Convergent strabismus fixus after bilateral abducens nerve palsies due to aneurysms: A case report.

RATIONALE: Convergent strabismus fixus is an ocular motor abnormality in which the eye is fixed in adduction. This condition is mostly associated with high myopia and is caused by a displacement of the extraocular muscles. We report a nonmyopic woman with convergent strabismus fixus due to aneurysms. PATIENT CONCERNS: A 79-year-old woman complained of progressive worsening of esotropia about 50 years prior to her visit. Neuroimaging showed that the eye was not dislocated, and the extraocular muscles were not displaced. However, aneurysms were found bilaterally from the intracavernous carotid …

Magnetic resonance imaging in dissociated strabismus complex demonstrates generalized hypertrophy of rectus extraocular muscles.

BACKGROUND: Dissociated strabismus complex (DSC) is an enigmatic form of strabismus that includes dissociated vertical deviation (DVD) and dissociated horizontal deviation (DHD). We employed magnetic resonance imaging (MRI) to evaluate the extraocular muscles in DSC. METHODS: We studied 5 patients with DSC and mean age of 25 years (range, 12-42 years), and 15 age-matched, orthotropic control subjects. All patients had DVD; 4 also had DHD. We employed high-resolution, surface coil MRI with thin, 2 mm slices and central target fixation. Volumes of the rectus and superior oblique muscles in the …

Functional and Motor Outcomes of Strabismus Surgery for Chronic Isolated Adult Sixth Nerve Palsy.

Abducens nerve palsy is the most common acquired ocular motor nerve palsy in adults. Chronic cases of abducens palsy often require surgical intervention to relieve disabling diplopia. The goal of this study was to identify factors associated with surgical outcomes in isolated abducens palsy. Medical records of all adult patients from 1988 to 2012 with abducens palsies who underwent strabismus procedures were retrospectively reviewed. Motor alignment, extraocular motility, and sensory outcomes were recorded. Success was defined as absence of diplopia without prisms or face turn, vertical deviat…

Time trade-off utility analysis for surgical intervention in comitant strabismus, glaucoma, and cataract.

The utility value was compared among 3 surgical interventions, and the validity of the time trade-off (TTO) method was evaluated by analyzing the correlations of the utility value with the results of the Visual Function Questionnaire-14 (VF-14) and other variables. The subjects were 127 patients aged 40-85 years who were surgically treated between January 2008 and March 2010, including 26 patients with glaucoma, 50 with cataracts, and 51 with comitant strabismus. The scores on VF-14 and utility values determined using TTO were calculated retrospectively. The mean value (SD) of the utility gain…

Severe strabismus after scleral buckling: multiple mechanisms revealed by high-resolution magnetic resonance imaging.

PURPOSE: Strabismus occurring after scleral buckling for retinal detachment typically is attributed to scarring, but this is not the only mechanism. We investigated use of magnetic resonance imaging (MRI) to identify a spectrum of mechanisms of strabismus occurring after scleral buckling. DESIGN: Prospective, noncomparative, observational case series in an academic referral setting. PARTICIPANTS: Six patients with long-standing, large-angle strabismus after monocular or binocular scleral buckling. Horizontal and vertical preoperative deviations, present from 8 to 120 months, ranged from 25 p…

Force-length recording of eye muscles during local-anesthesia surgery in 32 strabismus patients.

Force-length recordings were made from isolated human eye muscles during strabismus surgery in local, eye-drop anesthesia in 32 adult patients. From each muscle three recordings were made: (I) while the patient looked with the other eye into the field of action of the recorded muscle, (2) looked ahead, and (3) looked out of the field of action of the recorded muscle. Non-innervated eye muscles (state 3) had an approximately exponential relation between force and length. During contraction evoked by letting the patient look ahead or into the field of action of the muscle (states 1 or 2), the re…

[Comparative refractioning in juvenile strabismus patients. Experience with the Auto-Refractor 6600 (author's transl)].

122 strabismus patients have been refracted in order to compare the cycloplegic effect of Cyclopentolat-HCl (Zyklolat) and atropin. In young patients the cycloplegic effect of Zyklolat is not as sufficient as that of atropin is. Moreover the positions of cylindrical axes show differences up to 30 degrees under Zyklolat in comparison with atropin. On the other hand we give a report of our experience using the Auto-Refractor 6600, the before-mentioned measurings were performed with. -- The important field of the Auto-Refractor is the routine refractioning of co-operative patients. Using it on yo…

"Tissue-drag" with polyglycolic acid (Dexon) and polyglactin 910 (Vicryl) sutures in strabismus surgery.

The new absorbable synthetic sutures, polyglycolic and (Dexon) and Polyglactin 910 (Vicryl), are excellent sutures for strabismus surgery, but they have a bothersome tendency to attract adjacent tissue (tissue-drag). This difficulty is found especially with the 5-0 size suture, less so with the 6-0 size suture, and minimally with the 7-0 size suture. The presence and number of short, stubbly, protuberant strands (hooklets) seen on the surface of these sutures under high magnification correlates with the lack of ease with which the sutures pass through tissue. Other factors involved in tissue-d…

Neuromuscular Blockade Depth and Oculocardiac Reflex in Pediatric Strabismus Surgery: A Randomized Clinical Trial.

BACKGROUND: Although neuromuscular blockade reduces oculocardiac reflex (OCR) incidence compared with no neuromuscular blockade, the effect of blockade depth on OCR has not been prospectively evaluated. This study aimed to evaluate whether the depth of neuromuscular blockade influences the incidence of the OCR during pediatric strabismus surgery. METHODS: In this prospective, double-blinded randomized controlled trial, 204 pediatric patients aged 3 to 18 yr undergoing strabismus surgery under general anesthesia were randomly assigned to moderate to deep neuromuscular blockade (MD-NMB; train-o…

Transcriptional profiling of extraocular motor neurons reveals sim1a as a candidate strabismus-related gene.

Strabismus, or misalignment of the eyes, is a heritable disorder frequently associated with vision loss and decreased quality of life. Incomitant strabismus, where the degree of misalignment differs based on gaze angle, can arise from mutations in genes that regulate the development of extraocular motor neurons. To date, few such genes have been identified. The extraocular motor system is highly conserved across vertebrates, suggesting a comparative transcriptomic discovery approach would be fruitful. Using bulk and single-cell sequencing in a small accessible vertebrate, the larval zebrafish,…
