BACKGROUND: Sphenoid sinus lateral wall cerebrospinal fluid (CSF) leaks are rare, with debated anatomical and physiological bases. We evaluated the role of pneumatization in surgical outcomes and compared patient morphometrics with cadaveric specimens.
METHODS: We retrospectively reviewed 11 patients (2017-2025) with lateral sphenoid CSF leaks. The patient characteristics, site of leaks, type of repair done, postclamping opening pressure, surgical approach, reconstruction technique, flap choice, and recurrence were documented. Fine-cut computed tomography (CT) provided morphometrics [foramen

Acute bilateral post-traumatic facial paralysis is rare in the literature. Post-traumatic facial paralysis is frequently accompanied transverse fractures of temporal more. The incidence of acute bilateral post-traumatic facial paralysis has been reported as 1 to 5 per million in the literature. Trauma and concurrent facial paralysis are usually in the same subsite (right temporal bone fracture and right facial paralysis). There is one pathophysiological pattern for a single temporal bone fracture in a subsite. The authors present a bilateral isolated different pathophysiological pattern sudden

Computed tomography (CT) is a very important diagnostic tool prior to endoscopic nasal or paranasal sinus surgery. However, it is frequently notice during endonsal surgery of the paranasal sinuses, that the intraoperative results do not completely agree with the radiological ones. The objective of the present study was to compare clinical and operative findings with those of CT investigations. We collected 200 cases treated during the past 2 years and studied their CT reports. Paranasal sinuses were separated into six regions, and the pathological changes due to sinusitis found by CT were clas

PURPOSE: The purpose of this work is to demonstrate the clinical utility of magnetic resonance (MR) imaging-based digitally reconstructed radiographs (DRRs) for the setup and verification of patients with intracranial lesions.
METHODS AND MATERIALS: MR images of 16 patients with various intracranial lesions were obtained for treatment planning and virtual simulation. Five-millimeter-thick contiguous T1-weighted postcontrast transverse slices were obtained using a standard head coil in a General Electric Signa 1.5T MR scanner. MR-DRRs were generated using the "pseudo density" technique on an e

For two cases of a high basilar tip aneurysm accompanied by a short intracranial internal carotid artery, the orbitozygomatic temporopolar approach consisting of an en bloc fronto-orbitozygomatic temporal craniotomy and temporopolar approach was carried out. On angiograms, the height of the bifurcation of an elongated basilar artery and the length of the intracranial internal carotid artery from the interclinoid line between the anterior and posterior clinoid process were 20 mm and 6 mm in Case 1, and 18 mm and 5 mm in Case 2, respectively. The skin flap was separated subfascially to preserve

Hutchinson-Gilford progeria syndrome (HGPS) is a rare dysmorphic syndrome characterized by several features of premature aging with clinical involvement of the skin, bones, and cardiovascular system. HGPS has an estimated incidence of one in four million to one in eight million births. The main clinical features of HGPS include short stature, craniofacial dimorphism, alopecia, bone fragility, and cardiovascular disorders. The most frequent cause of death is myocardial infarction at a mean age of 13 years old. Dental manifestations include delayed development and eruption of teeth, discoloratio

BACKGROUND: Most giant cell tumors (GCT) occur at the ends of long bones. There is little information about GCT of the skull bones.
METHODS: The authors reviewed the Mayo Clinic files, which contained 546 cases of GCT, and their own consultation files, which contained approximately 1500 cases.
RESULTS: Eleven tumors occurred in the sphenoid bone with extension to the surrounding bones and structures in 8 patients. One tumor (in Paget's disease) occurred in the frontal bone, one tumor was in the occipital bone, and one tumor was in the temporal bone. There were 4 men and 11 women whose ages r…

PURPOSE: Endoscopic endonasal and transcranial approaches are used for the resection of various pathological lesions in neurosurgery, especially pituitary adenomas, craniopharyngiomas, chordomas, or meningiomas. The video feed provided by endoscopes is generally two-dimensional, which can hinder depth perception. Thus, generating three-dimensional imaging without the need for special endoscopes using deep learning might be beneficial for enhanced intraoperative orientation.
METHODS: DINOv2 is a pre-trained deep-learning model published by Meta in 2023. One of its capabilities is to estimate t

BACKGROUND: Extradural anterior clinoidectomy (EAC) and dural peeling of the lateral wall of the cavernous sinus (CS) are challenging skull base techniques that enhance exposure to anterior and middle cranial fossa lesions. Intimate knowledge of dural anatomy enables safe dissection and identification of critical neurovascular structures without cranial nerve deficit postoperatively.
METHODS: In a patient with a middle/inner ridge sphenoid wing meningioma, EAC and targeted dural peeling allowed for a Simpson grade I resection.
CONCLUSION: EAC and targeted dural peeling enable a feasible Simp

The middle cranial fossa exhibits an interesting typical and variant anatomy. Accessory foramina frequently or rarely exists and transmits emissary veins. The sinus computed tomography scan of a 63-year-old male patient was further examined for its unique morphology. An accessory foramen rotundum (FR) was identified on the left side, 2.67 mm laterally to the typical foramen. Posterolaterally to the typical FR (10.33 mm distance), the intracranial opening of a sphenopterygoid canal was detected. This canal had a length of 22.85 mm and opened extracranially through a pterygoid foramen on the lat

OBJECTIVE: To evaluate the effectiveness of a "telestration" system in which the mentor annotates the view of the surgical field, for endoscopic transsphenoidal surgery (ETS).
METHODS: The use of telestration was evaluated for sellar floor-opening during ETS and for a task performed using ETS simulation training. During ETS, the mentor outlined the opening area of the sella turcica on the monitor and then the trainee surgeon opened the sella, either with the telestration displayed (telestration (+) group, n = 8) or without (telestration (-) group, n = 7). In the task using an ETS training mod

Sphenoid wing dysplasia is a characteristic finding in children with neurofibromatosis type 1 (NF1). Some of these children develop proptosis and vision loss secondary to the spheno-orbital encephalocele. A 6-year-old boy presented to us with complaints of painless progressive uni-ocular vision loss and progressive pulsatile proptosis. Imaging revealed spheno-orbital encephalocele into the orbit through the dysplastic posterior orbital wall. 3D printed customized implant was designed and placed to fit the defect. This prevented further herniation of the temporal lobe into the orbit, leading to

BACKGROUND: Superb knowledge of anatomy and techniques to remove the natural barriers preventing full access to the most lateral aspect of the skull base determines the ease of using the transpterygoid approach (ETPA) as the main gateway for all the coronal planes during endonasal surgeries.
METHODS: Throughout stepwise image-guided cadaveric dissections, we describe the surgical anatomy and nuances of the ETPA to the pterygopalatine fossa (PPF) and upper parapharyngeal space (UPPS).
CONCLUSION: The ETPA represents a lateral extension of the midline corridor and provides a valuable route to

The orbitomeningeal or meningo-orbital band (MOB) has been described as the most superficial dural band responsible for tethering the frontotemporal basal dura to the periorbita.1,2 The MBO usually interferes with the surgical approach to the most profound areas of the anterior and middle skull base. It is known that there are no cranial nerves on the lateral surface of the superior orbital fissure; therefore, the neurosurgeon can cut the MOB without causing any neurological deficit and, at the same time, achieving fully exposure of the anterior clinoid process1-4 and/or the lateral wall of th

BACKGROUND: Surgical management of intraconal pathology represents the next frontier in endoscopic endonasal surgery. Despite this, the medial intraconal space remains a relatively unexplored region, secondary to its variable and technically demanding anatomy. The purpose of this study is to define the neurovascular structures in this region and introduce a compartmentalized approach to enhance surgical planning.
METHODS: This study was an institutional review board (IRB)-exempt endoscopic anatomic study in 10 cadaveric orbits. After dissection of the medial intraconal space, the pattern and

A transcranial approach in combination with a transpetrosal setting has been the mainstream of surgical treatment of cholesterol granulomas in the petrous apex. However, endoscopic endonasal surgery has become a choice of treatment for these lesions with recent advancements in surgical techniques and instruments. We report a successful case of cholesterol granuloma managed with endoscopic endonasal surgery. A 45-year-old woman, who had a long-standing history of otitis media, presented with left abducens nerve palsy and discomfort around the left eye. Magnetic resonance (MR) imaging showed a l
