Nonarticular rheumatism.

Nonarticular rheumatism refers to a group of conditions characterized by musculoskeletal aches and pains which do not arise from joints. A number of different entities are considered under this definition. They are classified either as generalized conditions which include polymyalgia rheumatica, fibrositis and aches and pains NYD, or localized conditions which include the enthesopathies: tendonitis, epicondylitis and fasciitis, tenosynovitis and bursitis. The treatment of most of these conditions is expectant, because they tend to be self-limiting and improve despite therapeutic interventions.…

Ultrasound of shoulder and knee improves the accuracy of the 2012 EULAR/ACR provisional classification criteria for polymyalgia rheumatica.

OBJECTIVE: Recent studies suggest that the knee is frequently involved in PMR. In this study, we aimed to determine whether the US assessmen … ll patients underwent comprehensive US assessment of the shoulder and knee for synovitis, bursitis, tenosynovitis, tendinitis and ligament inflammation at baseline. RESULTS: US scores for tenosynovitis, tendinitis and ligament inflammation better discriminated the PMR-definite group from the PMR-mimic and RA groups than do those for synovitis or bursitis. Amon …

Reliability of the novel 7-joint ultrasound score: results from an inter- and intraobserver study performed by rheumatologists.

OBJECTIVE: To assess the inter- and intraobserver reliability of 26 rheumatologists when performing the 7-joint ultrasound score (US7). METHODS: Six patients with rheumatoid arthritis were examined by 26 sonographers in 12 rater groups who performed the US7 score. The US7 score includes the clinically dominant wrist, the second and third metacarpophalangeal (MCP) and proximal interphalangeal joints, and the second and fifth metatarsophalangeal (MTP) joints, which were evaluated for synovitis, tenosynovitis/paratenonitis, and erosions from the dorsal side and palmar/plantar aspects by gray-sca…

Rheumatic manifestations of diabetes mellitus.

DM is associated with various musculoskeletal manifestations. The strength of this relationship varies among the various musculoskeletal disorders; the associations are based mostly on epidemiologic data. For most of these conditions, definitive pathophysiologic correlates are lacking.Hand and shoulder disorders occur more frequently than other musculoskeletal manifestations of DM. Recognition of the association between DM and shoulder adhesive capsulitis, DD, and stenosing flexor tenosynovitis facilitates their correct diagnosis in the setting of DM and prompt initiation of appropriate treatm…

Disorders of the Flexor Hallucis Longus and Os Trigonum.

Os trigonum syndrome with disease of the flexor hallucis longus tendon, so-called stenosing flexor tenosynovitis, is a common cause of posterior ankle impingement. Conservative treatment is the recommended first line of treatment, with secondary treatment options of either open or arthroscopic os trigonum excision with flexor hallucis longus retinaculum release. The arthroscopic approaches have gained popularity in the past decade because of less scarring, less postoperative pain, minimal overall morbidity, and earlier return to activities. However, comprehensive understanding of the anatomy o…

Management of human and animal bite wounds: an overview.

Serious infections can result from wounds that are caused by bites from animals and humans. Organisms recovered from bite wounds generally originate from the oral cavity of the biting animal, as well as from the patient's skin flora. Anaerobes have been isolated from animal and human bite wound infections, especially those associated with abscess formation. In addition to local wound infections, common complications, such as lymphangitis, septic arthritis, tenosynovitis, and osteomyelitis, and uncommon complications, such as endocarditis, meningitis, brain abscess, and sepsis, may occur. Wound…

Tendoscopy in stage I posterior tibial tendon dysfunction.

Stage I PTTD was defined by Johnson and Strom as tenosynovitis or tendinitis whereby tendon length remains normal, there is no hindfoot deformity, and diagnosis is basically clinical, characterized by swelling and tenderness posterior to the medial malleolus. The PTT has a hypovascular zone 40 mm proximal to the insertion of the tendon and 14 mm in length. Pain often is localized to this portion of the tendon. Tendon power might be normal, and the patient can perform single heel rise, sometimes with slight discomfort. This condition is often misdiagnosed as ankle sprain, which delays correct d…

[Injection therapy of trigger finger].

BACKGROUND: Since 1996 we have treated patients with trigger finger with corticosteroid injection as an alternative to surgery, on the basis of good results from this treatment reported in the literature. We wished to evaluate the results. MATERIAL AND METHODS: 27 patients with 32 trigger fingers were treated with corticosteroid injection. Two injections had been used in 17 fingers. The patients were contacted by mail or telephone median 4 years and 2 months (range 29-56 months) later, and patients with persistent pain were examined clinically. RESULTS: 12 fingers had been operated by the ti

Cumulative trauma disorders.

The relationship between work activities and the diagnostic entities discussed in this article is poorly understood, but their development is probably multifactorial. The enormous cost to industry and society is driving many investigators to study the causes and pathologic manifestations of CTDs, and this research should lead to improved strategies for treating and preventing work-related injuries. Prevention, however, will work only if both management and labor participate in preventive efforts. When a task force approach can be implemented and senior management gives it the power to make eff

[Trigger thumb--stenosing tendovaginitis in childhood. Results of a 9-year application of subcutaneous tendon sheath discission].

"Trigger finger" in childhood has to be delimited from the corresponding disease in adults. The therapy is done by subcutaneous tendovaginal discission as the most optimal and for the child the least of all straining method of treatment. Surgical intervention is recommended to be done at the age of two years.

Comprehensive evaluation of bone structural and microarchitectural changes in rheumatoid arthritis treated with baricitinib using HR-pQCT, imaging, and biomarker analyses.

OBJECTIVES: To comprehensively evaluate the effects of baricitinib on bone structure and bone-related biomarkers in patients with rheumatoid arthritis (RA) using high-resolution peripheral quantitative computed tomography (HR-pQCT). METHODS: RA patients initiating baricitinib were enrolled in this prospective study. Bone structure in the second and third metacarpal heads was assessed using HR-pQCT at baseline and after 6 and 12 months. Synovitis, tenosynovitis, and osteitis were evaluated using ultrasound and MRI. Serum cytokines, chemokines, and bone-related biomarkers were analysed through …
