Hypoadrenalism in patients with pulmonary tuberculosis in Tanzania: an undiagnosed complication?

Addison's disease is rarely diagnosed in most African countries although tuberculosis, one of its major causes, is a widespread problem. In this study adrenal function was assessed using the Synacthen test in 50 patients with chronic pulmonary tuberculosis admitted to hospital in Dar es Salaam. Sixteen patients (32%) had an impaired response. Two had subnormal basal cortisol levels, one of whom had a normal response to Synacthen. There was no significant difference between the patients with an impaired cortisol response and those with a normal response with respect to frequency of non-specific

[The elimination of chemotherapy side effects in pulmonary tuberculosis patients].

Neurotoxic side-effects of tuberculosis chemotherapy occurred in 14.9% of patients with tuberculosis treated prophylactically with intramuscular pyridoxine infusions. Use of small doses of nootropil (piracetam) allowed to control the side-effects (headache and vertigo, sleep disorders, irritation, memory disorders) and to continue treatment with isoniazide, one of the most potent tuberculostatic agents.

[Toxic-allergic and viral hepatitis in patients with pulmonary tuberculosis].

Forty four patients with various forms of pulmonary tuberculosis in association with virus hepatitis B (21) and toxic-allergic hepatitis (23) were followed up in the time course of the diseases. There was a marked clinical similarity between toxic-allergic hepatitis and virus hepatitis B which differed in their qualitative indices and duration. Differential diagnosis of virus hepatitis B and toxic-allergic hepatitis was possible on the basis of the clinical signs and routine tests: determination of bilirubin and its fractions in blood, estimation of activity of alanine and aspartate aminotrans

[Antiproteolytic activity of the blood serum in adolescents with pulmonary tuberculosis].

The antiproteolytic activity of the blood serum was examined in the course of hospital treatment of 128 adolescents with prevailingly torpid forms of pulmonary tuberculosis and in 19 adolescents with tubercular infection (control). A direct correlation has been found between the degree of clinico-roentgenological manifestations of the disease and level of serum alpha 1-inhibitor of proteinases as well as between the efficacy of treatment and normalization of the antiproteolytic activity of the blood of the patients. It proposed to use the determination of alpha 1-inhibitor of proteinases as an

[The optimization of the corticosteroid therapy of patients with destructive pulmonary tuberculosis].

Corticosteroids were administered in 120 patients with active pulmonary tuberculosis during the morning hours every other day in association with tuberculostatic agents. It was found that corticosteroids in a dose similar to 15 mg of prednisolone did not influence the efficacy of antibacterial agents while an increase to 20 mg used for not less than 2 months increases the treatment efficacy to 83.3% and over 3 months--to 89.5%.

Ten-year experience with mycetomas in patients with pulmonary tuberculosis.

We studied 33 consecutive patients with tuberculous pulmonary cavities complicated by fungus balls to evaluate their treatment. Nineteen had surgical resection for massive or recurrent bleeding or possibility of tumor. One patient died of postpneumonectomy empyema (30-day surgical mortality, 5 percent). Fourteen had no surgery. No patient died of hemoptysis. Respiratory failure contributed most often to death. Hepatic complications and other problems of alcoholism were also prominent. Good results can be obtained by resection in these severely ill patients if care is taken to preserve function

The role of aspiration biopsy cytology in the diagnosis of pulmonary tuberculosis.

The experience at one institution concerning the diagnosis of pulmonary tuberculosis by aspiration biopsy is reviewed. Twelve cases in which acid-fast bacilli were identified in pulmonary aspiration material are reported. The cytologic findings were confirmed by microbiologic culture in nine cases. The advantages of using aspiration biopsy cytology in the diagnosis of pulmonary tuberculosis are discussed. In our experience we conclude that the use of aspiration biopsy cytology in the diagnosis of pulmonary tuberculosis is not only accurate, but also offers several advantages that are important

Cause of death in patients admitted to hospital for pulmonary tuberculosis.

Study of hospital case-notes and necropsy reports of 60 patients certified as dying from tuberculosis over 7 years identified a group of 16 patients (27%) who died suddenly and unexpectedly and in whom the cause of death was obscure. In 14 of the 16 death followed initiation of antituberculous chemotherapy.

Nursery exposure of 528 newborns to a nurse with pulmonary tuberculosis.

Newborn infants are thought to be particularly susceptible to infection with Mycobacterium tuberculosis, and the possibility of exposure to unrecognized clinical tuberculosis in nursery and hospital personnel is ever present. To reduce this possibility, periodic skin testing of hospital personnel is often carried out, and tuberculin-positive personnel are given preventive treatment with isoniazid (INH). Also, environmental controls, such as ultraviolet light and periodic air exchange, are used. The efficacy of these measures has not been fully established. A nursery supervisor with smear- and
