A histopathological study on pyloric ulcer.

After the pyloric ulcer was defined as a peptic ulcer occurring at the pylorus or having its centre in the prepyloric region within 2 cm from the pylorus, its histopathological features were studied on 94 pyloric ulcers in 88 surgically resected stomachs. 72.3 per cent of these ulcers were less than 1.0 cm in diameter. The incidences of UI-II, III and IV ulcers were almost equal, and the majority of UI-II and UI-III ulcers were found in scarring stage in the specimens. These pyloric ulcers were classified into three types according to whether the centre of ulcer is situated above (Type-1), on …

Chronic gastric ulcer and life events.

The purpose of this study was to define the frequency of stress preceding or during the course of chronic gastric ulcer (CGU). Stress was measured by the occurrence of 62 selected life events over 2 yr in two groups of CGU patients. One group (A) was studied for events during the 2 yr before their diagnosis (147 patients) and the second group (B) was studied for events occurring after diagnosis (73 patients). The ulcer groups were compared with community controls matched for age, sex, and social grade. Groups A and B did not differ in any significant way in their reported life events, indicati…

The surgical treatment of duodenal ulcer.

Based on experience of a pilot series of 100 patients treated by parietal cell vagotomy (PCV) and a prospective clinical comparison between … rative gastric acid secretion had no predictive value in these studies. In non-obstructing duodenal ulcer the surgical choice appears to be between PCV (no sequelae) and SGV + A (no recurrence). The optimal operation for prepyloric ulcer cannot as yet be determined.

Prognosis of completely healed gastric ulcer.

Eighty patients with chronic gastric ulcer, shown to have complete healing by gastric photography, were followed for 18 months to nine years. Ulceration recurred in 24, always at the site of the original ulcer, the majority (14) of relapses occurring within 12 months. Those with recurrent ulcers were more likely to show delayed healing of their initial ulcer, than the ulcers not shown to recur. At two months, this was statistically significant (P = 0.004) and even more so at three months (P = 0.004). Recurrences were more frequent in males (P = 0.09). There were no differences in ulcer site or…

Antacids and peptic ulcer--a reappraisal.

Antacids can reduce gastroduodenal acidity for long periods if taken in substantial quantities after food. Their healing effect on gastric ulcer is minimal, if present at all, and easily overwhelmed by the benefit obtained from admission to hospital. Intensive antacid therapy appears effective in healing duodenal ulcer and preventing haemorrhage from stress ulcer, and is comparable in these respects with cimetidine but with a higher incidence of side-effects. Clinical impression strongly suggests that antacids relieve pain in peptic ulcer but objective confirmation is lacking.

[Epidemiology of duodenal ulcer (author's transl)].

The epidemiology of peptic ulcer was investigated in 1105 endoscopy patients in a Zurich city hospital and in a random nongastroenterological hospital population. A raised susceptibility for duodenal ulcer was found in young, mainly unskilled, foreign labourers. There was no raised susceptibility in female foreign workers who also suffered less often from gastric ulcer than Swiss women. A tendency to ulceration in certain other diseases previously incriminated and in alcohol and nicotine consumers was not observed. A connection between susceptibility to ulceration and migration is assumed.

[Gastroduodenal ulcer: resection procedures (author's transl)].

In recent years vagotomy has become more and more prefered in ulcer surgery. Based on the results of prospective trials, gastric resection should be abandoned as treatment of uncomplicated duodenal ulcer. Resections are still indicated for most ulcer complications, such as bleeding, penetration of chronic ulcer, and in recurrent ulcers after vagotomy. The same is true for gastric ulcer type I. Questionnaires indicate, however, that German surgeons still favor the two-thirds resection.
