Over 500million people live in yellow fever (YF) endemic areas and more than 3million travel to endemic countries every year. Prevention against YF, caused by the YF virus, an arbovirus, can only be efficiently obtained by active immunization. The vaccine is well tolerated and severe adverse events are very rare. Exceptionally, YF vaccination may result in serious adverse events, such as viscerotropic and neurotropic diseases, sometimes with a fatal outcome. BACKGROUND: Guidelines issued by the WHO call for immunization against yellow fever (YF) at least 10 days before entering an endemic area. We assessed the extent at which travelers complied with these guidelines.
METHODS: A two-phase study at one travel center in Israel. This report describes a case of yellow fever vaccine-associated viscerotropic disease (YEL-AVD) that occurred after vaccination in a 22-year-old female. In 1793, during a yellow fever epidemic in Philadelphia, Benjamin Rush adopted a therapy that centered on rapid depletion through purgation and bleeding. His method, especially his reliance on copious bloodletting, was at first widely condemned, but many American practitioners eventually adopted it. Although the therapy struck many observers as being radical, in large part it grew from premises that had substantial support. Rush was convinced that it worked and that heroic methods were the key to conquering disease. In particular, massive bleeding became central to his therapeutics.

The first case of yellow fever in French Guiana since 1902 was reported in March 1998. The yellow fever virus genome was detected in postmortem liver biopsies by seminested polymerase chain reaction. We report the cloning and primary characterization of both cDNA and genomic fragments from the white gene of the yellow fever mosquito, Aedes aegypti.
