Chronic nonhealing ulcer in a finger of a toddler--a rare presentation of the hair thread tourniquet syndrome.

BACKGROUND: Hair-thread tourniquet syndrome is caused by a strand of hair or thread tightly wrapped around an appendage. Affected parts include toes, phallus, fingers, clitoris, labia and even uvula. This disorder can be quite serious if not promptly recognized and treated; permanent tissue damage or even loss of the appendage can occur. MATERIAL: A rare and delayed presentation of hair-thread tourniquet syndrome as a non-healing, discharging ulcer in a two-year-old child is presented. RESULTS: Identification of the fiber with loupes, complete removal of the fiber, and a dorsal longitudinal …

Rapp-Hodgkin syndrome and the tail of p63.

We report the clinical and molecular abnormalities in a 19-year-old woman with Rapp-Hodgkin ectodermal dysplasia syndrome. The physical features include mid-facial hypoplasia, uncombable hair, cleft palate and bifid uvula, lacrimal duct obstruction and dry skin. Sequencing of the p63 gene reveals a new heterozygous frameshift mutation, 1787delG, in exon 14. The frameshift results in changes to the tail of p63 with the addition of 68 missense amino acids downstream and a delayed termination codon that extends the protein length by 21 amino acids. These changes are predicted to disrupt the norma…

Activation patterns of the tongue muscles with selective stimulation of the hypoglossal nerve.

The upper airways were imaged in anesthetized beagles to investigate the effect of selective stimulation of the hypoglossal nerve (HG) with a multi-contact peripheral nerve electrode. A fiberscope lead (diam. 5 mm) was inserted through the proximal end of the tracheostomy and fixed at 1 cm behind the uvula to image the oro- and nasopharynx while the head was fixed at either 30 degrees or 60 degrees from the horizontal with the mouth open or closed. Images of the tongue were recorded with a camera in open mouth positions. These images show that stimulations delivered through the electrode conta…

Dominant inheritance of cleft palate, microstomia and micrognathia--possible linkage to the fragile site at 16q22 (FRA16B).

We report a family in which a father and his three children are affected with microstomia, micrognathia and partial or complete cleft of the hard and soft palate. The probands were non-identical twins, a boy and a girl, both noted to have the above features soon after birth. Their father was diagnosed with a submucous cleft of the palate at the age of 4 years and their older brother has milder facial features and a bifid uvula. All affected family members were demonstrated to have a fragile site on chromosome 16q22 but otherwise normal karyotypes. Of interest is a previously described family w…

[Surgical options for treatment of snoring and sleep apnea].

Simple snoring and obstructive sleep apnea are part of a complex disease process with a high prevalence. Surgical treatment options have been developed by otolaryngologists over the last decades. Knowledge of different surgical procedures is required to address the various findings during preoperative assessment. Patients with nasal airway blockage due to septal deviation, large turbinates or polyps may benefit from endonasal surgery. Obstructive tonsils, palatal webbing, and an enlarged uvula require surgical approaches including the soft palate and tonsils. The controversy on indications and…

A review of uvulopalatopharyngoplasty for pediatric obstructive sleep apnea.

OBJECTIVES: To evaluate existing literature to understand the utility and safety of uvulopalatopharyngoplasty (UPPP) for treatment of pediatric obstructive sleep apnea (OSA). METHODS: A literature review was conducted by two authors to search for studies from the inception of two databases until March 1, 2023. Studies in which participants were under 18 years of age and underwent UPPP for OSA or upper airway obstruction were selected. Data on variables such as pre- and postoperative severity, efficacy, complications, and follow-up were collected from all studies. RESULTS: After applying incl

Functional Outcomes of Swallowing Following Surgery for Obstructive Sleep Apnea.

OBJECTIVE: Identification and evaluation of swallowing dysfunction in patients undergoing upper airway surgery for obstructive sleep apnea (OSA) is limited. This study evaluated subjective swallowing function pre and postoperatively in patients undergoing multi-level reconstructive pharyngeal sleep surgery. METHODS: A retrospective analysis of prospectively-administered Eating Assessment Tool (EAT-10) scores was conducted among adult patients undergoing surgery for OSA at a tertiary sleep surgery center. Preoperative and 1, 3, and 6-month postoperative time points were assessed. Patients were

The potential effects of obesity on predicting outcomes of velopharyngeal surgery for obstructive sleep apnea.

PURPOSE: To explore the outcome associated factors of velopharyngeal surgery for treating obstructive sleep apnea (OSA) and the effects of obesity. METHODS: A total of 175 adult OSA patients who underwent velopharyngeal surgery, including the revised uvulopalatopharyngoplasty with uvula preservation (H-UPPP) alone or the combination of H-UPPP and transpalatal advancement pharyngoplasty, were retrospectively studied. The pre-operative information of these patients, including physical examination, polysomnography (PSG), and upper airway CT, were collected for analysis. Post-operative PSG used f…

Comparison of radiofrequency and transoral robotic surgery in obstructive sleep apnea syndrome treatment.

INTRODUCTION: Radiofrequency tissue ablation (RFTA) and transoral robotic surgery (TORS) are the methods used in OSAS surgery. We also aimed to compare the advantages and disadvantages of RF and TORS as treatment methods applied in OSAS patients in terms of many parameters, especially apnea hypopnea index (AHI). MATERIALS AND METHODS: Patients were classified by performing a detailed examination and evaluation before surgery. 20 patients treated with anterior palatoplasty and uvulectomy -/+ tonsillectomy + RFTA (17 males, 3 females) and 20 patients treated with anterior palatoplasty and uvule

Hair tourniquet syndrome in the dental patient.

Hair tourniquet syndrome is a condition where a hair becomes entangled around an appendage. In some cases a knot will form and the resulting tightened noose will slowly strangulate the appendage. Rarely, this condition will affect the oral cavity, but even more rarely, this condition will affect a dental structure.

Upper respiratory tract infections in adolescents.

While upper respiratory tract infections (URTIs) cause much infectious morbidity in infants and young children, adolescents are not immune to infections. Adolescents experience two to four episodes of viral nasopharyngitis annually. In addition to group A streptococcus (GAS), pharyngitis may occur with other streptococci, Arcanobacterium haemolyticum, Epstein-Barr virus, Neisseria gonorrhoeae, and other pathogens. Uvulits, typically in association with GAS, occurs occasionally. Peritonsillar abscess is the most common deep neck space infection seen in adolescents, but retropharyngeal and parap
