We report a case of distal radius fracture after a Sauvé-Kapandji procedure combined with synovectomy and tendon transfer in a rheumatoid arthritis patient. This case shared several unusual features that were also seen in a previously reported case. Based on these features, we discuss favorable surgical treatment for the rheumatoid wrist with extensor tendon rupture, and also the optimal treatment for distal radius fracture after such procedures.

Osteochondromas usually arise from the metaphyses of long bones. Involvement of the carpal bones is extremely rare. We report a case of an osteochondroma originating from the dorsal capitate in a 51-year-old man. Attritional rupture of the extensor digiti minimi tendon resulted from mechanical irritation as it coursed over the mass. Surgical excision of the mass with tendon transfer of the extensor digiti minimi was subsequently performed.

Symptomatic irrepairable rotator cuff tears usually entail complete loss of the substance of the supraspinatus and infraspinatus tendons. Loss of external rotation control and cranial migration of the humeral head on attempted flexion or abduction of the shoulder are the functional hallmarks. Transfer of the latissimus dorsi tendon from the humeral shaft to the superolateral humeral head provides a large, vascularized tendon that can be used to close a massive cuff defect and that exerts an external rotation and head-depressing moment that allow more effective action of the deltoid muscle. Thirty-one patients who were unable to abduct the involved arm after reduction of a primary anterior dislocation of the glenohumeral joint were found to have a ruptured rotator cuff. All of the patients were more than thirty-five years old. Twenty-nine of them were initially presumed to have had an injury to the axillary nerve, although this injury was confirmed in only four of the twenty patients who had electrodiagnostic studies. In eight patients, the subscapularis tendon and anterior part of the capsule had ruptured from the lesser tuberosity. Recurrent instability developed in all eight p

CASE: A 73-year-old woman presented with wrist pain and loss of extension in the middle and ring fingers. Radiography revealed a dorsally displaced lunate fragment, resulting in a diagnosis of Kienböck disease with extensor tendon rupture. Artificial lunate replacement and tendon transfer were performed as treatment. Two years postoperatively, the pain was relieved, and the extension lag was 0°. The wrist motion and carpal height had also improved.
CONCLUSION: Lunate excision, partial wrist arthrodesis, or proximal row carpectomy are known treatments for Kienböck disease with extensor tendon …

Surgical treatment of progressive collapsing foot deformity (PCFD) relies on understanding the dynamic and deforming musculotendinous structures that contribute to hindfoot valgus, forefoot abduction, forefoot varus, and collapse or hypermobility of the medial column. Equinus commonly is seen in PCFD and consideration should be given to isolated gastrocnemius or Achilles lengthening. Although transfer of the flexor digitorum longus tendon is performed in PCFD attributed to dysfunction and pathology of the posterior tibialis tendon (PTT), retention of PTT is an area for further research. The pe

Rupture of the extensor pollicis longus tendon at the tip of a prominent fixation screw occurred in two cases after fixation of fractures of the radial shaft and in one case after fixation of a fracture of the scaphoid. Patients were referred because of their inability to extend the thumb. Active thumb extension was restored in two patients by screw removal and tendon transfer. The third patient elected nonoperative care. Close scrutiny of good-quality intraoperative x-ray films after internal fixation about the wrist may detect screws that are too prominent and thereby prevent rupture of the …

Rupture of the posterior tibial tendon results in a progressive, painful flat-foot deformity. Other physical findings are an inability to invert the heel on tip-toe standing and loss of inversion power. In this study we report on seventeen patients with this condition who were surgically treated with a transfer of the flexor digitorum longus tendon into the navicular or an advancement of the posterior tibial tendon. Follow-up results were excellent in twelve, good in one, fair in three, and poor in one patient. Arthrodesis of the talonavicular and calcaneocuboid joints was subsequently done fo

BACKGROUND: Tendon rupture has been recognized as a complication of distal radius fracture (DRF); however, the clinical outcome of reconstru … ured tendons received a free tendon graft from the palmaris longus tendon, and the others underwent tendon transfers. All patients were managed postoperatively by our hand therapy unit according to a controlled active mobilization regime.
RESULTS: The mean follow-up period was 28 weeks (range: 12-80 weeks). Patients with extensor tendon ruptures were significant …

There are multiple techniques to treat tendon defects in the event end-to-end repair cannot be achieved after débridement. In general, the choice of treatment technique is based on size of the resultant gap. Although each treatment technique has literature to support its use, there are no data to support the use of one technique over another. Treatment should be based on the experience and discretion of the treating surgeon. This article proposes an algorithm for wound breakdown, infection, and rerupture after Achilles tendon surgery. This algorithm should be used as a guide.

BACKGROUND: Brachial plexus injuries (BPI) can be devastating for patients, often resulting in notable physical, psychological, and socioeco … bow flexion.
ALTERNATIVES: Surgical alternatives include nerve transfers (e.g., Oberlin transfer), tendon transfers, or other free muscle transfers (e.g., latissimus dorsi transfer). Nonsurgical alternatives include orthotic devices to compensate for elbow flexion loss, and physical therapy to maximize existing function.

Chronic Achilles tendon ruptures typically are treated with surgical intervention except in low-demand patients or patients who are unable t … with direct end-to-end repair. Medium-sized gaps can be treated with tendon-lengthening procedures. Tendon transfers, autograft, allograft, xenograft, and synthetic grafting are described for the reconstruction of large defects.

INTRODUCTION: Spontaneous rupture of the extensor pollicis longus (EPL) tendon has been reported in the literature. Various mechanisms have been proposed to account for this problem, but gouty infiltration is a rare mechanism. Here we report a patient with a long-standing history of gout who presented with sudden loss of interphalangeal extension of the left thumb. Spontaneous rupture of the EPL tendon caused by gout was discovered.
MATERIALS AND METHODS: The successful treatment done involved surgical exploration and extensor indicis proprius tendon transfer. Postoperative thumb spica immobi…

We led a retrospective study to determine the causes of the tendon ruptures post-operating in the surgery of the wrist rheumatoid dorsal and to estimate the clinical result. At follow-up, we measured the extension lag and the rolling-up of fingers by the distance palm-pulps. Nine patients were so revised in the average of 40 months, average age was of 50.7 years. The tendon ruptures arose in 3 months in 67% of the cases. Seven times, a procedure on the distal radio-ulnar joint had been necessary (5 Sauvé-Kapandji and 2 Darrach). Thirty tendons had been concerned in this study, that is 3.3 tend

A spontaneous rupture of the extensor pollicis longus (EPL) tendon after a fracture of the distal radius is a known complication in adults. In contrast, there are a paucity of reports concerning EPL tendon ruptures in children and adolescents. The authors present a case of a spontaneous rupture of the EPL tendon in a 15-year-old girl after a non-displaced distal radius fracture. The patient had no predisposing factors including rheumatoid arthritis or steroid injection. During surgery, the EPL tendon was found to be ruptured at the extensor retinaculum (third compartment). We previously reported the beneficial effects of tendon transfer to eliminate extension lag of the interphalangeal joints, using the extensor carpi radialis longus prolonged by palmaris longus tendon grafts after crushing-penetrating injuries around the metacarpophalangeal (MP) joint of the middle finger. We used the flexor digitorum superficialis (FDS) as the alternative donor muscle and treated two cases of severe crushing injuries to MP joint, and then obtained good outcomes.
