Lymphoid dendritic cells play an essential role in antigen presentation in primary immune responses and are believed to be important in normal healthy responses of the mucosal immune system. Advanced lesions of the oropharynx are known to have increased survival with surgical excision combined with radiation therapy. Traditional surgery involves either a mandibular resection or, more recently, a mandibular sparing approach primarily through a lip-splitting incision. At our institution posterior oropharyngeal lesions are approached via a combined intraoral and transhyoid technique. Lymphocytes in the lingual tonsil were analyzed using monoclonal antibodies to cell surface markers. Immunohistologic staining of lingual tonsillar tissue showed localization of the various lymphocyte subsets in the germinal center, mantle zone, and inter-follicular area. Populations of lymphocyte subsets, i.e., T cells, B cells, CD4 positive cells, CD8 positive cells, alpha beta T-cells, gamma delta T-cells, and NK cells were assessed by laser flow cytometry analysis, and we compared the following tissues: peripheral blood, lingual tonsils, palatine tonsils, and pharyngeal tonsils (adenoids).

The possibilities for using acoustical analysis as a noninvasive measure of the degree of obstruction created by an enlarged pharyngeal tonsil (adenoid) are discussed. The method capitalizes on the relationship which exists between a specific obstruction of the respiratory tract and the resulting respiratory sound. Tonsillectomy for enlarged tonsils has been so soundly criticized that doctors no longer date to suggest the operation. If they do come to the conclusion that the procedure in inevitable, it is only after an expensive and sometimes aggressive work-up. In order to better establish the criteria which should determine this indication for amygdalectomy, we studied a series of 47 patients undergoing this procedure for enlarged tonsils. Morphofunctional regularities in the development of the lingual tonsil were examined in 120 human embryos and fetuses of gestation age of 5 weeks to 9 months with the use of a complex of histologic and injection methods and transmission electron microscopy. Two 12-year-old boys with severe sleep apnea syndrome but normal-sized tonsils were satisfactorily corrected by tonsillectomy and uvulopalatopharyngoplasty. One of the boys had muscular hypotony as contributing cause of the condition. The other had mandibular hypoplasia in combination with a long soft palate. Thorough preoperative anamnesis and examination in a sleep laboratory are necessary to determine which cases will benefit from surgery in spite of normal-sized tonsils and which surgical procedure will be most helpful.

Peripheral Sleep Apnea syndrome has been associated with enlarged tonsils and adenoids as well as other abnormalities which may cause upper airway obstruction in children.
