Incidence, risk factors, and outcomes of anti-tuberculosis drug-induced hepatotoxicity in people with and without HIV in Thailand and the Philippines.

BACKGROUND: Anti-tuberculosis drug-induced hepatotoxicity (DIH) is a common complication among people living with HIV (PLWH), potentially leading to treatment interruptions. OBJECTIVE: To determine the incidence of DIH and identify risk factors in Thai and Filipino PLWH on tuberculosis (TB) treatment, compared with those with TB mono-infection. METHODS: We conducted a cohort study of Thai and Filipino adults with active TB using retrospective data from Thailand (2000-2024) and prospective data from the Philippines (2024). DIH was defined as AST or ALT ≥3 times the upper limit of normal. Logi

Epidemiology of communicable diseases in Jazan region: Situational assessment, risk characterization, and evaluation of prevention and control programs outcomes.

The incidence of communicable diseases has witnessed a reduction in Saudi Arabia during the last 4 decades. Nonetheless, the Jazan region has been indicated as one of the main regions affected by various communicable conditions. The geographical, socioeconomic, and climate characteristics of Jazan have made it vulnerable to communicable and vector-borne diseases. This review aims to provide a comprehensive overview of the epidemiology of communicable diseases in Jazan, Saudi Arabia. This review also gives a historical description of infectious diseases in the region and the relevant prevention

Analysis of research intensity on infectious disease by disease burden reveals which infectious diseases are neglected by researchers.

Infectious diseases are associated with considerable morbidity and mortality worldwide. Although human, financial, substantial, and time resources are limited, it is unknown whether such resources are used effectively in research to manage diseases. The correlation between the disability-adjusted life years to represent disease burden and number of publications as a surrogate for research activity was investigated to measure burden-adjusted research intensity for 52 infectious diseases at global and country levels. There was significantly low research intensity for paratyphoid fever and high i

Tuberculosis diagnosed after major hepatectomy for suspected malignancy.

A patient with a history of surgery and adjuvant chemotherapy 2 1/2 years previously for Dukes C colonic adenocarcinoma was diagnosed with a focal liver lesion on follow-up examinations. Ultrasound and computed tomography scan revealed a 3.8 cm soft tissue mass. Positron emission tomography scan showed intense uptake, corroborating the diagnosis of a colonic liver metastasis. Major hepatectomy was performed but pathology revealed that the lesion was in fact a benign tuberculosis pseudo-tumour. In developed countries liver tuberculosis remains extremely rare, particularly the macronodular form.

Utility of Point-of-care Ultrasound in Children With Pulmonary Tuberculosis.

BACKGROUND: Point-of-care ultrasound (POCUS) detects extrapulmonary tuberculosis (EPTB) in HIV infected adults but has not been evaluated in children despite their higher risk of EPTB. This study's aims were to investigate feasibility of POCUS for EPTB in children, frequency of POCUS findings suggestive of EPTB and time to sonographic resolution of findings with treatment. METHODS: This prospective South African cohort study enrolled children with suspected pulmonary tuberculosis (PTB). POCUS for pleural, pericardial or ascitic effusion, abdominal lymphadenopathy or splenic or hepatic microab

Factors that complicate the treatment of tuberculosis in HIV-infected patients.

Treatment of tuberculosis (TB) in persons coinfected with HIV has become increasingly complex during the past decade. We describe the factors that complicate anti-TB therapy in a large observational cohort of HIV-infected persons in the United States. Among 367 HIV-infected patients with 372 episodes of culture-confirmed TB, 44.1% had injection drug use as a mode of HIV transmission. Hepatic disease was present at the time of TB diagnosis or during anti-TB therapy for 91 episodes (24.5%). Elevation at least twice the upper limits of normal of aminotransaminases was observed during the first mo

Dengue-like presentation of acute viral hepatitis in an adolescent receiving antitubercular therapy: a case report.

INTRODUCTION AND IMPORTANCE: Severe drug-induced liver injury, dengue, and viral hepatitis are important causes of acute liver dysfunction, but their co-occurrence in young patients is extremely uncommon and can create diagnostic challenges. CASE PRESENTATION: We report a 15-year-old boy who developed severe acute hepatitis while receiving antitubercular therapy (ATT). He presented with fever, vomiting, and myalgias 4 weeks after starting ATT and was initially diagnosed clinically as dengue. Laboratory tests showed markedly elevated transaminases [aspartate aminotransferase (AST) 4172 U/l and

Might infection explain the higher risk of coronary heart disease in South Asians? Systematic review comparing prevalence rates with white populations in developed countries.

OBJECTIVES: South Asians in developed countries such as the UK are at comparatively high risk of coronary heart disease for reasons which are not fully understood. One unexplored hypothesis is more infections in this ethnic group. This study assessed whether the prevalence of infections among South Asians differs from that among White populations of European origin in developed countries. STUDY DESIGN: Systematic review. METHODS: Medline, Web of Science and Google Scholar databases were searched. In addition, reference lists and citations were reviewed. RESULTS: Twenty-one studies reported

Ischemic colitis during interferon-ribavirin therapy for chronic hepatitis C: a case report.

Ischemic colitis is a rare complication of interferon administration. Only 9 cases in 6 reports have been described to-date. This report describes a case of ischemic colitis during pegylated interferon and ribavirin treatment for chronic hepatitis C, and includes a review of the relevant literature. A 48-year-old woman was treated with pegylated interferon α-2a and ribavirin for chronic hepatitis C, genotype Ib. After 19 wk of treatment, the patient complained of severe afebrile abdominal pain with hematochezia. Vital signs were stable and serum white blood cell count was within the normal ran

Short term evaluation of a rural immunization program in Nigeria.

BACKGROUND: Immunization remains the primary strategy in both the control and prevention of common childhood diseases, particularly in the developing world. Immunization and preprimary health care services were commenced in a rural community in Nigeria in 1998, when vaccine coverage for all Expanded Program on Immunization (EPI) diseases (tuberculosis, polio, diphtheria, pertussis, tetanus, measles, and hepatitis B) was considerably low with only 43% of children fully immunized. METHODS: Children aged 0-2 years and living in a rural community were recruited into the study. Data on vaccination
