The effect of acoustic neuroma removal on hearing in the contralateral ear.

22 patients with unilateral acoustic neuromas were assessed audiometrically pre- and post-operatively. A high proportion (36%) were found to have a hearing loss of 10 dB or more (6 frequency average) in the contralateral ear in the post-operative period. The evidence points to a cochlear site of the loss, and serial audiograms suggest that the hearing deteriorates in the immediate post-operative period. Follow-up audiometry at three months or more showed that the hearing recovered to pre-operative levels. The possible mechanism of the hearing loss is discussed.

Preservation of hearing in acoustic tumor surgery: audiologic indicators.

Preservation of hearing in patients with acoustic nerve tumors can be a goal when tumor size is small and residual hearing is intact. Overall success rates for preservation have been reported to be 20% to 40%. The overall success rate in this series is 30.7%. However, indicators of intact auditory brain stem response (waves I-III-V), good speech discrimination score, and intact acoustic (stapedial) reflex were associated with a 68.2% rate of success. Thus, a comprehensive audiologic evaluation is a guideline for selecting and counselling patients with acoustic tumors before hearing preservatio

Airway area by acoustic response measurements and computerized tomography.

In order to determine more precisely the accuracy with which the acoustic reflection technique (ART) can infer airway area during spontaneous breathing, we compared acoustic measurements of airway area with equivalent areas measured from computerized tomographic (CT) scans of the neck and chest in 7 patients (mean age, 54 yr; range, 33 to 69 yr) with a history of upper airway abnormalities. At the time of the study, all patients were clinically stable and had no recurrent nerve palsy. Measurements of airway area by ART and CT were performed in the supine posture while patients breathed quietly

Influence of acoustic stress by noise on gastrointestinal motility in dogs.

The effects of acoustic stress (AS) on gastrointestinal motility and their prevention by previous treatment with naloxone, phentolamine, propranolol, muscimol, and diazepam were investigated in intact and vagotomized fasted dogs fitted with chronically implanted strain gauges on the antrum at 10 cm from pylorus and on the jejunum at 70 and 140 cm from the pylorus. These effects were compared to those produced by intracerebroventricular administration of ovine corticotropin releasing factor (oCRF). Beginning 40-50 min after the occurrence of a gastric migrating motor complex (MMC), a 1-hr heari

The acoustic reflex in diagnostic audiology: from Metz to present.

In 1946, Metz described what was probably the first practical impedance bridge for measurement of the acoustic reflex. Over the next four decades, diagnostic use of the acoustic reflex progressed in parallel with instrumentation refinements. Today, acoustic reflex threshold and decay testing are a routine component of the diagnostic audiology test battery. Recent advances in the measurement of suprathreshold latency and amplitude have further enhanced the diagnostic value of acoustic reflex measurement.

[Intracranial schwannomas of the cranial nerves. The acoustic nerve excluded].

The schwannomas of the cranial nerves (except the IId and VIIIth nerves) are known to be infrequent although progress in diagnosis of these tumors have been obtained by C.T. scan. We report twelve cases involving the facial nerve and five cases of the caudal nerves that were developed into the foramen jugulare, and two cases of trigeminal schwannomas. One may suspect these tumors in presence of any minimal, unilateral defect of otological, facial, and pharyngo-laryngeal functions. The surgical approach to the tumors depends on both its location and symptomatology.

The acoustic middle ear muscle reflex in albino rats.

The acoustic middle ear muscle reflex was studied in albino rats anesthetized with chloralose. The best frequency of the reflex and the threshold at this frequency were on average about 3 kHz and 57 dB SPL, respectively. The threshold increased as frequency increased above, and decreased below, the best frequency at a rate of about 20 dB/octave. Above about 12 kHz, the muscular response showed instability and habituation. Thresholds were similar between stapedius and tensor tympani reflexes and between ipsilateral and contralateral reflexes. The middle ear transmission loss due to the reflex w

Characteristics of the acoustic reflex in response to narrow bands of noise.

Normal-hearing young adults (5 M, 6 F) were exposed monaurally for 3 min to narrow-band noises centered at .5, 1, and 4 kc/s, at 20 db above respective acoustic reflex (AR) thresholds. Strip-chart recordings (1 mm/sec) of middle-ear muscle contractions were graphed and evaluated for peak amplitude, latency, adaptation, and recovery from contraction. The relative AR amplitude was greatest to the .5-kc/s band and decreased with increasing stimulus frequency. Women exhibited greater AR than men in response to the lowest frequency of stimulation. The AR latency was significantly faster for the .5-

The neuro-otologist's view of the surgical management of acoustic neuromas.

The experience of our group in the management of 1667 patients with the diagnosis of an acoustic neuroma has been reviewed. Certain features of the operation have been discussed. It is our feeling at this time that with the techniques available to the neuro-otologist for the management of patients harboring acoustic neuromas, the goals of surgery should be total removal of the tumor with preservation of the facial nerve and avoidance of injury to the adjacent important brain structures. Our patients, at this time, should reasonably expect this type of result from their surgery. Hearing preserv

[Radiologic diagnosis of acoustic neurinoma. Apropos of 130 patients].

Diagnostic value of radiologic examinations was assessed in a homogeneous series of 130 cases of neurinoma of acoustic nerve seen over the last 10 years. Radiologic exploration is conceived and used at the present time for two purposes. Firstly, it enables selection of patients with suspected acoustic nerve neurinoma. Radiotomography appears to be currently an accessory examination because of the discriminative performance of early auditory evoked potentials tests on the brain stem. Secondly, radiology is necessary for confirmation of diagnosis. Visualization of the tumoral syndrome can be obt

The human acoustic tensor tympani reflex. A case report.

In a patient with unilateral lower motoneuron facial paralysis secondary to Bell's palsy, we evaluated the characteristics of what we believe to be an acoustically-evoked reflex contraction of the tensor tympani muscle. Ipsilateral stimulation of the involved ear yielded a pattern of impedance change different from the pattern characterizing normal stapedius muscle contraction. Threshold of the reflex was elevated, amplitude was large and variable, onset latency was greater, onset rise was more gradual, and offset decay was more rapid. Contralateral stimulation, with probe in the involved ear,

Otosclerosis in a black child: diagnostic acoustic impedance studies.

Otosclerosis classically describes an osteodystrophic change in the bony labyrinth and stapes footplate, of autosomal dominant inheritance, reported rare under the age of 5, extremely "rare" in the Oriental and Black race, "non-existent" in the American Indian, and with a clinical incidence of 5 per 1000 Caucasians. The differential diagnosis of a non-effusion conductive hearing loss in a child should include otosclerosis, congenital malleus or footplate fixation, tympanosclerotic fixation, congenital cholesteatoma, lysis of the incus long process, Paget's disease, osteogenesis imperfecta, and

Auditory brain-stem response and acoustic reflex test.

Auditory brain-stem response (ABR) and acoustic reflex test (ART) results were reviewed for 30 patients with cerebellopontile angle tumors and for 30 patients without tumor. Patients included in the nontumor group were selected to match the audiometric configurations for individual patients in the tumor group. The ABR waveforms were abnormal for all but one of the patients with tumor and for seven of the patients without tumor. The ART results were abnormal for 25 of the patients with tumor and two of the patients without tumor. Patients without tumor yielding abnormal ABR results usually had

Acoustic analysis of voice for computerized laryngeal pathology assessment.

The purpose of this paper is to review the techniques of computerized acoustic analysis of voice signals used for laryngeal pathology assessment. Results of using a methodology described by Davis for distinguishing between T1 glottic cancer, noncancerous pathology, and normal speakers are presented. These results indicate that this technique is not sufficiently sensitive for general clinical usefulness. There is an intentional lack of mathematical detail in this paper, as its prime intent is to conceptually outline issues and approaches.

Stereotactic radiosurgery in cases of acoustic neurinoma: further experiences.

Fourteen patients with acoustic neurinomas varying in size from 7 to 30 mm were treated by stereotactic radiosurgery and then were followed for 4 years. The tumors of eight patients decreased 1 to 10 mm in diameter, no change was found in two, and the tumor increased slightly in three patients. Loss of the ability to enhance with contrast administration on computed tomographic scan was a common effect of the treatment. One patient in poor general condition died from intercurrent disease 6 months after radiosurgery. At postmortem examination, a large central necrosis was found. Among five patie

[Acoustic variation after surgery for severe Bell's palsy].

Auditory thresholds were studied before and after surgical decompression of the facial nerve in 35 patients following either transmastoidal (20 cases) or transmastoidal and suprapetrosal (15 cases) operations. Seventeen of the patients were under 40 years of age at the time of surgery, which did not affect hearing in the opposite ear. However, auditory threshold levels fell in the operated ear. These were of the bone conduction type, and possessed characteristics of an acute hearing lesion (maximal in the 4000 frequency range and improving progressively) of 0 to 35 dB (mean 5 dB). Bilateral acoustic neuromas: clinical aspects, pathogenesis, and treatment.

Fifteen patients with bilateral acoustic neuromas are presented. Ten patients became symptomatic before age 21, and 9 of these patients developed additional central nervous system tumors. Cutaneous manifestations of neurofibromatosis and a positive family history of this disorder were absent in many of the patients. Most patients required multiple surgical procedures, and the timing, techniques, and results of these operations are reported. The pathogenesis of these tumors, the genetic aspects of this disorder, and potential treatment alternatives are discussed.
