Angiolipomas in the lumbar spine are rare, with only a few reported cases presenting in a dumbbell shape. We report a case of a dumbbell-shaped lumbar angiolipoma extending not only into the spinal canal and the intervertebral foramen but also extensively into the iliopsoas muscle. A male patient in his early 60s, presenting with an 8-month history of low back pain and bilateral lower extremity pain, was referred to our department for treatment. MRI revealed a large dumbbell-shaped tumour located around the L2/3 spinal canal and extending into the left iliopsoas muscle. Angiolipoma was suspect…

BACKGROUND AND OBJECTIVE: Orthobiologics play a critical role in contemporary spine surgery. While autologous bone graft remains the gold standard for spinal fusion, its use is limited by donor site pain and morbidity, finite graft volume, and reduced osteoinductive capacity in the mineralized state. A range of orthobiologic materials has been developed to address these limitations. The objective of this narrative review is to summarize the orthobiologic options currently used in spine surgery and outline their key features, advantages, and limitations.
METHODS: A nonsystematic review of the …

Spinal abscesses are normally confined to the epidural space. Due to the anatomical seclusion of the intradural space, it is rare for the infection to spread to the area or for the cause to be idiopathic, iatrogenic in nature, or due to another phenomenon. We report a case of a 45-year-old male who was found to have a rare intradural spinal abscess two days post-laminectomy for severe central canal stenosis.

Background/Objectives: A superinfection occurs when a new, secondary organism colonizes an existing infection. Spine infections are associated with high patient morbidity and sometimes require multiple irrigations and debridements (I&Ds). When multiple I&Ds are required, the risk of complications increases. The purpose of this study was to report our experience with spine superinfections and determine which patients are typically affected. Methods: A retrospective case series of spine superinfections and a retrospective case-control analysis were conducted. Data were collected manually from el…

Osteobiologics are increasingly used in orthopedics and spine surgery to facilitate bone healing and prevent nonunion. Synthetic osteobiologics are artificial materials crafted in laboratories that aim to replicate the natural composition and functionality of bone. Notable materials such as calcium phosphate and calcium sulfate are engineered to mirror the mineral aspect of bone. They mimic human bone functionality, exhibiting osteoconductive, osteoinductive, and osteogenic properties. These characteristics promote cell attachment, migration, recruitment, and differentiation. Consequently, syn…

This case report details a rare instance of calcium pyrophosphate dihydrate crystal deposition disease (CPPD), commonly known as pseudogout, affecting the lumbar spine. A man in his mid-50s of age presented with severe low-back pain and fever, initially suspected as a spinal infection. Elevated erythrocyte sedimentation rate and leucocytosis were observed, while the initial imaging showed only lumbar spondylosis with arthritic changes in the right L4-L5 facet joint. However, an MRI revealed a cystic lesion at the right L5-S1 facet joint without signs of spondylodiscitis. Ultrasound-guided need…

INTRODUCTION: Intervertebral discs are part of the vertebral column and are considered the pads that help in cushioning and flexibility. These discs consist of nucleus pulposus, annulus fibrosus, and cartilage end plate and their major functions include spinal motion and loading. However, they are prone to numerous pathologies such as intradiscal hematoma and discal cyst formation that may significantly alter the biomechanics of the spine and hence the quality of life of the patient. This case report aims to shed light on these rare pathologies.
CASE PRESENTATION: The following is a report on…

Injuries of the spine and nerve root may occur during spine or spinal cord surgery, which can induce serious neurological deficits. Intraoperative monitoring plays important roles in monitoring the nerve function in various surgical manipulations, such as surgical positioning, mechanical compression, and tumor removal. This monitoring issues a warning to neuronal injuries at an early stage, and surgeons are able to prevent postoperative complications. The appropriate monitoring systems should be chosen with compatibility to the disease, surgical procedure, and lesion localization. The team sho…

Spine metastases are a common cause of pain in the oncologic patient which can generate functional limitation, in addition to complications derived from spinal cord compression, radicular compression and fractures. A complex approach to these metastases is required due to the risk of permanent sequelae. With the increase in survival rates due to new emerging treatments, the possibilities of presenting vertebral metastases are increasing, therefore, the management of these lesions should be aimed at pain relief and maintenance of ambulation. Radiotherapy has a fundamental role in the management…

PURPOSE: To summarize the mechanical loading of the spine in different activities of daily living and sports.
METHODS: Since the direct measurement is not feasible in sports activities, a mathematical model was applied to quantify spinal loading of more than 600 physical tasks in more than 200 athletes from several sports disciplines. The outcome is compression and torque (normalized to body weight/mass) at L4/L5.
RESULTS: The data demonstrate high compressive forces on the lumbar spine in sport-related activities, which are much higher than forces reported in normal daily activities and wor…

Tibial spine fractures (TSFs) are avulsion fractures at the site where the anterior cruciate ligament inserts onto the tibial eminence. TSFs typically affect children and adolescents aged 8-14 years. The incidence of these fractures has been reported to be approximately 3 per 100,000 per year, but the rising involvement of paediatric patients in sporting activities is increasing the number of these injuries. TSFs are historically classified on plain radiographs according to the Meyers and Mckeever classification system, which was introduced in 1959, but the renewed interest in these fractures …

Spine implants are becoming increasingly diversified. Taking inspiration from other industries, three-dimensional modeling of the spinal column has helped meet the custom needs of individual patients as both en bloc replacements and pedicle screw designs. Intraoperative tailoring of devices, a common need in the operating room, has led to expandable versions of cages and interbody spacers.

Tuberculosis of the cervical spine is a rare but deadly form of tuberculosis (TB), where the infection affects the cervical vertebrae or bones of the neck. It is also known as Pott's disease. The causative organism is Mycobacterium tuberculosis. It usually occurs when TB bacteria spread from other body parts, such as the lungs, to the cervical vertebrae through the bloodstream or lymphatic system. It also shows extrapulmonary involvement, including the central nervous, genitourinary, and lymphatic systems, bones, and joints. Tuberculosis of the spine is often seen. Cervical spine tuberculosis …

Hip-spine syndrome refers to concurrent hip and spine pathology with overlapping symptoms. Most of the literature has studied it in relation to total hip arthroplasty literature and has been shown to increase dislocation risk. Lumbar spine and pelvic mobility have been studied less frequently in relation to nonarthritic pathologic hip states. Understanding the biomechanical relationship between the lumbar spine, pelvis, and hip can help elucidate how hip-spine syndrome affects the nonarthritic hip and how it impacts outcomes of hip arthroscopy. Changes in lumbar spine motion may be the reason …

Bone grafting replaces damaged or missing bone with new bone and is used for surgical arthrodesis. Patients benefit from a huge variety of bone graft techniques and options for spinal fusions. This article reviews the rich history of bone grafts in surgery with particular emphasis on spinal fusion. During the early years of bone grafting in spine surgery, bone grafts were used on tuberculosis patients, and the structural support of the graft was most the important consideration. Between 1960 and 2000, many advances were made, specifically in the use of bone graft substitutes. The field of bone…
