who commit violent acts, sexual assaults and other criminal acts are thought to be in a state of automatism, lacking conscious awareness and criminal intent. They may be acquitted in criminal trials. On the other hand, criminal acts performed as the result of voluntary alcohol intoxication alone cannot be used as a complete defense. The alcohol-induced sleepwalking criminal defense is most often based on past clinical or leg

Electroencephalogram Data-Based Analysis of Paroxysmal Slow Wave Events Patterns in Brain Pathologies.

Slowing of brain activity observed in electroencephalography (EEG) recordings is normal under resting conditions such as sleep. However, a recent series of studies described a new pattern of cortical slowing in patients with epilepsy and Alzheimer's disease, consisting of transient paroxysmal slowing of the network. These paroxysmal slow wave events (PSWEs) were defined with median power frequency (MPF) less than 6 Hz and duration longer than 5 s. In this research, we are using clinical EEG recordings from the Temple University and Bonn University databases. We aim to: (1) Characterize the tem

Pathogenesis of giant somatosensory evoked potentials in progressive myoclonic epilepsy.

Fifty-seven consecutive patients with myoclonus from various causes were studied by electrophysiological techniques. Giant somatosensory evoked potentials (SEPs) were observed almost exclusively in patients with progressive myoclonic epilepsy (PME) and diseases with similar clinical features that included lipidosis, neuronal ceroid lipofuscinosis and posthypoxic myoclonus. On the basis of combinations of the giant SEP and the myoclonus-related cortical spike demonstrated by jerk-locked averaging, myoclonus in these patients was classified into four types. In patients with 'cortical reflex' myo

Morning vs. evening growth hormone injections and their impact on sleep-wake patterns and daytime alertness.

CONTEXT: Physiological growth hormone is secreted during slow-wave sleep. Traditionally, growth hormone (GH) therapy is given in daily GH injections before sleep. While morning and evening GH injections produce comparable effects on growth and IGF-1 levels, the evening schedule better imitates the physiological diurnal pattern of GH secretion and action. However, the inflexibility of bedtime injection schedules, coupled with the discomfort and psychological distress associated with the injection and local reaction, may cause sleep disturbances in patients, and may significantly burden them and

Arousals and sleep stages in patients with obstructive sleep apnoea syndrome: Changes under nCPAP treatment.

Nocturnal arousals are the essential cause of disturbed sleep structure in patients with obstructive sleep apnoea syndrome (OSAS). The aim of this study was to analyse the relationship between sleep stages, respiratory (type-R) and movement (type-M) related EEG arousals. Furthermore, the value of these arousals as a criterion for the efficiency of nCPAP treatment was estimated. We examined 38 male patients aged between 30 and 71 (49.1 +/- 20.9 SD) y. All patients suffered from OSAS. The mean respiratory disturbance index (RDI) was 47.3 +/- 27.8 per h. Polysomnographic monitoring was carried ou

Irreversible functional network deformation induced by preprocessing scheme order in multivariate EEG recordings.

INTRODUCTION: The analysis of Electroencephalographic recordings is non-trivial, the order in which filtering and re-referencing are applied during multivariate signal preprocessing has a critical and underappreciated consequence for functional network estimation. METHODS: Using EEG sleep recordings from healthy subjects, synthetic data from controlled numerical models (Nf-Tori and IAAFT surrogates), and surrogate-based statistical testing, we provide a quantitative comparison of two preprocessing schemes that differ solely in the order of filtering and re-referencing (filtering followed by r

Synthesis of a novel series of tricyclic indan derivatives as melatonin receptor agonists.

To develop a new therapeutic agent for sleep disorders, we synthesized a novel series of tricyclic indan derivatives and evaluated them for their binding affinity to melatonin receptors. In our previous paper, we proposed a conformation of the methoxy group favorable for the binding of the MT(1) receptor. To fix the methoxy group in an active conformation, we decided to synthesize conformationally restricted tricyclic indan analogues with the oxygen atom in the 6-position incorporated into a furan, 1,3-dioxane, oxazole, pyran, morpholine, or 1,4-dioxane ring system. Among these compounds, inde

[The distribution of sleep and wakefulness in human African trypanosomiasis].

Last century, patients with human African trypanosomiasis were described as sleepy by day and restless by night, and physicians referred to this condition as sleeping sickness. Such a description could have evoked a disturbance of circadian rhythms. However, it is only in 1989 that the first 24-hour recording was performed by our team in Niamey (Niger) in a patient with sleeping sickness. The patient was a Niger-born farm worker who had contracted the disease near Gagnoa (Côte d'Ivoire). Polysomnographic recordings (electroencephalogram, EEG, electrooculogram, electromyogram, electrocardiogram

[The influence of sleep breathing disorder on growth hormone secretion in children with tonsil hypertrophy].

INTRODUCTION: Recently it has been revealed that Obstructive Sleep Apnea Syndrome has an influence on the endocrine system, especially to the secretion of growth hormone (GH). It was previously reported that secretion of GH has a circadian rhythm along with a correlation with slow wave sleep. Normally in a pediatric patient, after 90 min of sleep the brain wave shows patterns of deep sleep, and a matching peak in GH secretion is observed. We have studied the effect of GH secretion in sleep breathing disorder of pediatric patients by observing the pre- and post-operative difference in GH secret

Neurobiology of sleep apnea and cognitive impairment.

Obstructive sleep apnea (OSA) has a broad and clinically important effect on cognition, most often impairing attention, executive function, memory, processing speed, and visuospatial ability, while language and immediate recall are relatively preserved. A substantial proportion of adults referred to sleep clinics already meet criteria for mild cognitive impairment at initial evaluation, particularly in moderate-to-severe OSA. This cognitive burden reflects multiple interacting mechanisms. Intermittent hypoxia promotes oxidative stress, mitochondrial dysfunction, and hippocampal injury through

Association of rapid eye movement sleep latency with multimodal biomarkers of Alzheimer's disease.

INTRODUCTION: Sleep disturbances are associated with Alzheimer's disease (AD) and Alzheimer's disease and related dementias (ADRD), but the relationship between sleep architecture, particularly rapid eye movement (REM) sleep, and AD/ADRD biomarkers remains unclear. METHODS: We enrolled 128 adults (64 with Alzheimer's disease, 41 with mild cognitive impairment [MCI], and 23 with normal cognition [NC]), mean age 70.8 ± 9.6 years, 56.9% female, from a tertiary hospital in China. Participants underwent overnight polysomnography (PSG), amyloid β (Aβ) positron emission tomography (PET), and plasma

Predictive factors of long-term compliance with nasal continuous positive airway pressure treatment in sleep apnea syndrome.

The long-term acceptability of treatment with nasal continuous positive airway pressure (CPAP) was studied prospectively in 44 patients with obstructive sleep apnea syndrome. At 14 months on the average after starting treatment with CPAP, 30 patients (68 percent) were found to be compliant (characterized by use of the apparatus every night throughout the night, for more than 5 h per night). The daily use of nasal CPAP was significantly correlated to the initial apnea/hypopnea index (p = 0.013; r = 0.37), as well as to the percentage of light sleep (p = 0.045; r = 0.30) and slow-wave sleep (p =

Computer analysis of cardiovascular changes during sleep-wake cycle in Sprague-Dawley rats.

Blood pressure (BP) and heart rate (HR) were recorded in seven Sprague-Dawley rats during a total of 491 normal sleep-wake cycles with the use of a computer-assisted method developed for this study. Significant changes of BP, HR, and BP variability (BPV) were found between the three states within the cycle, i.e., wakefulness (W), slow-wave sleep (SWS), and paradoxical sleep (PS). The highest BP, BPV, and HR values were found during W. Then all cardiovascular (CV) variables fell during SWS, whereas BP and BPV rose again during PS. The fall of BPV observed during SWS was the most important CV ch

[Body movements during sleep in Lennox syndrome].

In Lennox syndrome the brainstem which plays important roles in regulating sleep and its parameters is thought to be disturbed. In order to clarify the importance of the dysfunction of the brainstem in Lennox syndrome, polygraphic examination were studied and their findings were assessed with prognosis. 8 patients aged from 6 to 17 years were subjected to this study. They were divided into two groups according to their prognosis. Group 1 showed good prognosis. Seizures were easily controllable and have not occurred for more than 24 months. In group 2 seizures were intractable and were uncontro

Hyperinsulinemic response to oral glucose challenge in individuals with posttraumatic stress disorder.

BACKGROUND: Posttraumatic stress disorder (PTSD) is associated with a 2-4 fold increased risk of developing Type 2 diabetes mellitus. However, detailed assessments of glucose metabolism and insulin secretion in a study designed to minimize confounders are lacking. Furthermore, few studies examine potential mechanisms involved. We analyzed data from a case-control study of medically healthy, medication-free adults to determine whether individuals with PTSD had abnormal glucose or insulin response to oral glucose tolerance test (OGTT) compared to controls. Secondarily, we assessed potential medi

Narcolepsy patients have antibodies that stain distinct cell populations in rat brain and influence sleep patterns.

Narcolepsy is a chronic sleep disorder, likely with an autoimmune component. During 2009 and 2010, a link between A(H1N1)pdm09 Pandemrix vaccination and onset of narcolepsy was suggested in Scandinavia. In this study, we searched for autoantibodies related to narcolepsy using a neuroanatomical array: rat brain sections were processed for immunohistochemistry/double labeling using patient sera/cerebrospinal fluid as primary antibodies. Sera from 89 narcoleptic patients, 52 patients with other sleep-related disorders (OSRDs), and 137 healthy controls were examined. Three distinct patterns of imm

Autonomic dysfunction in children with sleep disordered breathing.

PURPOSE: Sleep disordered breathing (SDB) has adverse effects on cardiovascular health in adults, partly due to changes in autonomic activity. However, there have been limited studies in children. We analysed the impact of SDB and sleep stage on autonomic control of heart rate in 7-12-year-old children, utilizing spectral heart rate variability (HRV) as a measure of autonomic activity. METHODS: Eighty children underwent overnight polysomnography. Subjects were grouped according to their obstructive apnoea-hypopnoea index (OAHI): controls, OAHI ≤1 event/h and no history of snoring; primary sno
