In severe mitral valvular disease with a giant left atrium, there occur such abnormal findings as the compression and bending of the basal portion of the posterior wall of the left ventricle accompanied by its paradoxical movement. Following mitral valve replacement in this group of patients, it was found that the strut of the artificial valve is apt to lean against the ventricular septum and the inflow stream of blood was directed toward outflow tract of the left ventricle, appearing to be obstructed by the ventricular septum. Here, we devised a new surgical method, para-annular plication, fo

Complete circumferential aortic dissection with bidirectional intimo-intimal intussusception is a rare occurrence in Stanford type A dissections. The antegrade dissection flap can obstruct the left ventricular outflow tract and coronary sinuses, whereas the retrograde flap can obstruct the aortic arch and branch vessels. Sequelae include aortic regurgitation, myocardial ischemia, and neurologic complications. This case series highlights the need for prompt diagnosis with electrocardiography-gated computed tomography and surgical intervention for this unique pathologic process.

Supravalvar aortic stenosis is a rare congenital heart anomaly, producing left ventricular outflow tract obstruction. Of the two anatomic variants that have been described, diffuse type is the rarest. We report five such cases in children between two months and nine years of age. None had features of Williams syndrome. The entire aorta was involved in three cases, with abdominal aortic coarctation in two cases. Stenosis was mainly due to involvement of the media, which showed smooth muscle hypertrophy, abnormal elastic fibers, and mild collagenization. Predominant intimal change was seen in on

With advances in pediatric cardiology and cardiac surgery, the population of adults with congenital heart disease (CHD) has increased. In the current era, there are more adults with CHD than children. This population has many unique issues and needs. Since the 2001 Canadian Cardiovascular Society Consensus Conference report on the management of adults with CHD, there have been significant advances in the field of adult CHD. Therefore, new clinical guidelines have been written by Canadian adult CHD physicians in collaboration with an international panel of experts in the field. Part II of the g

BACKGROUND: Although thyroid disorders are present in approximately 3% of pregnant women, little is known about the association between maternal thyroid disease and birth defects.
METHODS: We assessed the association between maternal thyroid disease, thyroid medication use, and 38 types of birth defects among 14,067 cases and 5875 controls in the National Birth Defects Prevention Study, a multisite, population-based, case-control study. Infants in this study were born between October 1997 and December 2004. Information on exposures including maternal diseases and use of medications was collec

Intracardiac obstruction to the systemic circulation can develop in patients with many forms of congenital heart disease. When transposition of the great arteries accompanies tricuspid atresia, narrowing of the ventricular septal defect (VSD) leads to subaortic stenosis. In a similar fashion, a restrictive bulboventricular foramen compromises systemic arterial outflow in patients who have single left ventricle with subaortic outflow chamber. The same effect can be seen in VSD closure in mitral atresia with normally related great vessels. Although some forms of single ventricle can be treated b

This report describes a subgroup of 52 elderly patients with obstructive hypertrophic cardiomyopathy in whom certain clinical and morphologic features differed importantly from those of many other patients with this disease. Ages ranged from 60 to 84 years (mean 69) and 45 [87%] were women. Echocardiographic examination showed a relatively small heart, having only modest ventricular septal hypertrophy associated with marked distortion of left ventricular outflow tract morphology. By virtue of selection, left ventricular outflow tract size at end-diastole was substantially reduced, and anterior

Hypertrophic cardiomyopathy can be accompanied by dynamic obstruction in the left ventricular outflow tract and acute apical ballooning, which are among the very rare causes of cardiogenic shock. This condition requires a specific treatment approach that in many ways differs from the treatment of other causes of cardiogenic shock. We present a case and our treatment strategy (including extracorporeal life support) for refractory cardiogenic shock in a patient with previously undiagnosed hypertrophic cardiomyopathy.

Sinus of Valsalva aneurysms are a rare entity. Rupture of such aneurysms is a major cause of aortocardiac fistulas usually occurring between the right sinus of Valsalva and right cardiac chambers. We report an exceptional case of a ruptured congenital sinus of Valsalva aneurysm with fistulas involving both the right- and left-ventricular outflow tracts and causing RVOT obstruction. We also demonstrate the utility of computed tomography angiography and transesophageal echocardiography in diagnosing these fistulas.

BACKGROUND: This study sought to assess risk factors for late mortality after the Rastelli operation for patients with transposition of the great arteries, ventricular septal defect, and left ventricular outflow tract obstruction.
METHODS: Records of 39 patients who underwent the Rastelli operation between 1977 and 2004 were reviewed. Median age at the time of operation was 5.1 years (2.2 years within the last 5 years).
RESULTS: There were no early deaths. During a median follow-up of 8.9 years (range, 0 to 25 years), 2 patients died of sudden death, 1 of pneumonia, 1 during reoperation, and

BACKGROUND: Double-chambered right ventricle is a form of right ventricular outflow tract obstruction that develops over time, often in patients with an abnormally short distance between the moderator band and pulmonary valve. This lesion typically presents in childhood or adolescence and is often accompanied by a ventricular septal defect. Only a handful of previous cases have been described in which double-chambered right ventricle occurred in adulthood.
METHODS: Since 1992, three patients more than 30 years old (38, 43, and 66 years of age) have presented at our institution with unusual sy

Hypertrophic cardiomyopathy (HCM) is a condition of unknown etiology with thickening of the interventricular septum. It is better defined as a genetic disorder of autosomal dominant inheritance with variable left ventricular hypertrophy and symptoms. HCM has been rarely reported in association with other causes of hypertrophy such as hypertension and aortic stenosis. The combination of aortic stenosis or other arterial stenosis such as coronary artery disease with HCM is unusual. We present a case of a middle aged male with an unusual combination of cardiac diseases. Mild coarctation of aorta,

During a 31/2 year period, 11 consecutive infants with total anomalous pulmonary venous connection younger than 6 months of age underwent repair at our institution. Early mortality was 19% (one of 11) and long-term mortality 18% (two of 11), yielding an overall survival of 73%. One late death was due to bradyarrhythmia and the other was due to recurrent pulmonary venous obstruction. Postoperative catheterization in seven of eight survivors and in three additional patients disclosed normal or nearly normal pulmonary artery pressure and pulmonary arteriolar resistance. Left ventricular diastolic

BACKGROUND: Fabry disease is caused by mutations in the α-galactosidase A (GLA) gene, which is located in X-chromosome coding for the lysosomal enzyme of GLA. Among many gene mutations, E66Q mutation is under discussion for its pathogenicity because there is no clinical report showing pathological evidence of Fabry disease with E66Q mutation.
CASE PRESENTATION: A 65-year-old Japanese female was referred to our hospital for chest discomfort on effort. Transthoracic echocardiography showed severe left ventricular (LV) hypertrophy with LV outflow obstruction. Maximum LV outflow pressure gradient

BACKGROUND: Whether to perform uni or biventricular repair in ducto dependent neonates with hypoplastic but morphologically normal left ventricle and multi level left ventricle obstructions (hypoplastic left heart syndrome class III) remains unanswered. Echocardiographic criteria have been proposed for surgical decision.
HYPOTHESIS: Increased afterload and multi level left ventricle obstruction is constant. We assumed that restoration of normal loading conditions by relief of left ventricle obstructions promotes its growth, provided that part of the cardiac output was pre operatively supporte

In the Taussig-Bing malformation, residual subaortic obstruction from a hypertrophied subaortic conus is often a problem at the time of intraventricular repair. To alleviate this problem, during a Rastelli-type repair involving an extracardiac valved conduit and an intraventricular connection from the ventricular septal defect to both aortic and pulmonary outflows, an additional side-to-side anastomosis between the ascending aorta and the proximal portion of the divided pulmonary artery can be performed, creating a supplementary outflow for the left ventricle. This method was used in 2 patient

Intraoperative hypotension in noncardiac surgery is common and often multifactorial. We report a case of refractory hypotension during orthopedic surgery initially suspected to represent massive pulmonary embolism based on tachycardia and reduced end tidal carbon dioxide. Rescue transesophageal echocardiography demonstrated hyperdynamic biventricular function without right ventricular strain and identified systolic anterior motion of the mitral valve with dynamic left ventricular outflow tract obstruction and posteriorly directed mitral regurgitation. Targeted management with volume expansion,

Congenitally corrected transposition of the great arteries (CCTGA) is a rare congenital heart disease characterized by atrioventricular (AV) and ventriculo-arterial discordance;(1) that is, the left ventricle supports the pulmonary circulation and the right ventricle supports the systemic circulation. The most common cardiac anomalies in CCTGA include ventricular septal defect, pulmonary outflow tract obstruction and abnormalities of the systemic AV valve. (1) The dysfunction of the systemic ventricle occurs with increasing frequency in older patients with CCTGA, independent of their commonly

Non-obstructive hypertrophic cardiomyopathy, or apical hypertrophic cardiomyopathy (ApHCM), also referred to as Yamaguchi syndrome, is a type of hypertrophic cardiomyopathy (HCM) characterized by significant thickening of the left ventricular apex without blockage in the left ventricular outflow tract. It is a very rare variant of HCM. Patients with non-obstructive HCM often experience symptoms such as chest pain, palpitations, shortness of breath, and syncope, which may resemble those seen in various cardiovascular and non-cardiac conditions. Yamaguchi syndrome presents as a challenging yet m

Between 1991 and 1996, we performed a new technique of non-conduit repair on five consecutive patients with complete transposition of the great arteries associated with ventricular septal defect and pulmonary outflow tract obstruction. There were no late deaths. This technique consisted of constructing an intraventricular tunnel with a patch that connected the left ventricle to the aorta, closing the pulmonary outflow tract, drawing the main pulmonary artery directly to the right ventriculotomy, and reconstructing the new right ventricular outflow tract with a monocusp patch. The main feature
