Transcranial focal current stimulation in super-refractory status epilepticus in a pregnant patient.

Super-refractory status epilepticus (SRSE) during pregnancy is a rare, life-threatening condition with limited therapeutic options. Transcranial focal current stimulation (TFS) has shown benefit in refractory epilepsy, but its use in SRSE during pregnancy has not been reported. We describe a woman in her 20s at 12 weeks' gestation with longstanding focal drug-resistant epilepsy who developed SRSE despite escalation of antiseizure medications and multiple intravenous anaesthetic regimens, requiring prolonged intensive care and mechanical ventilation. Neuroimaging revealed frontal gliotic lesion…

Stage 1 Plus: A phenotype-based framework for rational polytherapy in status epilepticus.

Current guidelines for status epilepticus (SE) prioritize treatment with a benzodiazepine (BDZ), followed, if seizures continue, by sequential stepwise therapy. Yet this pragmatic framework does not fully capture the biological heterogeneity apparent at first clinical evaluation. Some patients are treatment-naive but may already be at increased risk of early BDZ failure, because duration, semiology, etiology, or delayed recognition have moved the disorder into a less responsive state, in the context of time-dependent receptor trafficking and evolving pharmacoresistance. These presentations can…

In Vivo Longitudinal Mapping of Brain Iron Accumulation After Pilocarpine-Induced Status Epilepticus.

Iron accumulations have been identified in resected tissue from patients with refractory temporal lobe epilepsy. These deposits are linked t … ty to detect the iron accumulations in vivo. In this study, we investigated how pilocarpine-induced status epilepticus contributes to the generation of iron deposits in diverse cerebral regions and whether QSM can detect these deposits longitudinally. We scanned 14 animals (n = 10 experimental and n = 4 control) at five different time points (pre-status epilepticus …

Non-convulsive status epilepticus revealing acute deterioration in AARS2 mutation-associated progressive leukoencephalopathy.

Nonconvulsive status epilepticus (NCSE) is defined as a condition of prolonged epileptiform activity on electroencephalography (EEG) accompanied by altered mental status without prominent motor manifestations. It is frequently underdiagnosed, particularly in patients with pre-existing cognitive impairment or neurodegenerative disorders [1, 2]. Reports describing NCSE in patients with AARS2-related leukoencephalopathy remain extremely limited. This case identifies NCSE as a potential cause of clinical deterioration in AARS2-related leukoencephalopathy and highlights the importance of considerin…

Gastrointestinal symptoms are common in patients with new-onset refractory status epilepticus (NORSE).

BACKGROUND: New-onset refractory status epilepticus (NORSE) is a rare clinical presentation in which patients develop refractory status epilepticus without a clear cause. Febrile infection-related epilepsy syndrome (FIRES) is a subset of NORSE preceded by febrile illness. Prolonged critical illness and treatments carry a risk of gastrointestinal (GI) disturbance. This study aimed to characterize GI symptoms in patients with NORSE. METHODS: This is a multicenter retrospective study of NORSE patients. Data from initial hospitalization and follow-up was collected via REDCap. Analysis was perform…

New onset refractory status epilepticus diagnosed in the second trimester: A case report.

New-onset refractory status epilepticus (NORSE) is a rare condition in which a previously healthy individual develops refractory seizures without an identifiable cause. In pregnancy, management is particularly challenging due to the need to control seizures while minimizing teratogenic risk for the fetus. We report a 22-year-old woman who developed NORSE at 19 weeks' gestation following recurrent tonic-clonic seizures. Treatment included multiple antiseizure medications: levetiracetam, oxcarbazepine, lacosamide, clobazam, and lamotrigine. Due to super-refractory status, she required intubation…

Atypical Presentation of Seronegative Autoimmune Encephalitis as Refractory Status Epilepticus: A Case Report.

BACKGROUND Autoimmune encephalitis (AIE) is considered one of the challenging neurological diseases marked by various neuropsychiatric symptoms. These symptoms can manifest as cognitive deficits, seizures, and status epilepticus. Diagnosis can often be complex due to the diverse clinical presentation and the absence of detectable neuronal autoantibodies. CASE REPORT We report the case a 27-year-old man who presented with generalized tonic-clonic seizures, headache, and fever, initially treated as infectious meningoencephalitis. He was given extensive antiviral and antibiotic treatment, but his…

Reducing Time to Electroencephalography in Pediatric Convulsive Status Epilepticus: A Quality Improvement Initiative.

BACKGROUND: Pediatric convulsive status epilepticus (CSE) is a neurological emergency utilizing electroencephalography (EEG) to guide therapeutic interventions. Guidelines recommend EEG initiation within one hour of seizure onset, but logistic and structural barriers often lead to significant delays. We aimed to reduce the time to EEG in pediatric CSE. METHODS: From 2017 to 2022, we implemented process improvements, including EEG order sets with priority-based timing guidance, technologist workflow changes, a satisfaction survey, and feedback from key stakeholder groups, over five plan-do-stu…

Sex Differences in Organophosphate Model of Benzodiazepine-Refractory Status Epilepticus and Neuronal Damage.

Sex differences are common in human epilepsy. Although men are more susceptible to seizure than women, the mechanisms underlying sex-specific vulnerabilities to seizure are unclear. The organophosphate (OP) diisopropylfluorophosphate (DFP) is known to cause neurotoxicity and status epilepticus (SE), a serious neurologic condition that causes prolonged seizures and brain damage. Current therapies for OP poisoning and SE do not consider neuronal variations between male and female brains. Therefore, we investigated sex-dependent differences in electrographic seizure activity and neuronal injury u…

Nonconvulsive Status Epilepticus With Generalized Spike-and-Wave Discharges: Pathophysiological and Nosological Considerations.

Absence status epilepticus (ASE) is the most common type of status epilepticus in patients with idiopathic generalized epilepsy (IGE). Like absence seizure, ASE is characterized by generalized spike-and-wave discharges (GSWDs) on the electroencephalogram (EEG). Once considered specific for IGE, GSWDs have increasingly been observed in other forms of epilepsy, as well as in patients with no prior epilepsy. Here, we report three patients with different types of nonconvulsive status epilepticus (NCSE) in which the EEG correlate was GSWDs: a 44-year-old woman with juvenile absence epilepsy who man…

Valproic acid as adjuvant treatment for convulsive status epilepticus: a randomised clinical trial.

BACKGROUND: Generalised convulsive status epilepticus (GCSE) is a medical emergency. Guidelines recommend a stepwise strategy of benzodiazepines followed by a second-line anti-seizure medicine (ASM). However, GCSE is uncontrolled in 20-40% patients and is associated with protracted hospitalisation, disability, and mortality. The objective was to determine whether valproic acid (VPA) as complementary treatment to the stepwise strategy improves the outcomes of patients with de novo established GCSE. METHODS: This was a multicentre, double-blind, randomised controlled trial in 244 adults admitte…

Teaching Video NeuroImage: Status Epilepticus Manifesting as Belly Dancing Secondary to Subdural Hematoma.

A 74-year-old man developed involuntary rhythmic contractions of his left abdomen, after drainage of a chronic right frontoparietal subdural hematoma (Figure). These movements had electroencephalographic correlation with periodic lateralized discharges over the right posterior quadrant (Video 1, Figure) and were classified as clonic abdominal seizures. Clonic abdominal seizures are a rare clinical finding in patients with seizure disorders. The symptomatogenic zone most commonly localizes to the contralateral paracentral frontoparietal region.1 Possible etiologies include primary brain tumors,

Vascular topology and blood flow are acutely impacted by experimental febrile status epilepticus.

Febrile status epilepticus (FSE) is an important risk factor for temporal lobe epilepsy and early identification of those at high risk for epilepsy is vital. In a rat model of FSE, we identified an acute (2 hrs) novel MRI signal where reduced T2 relaxation values in the basolateral amygdala (BLA) predicted epilepsy in adulthood; this T2 signal remains incompletely understood and we hypothesized that it may be influenced by vascular topology. Experimental FSE induced in rat pups reduced blood vessel density of the cortical vasculature in a lateralized manner at 2 hrs post FSE. Middle cerebral a…

Valproate in status epilepticus: Correlation between loading dose, serum levels, and clinical response.

BACKGROUND AND PURPOSE: Intravenous valproate (VPA) is an established treatment of status epilepticus (SE), but optimal loading dose was not fully assessed. We aimed at analyzing the correlation between VPA loading dose and subsequent plasma levels with clinical response in SE. METHODS: This was a retrospective study in one referral center of all consecutive VPA-naïve SE episodes treated with VPA between January 2013 and June 2019, in which total VPA trough plasma levels after intravenous loading dose were available. Response to VPA, defined as last antiseizure medication introduced before SE…

Bromisoval-induced bromism with status epilepticus mimicking Wernicke's encephalopathy: report of two cases.

BACKGROUND: Bromine compounds are used in several drugs, including over-the-counter drugs. They sometimes cause intoxication known as bromism. Although the acute neurological symptoms and sequelae of bromism vary, few reports have mentioned acute encephalopathy. CASE PRESENTATION: We report two cases of bromisoval-induced bromism with status epilepticus. Presence of pseudohyperchloremia and history of over-the-counter medication use guided the diagnosis. In the acute phase, our patients showed bilateral medial thalamic lesions on magnetic resonance imaging. The imaging findings were similar t…
