Controversies related to renal artery denervation and devices.

PURPOSE OF REVIEW: This review article discusses the controversies, strengths, and limitations of the current literature on renal artery denervation in the management of resistant hypertension, as well as the future directions of this intervention. RECENT FINDINGS: There have been conflicting data from the different randomized control trials assessing the efficacy of renal artery denervation in the management of resistant hypertension. SUMMARY: Renal artery denervation is achieved by ablating the sympathetic nerves surrounding the renal arteries using endovascular ultrasound, radiofrequency,

Cardiac sympathetic denervation for untreatable ventricular tachycardia in structural heart disease. Strengths and pitfalls of evolving surgical techniques.

Cardiac sympathetic denervation (CSD) is a valuable option in the setting of refractory ventricular arrhythmias in patient with structural heart disease. Since the procedure was introduced for non structural heart disease patients the techniques evolved and were modified to be adopted in several settings. In this state-of-the-art article we revised different techniques, their rationale, strengths, and pitfalls.

Histological Justification for the Need of Radiofrequency Ablation of Pulmonary Arteries in Patients with High-Grade Secondary Pulmonary Hypertension.

UNLABELLED: The aim of the study was to perform a histological assessment of the effectiveness of radiofrequency exposure for circular denervation of the pulmonary artery in patients with secondary high pulmonary hypertension. MATERIALS AND METHODS: The study was carried out on the autopsy material derived from non-operated patients. Three groups were formed. The experimental group included the material (207 histological samples) from the patients with chronic high pulmonary hypertension arising on the background of mitral heart disease. The samples of this group were exposed to circular radi

Ablation of Neuroaxial in Patients with Ventricular Tachycardia.

Ventricular tachycardia (VT) remains a common cause of sudden cardiac death. It is widely accepted that VTs are strongly associated with autonomic imbalance with reduced vagal and increased sympathetic activities. Pharmacologic therapy remains the first-line therapy, but antiarrhythmic agents may not be effective or carry significant side effects. Sympathetic denervation is an emerging therapy to prevent or treat VTs by rebalancing the sympathetic and parasympathetic activity. This article focuses on the role of sympathetic activation in VT, and the mapping and ablation of sympathetic nervous

Renal Sympathetic Denervation in Rats.

Experimental renal sympathetic denervation is a well-established technique. Classically, renal sympathetic denervation is achieved by dorsal rhizotomy. While more recently, direct renal sympathetic denervation is typically applied by stripping all visible renal nerve bundles followed by painting with a solution of 10 % phenol in ethanol to remove the remaining nerves. In clinical settings, a reliable marker of renal sympathetic denervation or renal sympathetic overactivity has not been established. However, in experimental models, successful renal sympathetic denervation is validated by a decr

Reversal of sympathetic interruption by removal of clips.

BACKGROUND: Sympathetic interruption offers excellent control of hyperhidrosis and facial blushing (FB), but some patients have side effects … through removal of clips (Table 1). CONCLUSIONS: The percentage of patients requesting reversal of sympathectomy is significant and appears to most commonly result from compensatory sweating (CS); however, excessive palmar dryness also occurs in those without primary palmar hyperhidrosis. It appears that the best results with reversal are obtained when the cli …

One swallow does not a summer make: safety and renal denervation.

Renal denervation is increasingly being adopted as a treatment option in patients with resistant hypertension. The long-term safety of this procedure is unknown. Though the procedure interrupts the sympathetic nerves at the renal level, it also has effects on other organ beds, notably the heart and vasculature. These effects have been purported to be clinically beneficial and thus formed a rationale for examining the role of renal denervation in other conditions, including heart failure, arrhythmia, obstructive sleep apnoea and the metabolic syndrome. There is a theoretical concern that attenu

Renal denervation in the treatment of hypertension.

Despite advances in nonpharmacologic and pharmacologic therapy, blood pressure control rates in hypertension are low. About 10 % of patients with hypertension fulfill the criteria of therapy resistance, which is defined as noncontrolled blood pressure despite treatment with ≥3 antihypertensive drugs of different classes, including a diuretic, at optimal or maximal tolerated doses. Although the pathogenesis of resistant hypertension is multifactorial, an interaction between renal afferent and efferent sympathetic nerves and the central nervous system plays a key role, leading to increased renal

[Possible complications of transpleural neurotomy].

The occurrence of complications in course of thoracoscopic vegetative denervation has been found low in the author's analysis of his 2679 cases. There arose several small lung lesions with consecutive pneumothorax or tension pneumothorax, furthermore some events of emphysema of the skin, one bleeding of an intercostal artery, which was stopped under thoracotomy, one division of the thoracic duct, one thrombosis of the anterior spinal artery, one cardiac failure. On the other hand there could be avoided air embolism, Horner's syndrome, lesions of recurrent and phrenic nerve, damage on trachea,

Protective Effect of Gastric Distension Preconditioning on Renal Ischemia/Reperfusion Injury in Rats.

The stomach mechanoreceptors can be stimulated by gastric distension (GD) and through afferent vagal nerve, increased activity of the renal sympathetic pathways. Because renal sympathectomy can abolish the protective effect of ischemic preconditioning, it seems that GD preconditioning can be effective in renal ischemia/reperfusion (I/R) injury. Gastric inflate (8 ml of 37°C water for 20 min) by a latex balloon inserted into the stomach through the fundus; I/R group was subjected to 45 min of bilateral ischemia and 24 h of reperfusion. GD preconditioning decreases blood urea nitrogen, creatinin…

A case of dilated cardiomyopathy successfully managing refractory electrical storm through sympathetic modulation.

UNLABELLED: Recent research has demonstrated the effectiveness of cardiac sympathetic denervation in treating ventricular arrhythmias. We present a patient with dilated cardiomyopathy characterized by ventricular tachycardia (VT) resistant to standard treatments. Repeated left-sided stellate ganglion blocks provided temporary relief, followed by a thoracoscopic sympathectomy with T2-4 ganglionectomy, which significantly reduced the burden of VT. This intervention also allowed for the gradual reduction of antiarrhythmics. During the course of the hospital stay, the patient's activities of daily…

Protective effect of spinal cord stimulation on experimental early cerebral vasospasm. Conclusive results.

In this study we investigate the effects of cervical spinal cord stimulation (cSCS) on experimental 'early spasm' in rabbits as described in … ) up to the end of the experiments in all the stimulated animals. The role of reversible functional sympathectomy in mediating the effect of spinal cord stimulation on early spasm is discussed. cSCS is able to prevent 'early spasm' due to SAH in all the animals studied, independently from the occurrence and the sign of stimulation induced CBF variations.

Immunocytochemical and circadian biochemical analysis of neuroactive amino acids in the pineal gland of the rat: effect of superior cervical ganglionectomy.

Semiquantitative immunocytochemistry by immuno-gold techniques revealed differences in the spatial distribution of glutamate, glutamine, and … taurine. Other amino acids (alanine, arginine, aspartate, serine) were reduced by 23%-33% following sympathectomy. These data suggest an important role for glutamate in pinealocyte function(s) possibly related to the noradrenergic innervation of the gland.

Morphological changes in the rabbit submandibular gland after parasympathetic or sympathetic denervation.

Selective denervation was used to obtain further information about the relative roles of the nerves supplying the gland. Parasympathetic denervation soon caused atrophy in both acinar and granular tubule cells, and a substantial reduction in gland wet weight. The secretory cells had several unusual features, and striated duct cells tended to accumulate glycogen. Three to four weeks after pre-ganglionic parasympathectomy, the parenchymal cells were less abnormal but still smaller than in the control, unoperated gland. Thus, parasympathetic impulses are required to maintain normal parenchymal ce…
