An aberrant right subclavian artery is an uncommon anomaly. When associated with esophageal cancer, it can cause diagnostic confusion as the symptoms are similar. If unrecognized and injured during esophageal surgery, it can lead to disastrous complications. We report a patient in whom this aberrant artery was injured during a thoracoscopic mobilization of the esophagus. The embryological and radiologic aspects of this anomaly and its clinical significance are discussed. Pre-operative diagnosis will require a high index of suspicion, as the clinical and radiological features are not specific.

Chondromyxoid fibromas (CMFs) are rare tumours that comprise less than 2% of benign bone tumours; approximately 5-7.9% occur in the ribs. Bone hemangiomas are slow-growing benign tumors, with rib involvement being exceptionally rare. Independent lung ventilation is often used intra-operatively but has also been used in intensive care in a variety of clinical situations. Ventilating lungs independently of each other may be life-saving when a lung abnormality is predominantly unilateral. We successfully used independent lung ventilation in two patients, who benefited from anatomical and physiological separation of their lungs. These cases remind intensivists that independent lung ventilation is a viable option to be considered in cases of pulmonary abnormality when conventional mechanical ventilation is deleterious.

Malignant "Triton" tumor is a rare subtype of malignant peripheral nerve sheath tumor showing rhabdomyoblastic differentiation. We report the case of a 22-year-old patient who had excision of such a tumor by video-assisted thoracic surgery. He was reoperated on by thoracotomy 2 months later and received adjuvant radiation, with prolonged survival. Radical surgical excision of malignant Triton tumor followed by adjuvant radiation therapy may provide the longest survival and represent the treatment of choice.

Two cases of intrathoracic neurogenic tumors are discussed. The benign neoplasms were located in the posterior mediastinum and caused no clinical symptoms. In both cases, complete resection of the lesion was achieved by video-assisted thoracic surgery (VATS). There were no intraoperative complications. Postoperatively, one patient presented with Claude Bernard-Horner syndrome, which resolved spontaneously after 1 week. Although the great majority of mediastinal neurogenic neoplasms are benign, resection is necessary to prevent malignant transformation and intraspinal extension.

Lipoma of the diaphragm is an extremely rare entity. A case of this disease is herein reported, together with the review of the literature. The patient described was a 70-year-old male, who was admitted to our hospital with an abnormal shadow on his chest X-ray film. The primary tumor, located above the left hemidiaphragm, was successfully resected by video-assisted thoracic surgery, and the final diagnosis of lipoma originating in the diaphragm was made.

Plasma-cell granuloma of the mediastinum is an unusual non-neoplastic lesion; a case of rare mediastinal localization is reported. Only the histopathological examination leads to a correct diagnosis. Since the possibility of local recurrence has been described, total excision is requested. Video-assisted thoracic surgery technique may be difficult to use in the management of these mediastinal tumors due to adhesions between the mass and the surrounding tissues.
