Subglottic Stenosis.

Subglottic stenosis refers to narrowing of the airway diameter below the vocal folds and may be congenital or acquired. Typical signs and symptoms range from recurrent croup and exertional stridor to complete airflow obstruction requiring tracheotomy. Management of moderate and severe subglottic stenosis often requires intricate surgical techniques. To optimize the success of these surgeries, a thorough assessment of the child's airway, lungs, reflux, and swallow needs to be evaluated. In order to provide concerted and coordinated care between typically otolaryngology (ENT), pulmonary, gastroe…

Pediatric tracheostomy.

Tracheotomy refers to a surgical incision made into a trachea. Tracheostomy, on the other hand, refers to a surgical procedure whereby the tracheal lumen is positioned in close proximity to the skin surface. Tracheostomy is an uncommon procedure in the pediatric population. When required tracheostomy is typically performed as an open surgical procedure under general anesthesia with the patient intubated. However, it may need to be performed under local anesthesia or over a rigid bronchoscope in the patient with a precarious airway. Over the past half century, the primary indication for pediatr…

[Laryngeal tube suction].

BACKGROUND: The difficult airway remains a challenge especially in emergencies. MATERIALS AND METHODS: The use of laryngeal tube suction in 8 cases involving difficult airways in emergencies was reviewed. RESULTS: Use of the laryngeal tube was successful in all cases to bridge patients until a secure airway was established; insertions were successful at the first attempt and classified as easy. In 6 out of 8 patients a secure airway was established by cricothyreotomy or by surgical tracheotomy while using the laryngeal tube for oxygenation and ventilation of the patient. CONCLUSION: The lar…

Anesthetic management of a patient with progressive supranuclear palsy.

A 74-year-old woman with progressive supranuclear palsy (PSP) was scheduled for laryngotracheal separation surgery. Her neck showed severe backward tilt as a symptom of PSP. Magnetic resonance imaging (MRI) showed a significant airway stenosis due to the neck deformity. In the operating room, awake orotracheal intubation failed because of the neck deformity and airway stenosis. Therefore, tracheotomy was performed for airway management. General anesthesia was induced and maintained with sevoflurane (1.0%-2.5%) and fentanyl (total, 200 microg). Vecuronium (total, 5 mg) was used as a muscle rela…

Tophaceous gout presenting as a dorsal nasal lump.

A literature review reveals that gout has been described as affecting many sites in the head and neck region, both in the arthritic and tophaceous form. Gout can often mimic malignancy or infection, and has been described as causing acute airway problems requiring emergency tracheotomy. Here we describe the first published case of tophaceous gout affecting the soft tissues overlying the nasal bones. The patient presented with a bony, hard, dorsal hump and requested aesthetic rhinoplasty. We also describe an endoscopic technique for removal of tophi using a powered microdebrider system with a p…

A severe case of Moebius syndrome with calcification on the fourth ventricular floor.

We report the case of a Japanese girl with a severe type of Moebius syndrome. Her morphological features were a mask-like face, limitation of horizontal eye movements, severe bulbar palsy, multiple and bilateral arthrogryposis including the elbow, knee, and ankle joints, and clubfeet. After birth, her general condition became worse because of repeated apneic spells and aspiration pneumonias due to dysphagia. She finally required tracheotomy. Computed tomography (CT) of the brain revealed minute calcifications on the fourth ventricle floor; this may have been due to severe damage to the brain s…

[The laryngologic achievement at the Blegdam Hospital in Copenhagen during the serious epidemic of poliomyelitis 1952/53].

During the serious epidemic of poliomyelitis about 3000 cases were admitted to the Blegdam Hospital in Copenhagen. In 2.341 cases the diagnosis was verified. 1.250 cases were paralysed and 345 had more or less insufficiency of respiration. At Blegdam Hospital only one Draeger tank respirator and six cuirass respirators were available. It was necessary to try other methods of treatment. Inspired by Dr. Mogens Bjørneboe Professor, H.C.A. Lassen called for assistance from the anaesthetist Dr. Bjørn Ibsen, who proposed to make tracheotomy followed by introduction of a cuffed tracheal rubber tube a…

[Long-term manifestations of laryngeal sarcoidosis].

We report the medical history of a 19 year old female patient with isolated laryngeal sarcoidosis, who was followed for 12 years. It illustrates the typical course of the disease with remissions and relapses until the inflammation is "burnt out." Although the laryngoscopic findings of pale-red swollen supraglottic structures are pathognomonic, diagnosis is rarely assumed at first sight. Long-term administration of cortisone in low dosage is the treatment of choice. Acute exacerbations with increasing airway obstruction may occur at any time, although in general a tracheotomy can be avoided bec…

[Treatment of obstructive sleep apnea with methods of orthognathic surgery].

The only surgical means available up to now for treating obstructive sleep apnea syndrome (OSAS) consisted in uvulopalatopharyngoplasty (UPPP), which failed to produce satisfactory results. The ultima ratio frequently resorted to was tracheotomy with all its medical and social drawbacks. Complete advancement of the maxillomandibular complex opens up new avenues for the treatment of upper airways obstruction. The present report is on 13 patients operated for polysomnographically established OSA. The maxillomandibular complex was brought as far forward as possible and esthetically justifiable. T…

Corticosteroid management of airway hemangiomas: long-term follow-up.

Nine children with airway obstructing hemangiomas were managed with corticosteroid therapy for durations of 6 weeks to 17 months. One patient required a tracheotomy. Steroid therapy improved the airways of the other 8 patients. Therapy was continued for 8 months or longer in 6 patients, none of whom developed life-threatening infections. Examinations 1 1/2 to 7 1/2 years after therapy revealed all of the patients to be within normal range for height and weight, but 3 of the 6 patients treated for 8 months or longer were in the lower range of normal. Steroid therapy is an effective method of pr…

Acute epiglottitis treated by intubation.

Acute epiglottitis is a severe disease which is especially common in children and may be fatal if not treated. The main problem in these patients is to secure a free airway. During the last decade there has been much debate as to whether one should perform a tracheotomy or secure the airway by means of long-term intubation. During the last 12 years we have treated 86 patients with acute epiglottitis by long-term intubation to secure the airway. Three of these patients have been tracheotomized. All patients have been investigated by direct laryngotracheoscopy about 3 months after recovery. Duri…

Acute cricoarytenoid arthritis: local periarticular steroid injection.

Acute cricoarytenoid arthritis is a frequent complication of rheumatoid arthritis and the most frequent otolaryngologic manifestation of the disease. Over 25% of rheumatoid arthritis cases have discomfort from this problem. Other etiologies can produce cricoarytenoid arthritis. Symptoms range from mild discomfort through hoarseness to complete airway obstruction requiring emergency tracheotomy. Chronic cricoarytenoid arthritis may result in joint ankylosis and vocal fold fixation. Single periarticular triamcinolone injections may bring rapid and dramatic relief of the symptomatology of nonanky…
