Analysis of work system components in interprofessional communication to determine shock etiology.

In critical care settings, lack of agreement among clinicians on shock etiology is associated with higher mortality. Effective interprofessional communication may facilitate agreement on etiology and reduce mortality. This study sought to understand the interaction of work system components (people, environments, tools, tasks) in interprofessional communication to determine shock etiology. We interviewed 28 physicians, advanced practice providers, nurses, and pharmacists at three United States hospitals using a Critical Incident Technique. Analysis was guided by the Systems Engineering Initiat

Optimizing Organ Donation Outcomes in the ICU: An Evidence-Based Practice Quality Improvement Initiative.

Despite scientific advancements, demand for life-saving organs continues to outpace supply. Together, hospitals and organ procurement organizations facilitate deceased organ donations; however, a lack of structured inpatient processes and quality improvement mechanisms for potential donor management can lead to delayed referrals by organ procurement organizations, physiological instability, and suboptimal donation outcomes. Goal-directed strategies applied during the inpatient and preprocurement phases of care can significantly increase the number and quality of organs recovered. The aim of th

Harnessing the Power of Health Information Exchange.

In the complex landscape of health care, the journey of a patient with end stage kidney disease (ESKD) is representative of the challenges common to those with chronic health conditions. This article delves into the fragmented nature of health care data and the pivotal role of health information exchange (HIE) in enhancing care coordination. While HIE has the potential to improve patient outcomes by facilitating the seamless transfer of information among providers, its success depends on having a strong operational framework in place. This article provides a foundational review of HIE, tracing

A Broken Pathway: Understanding Congenital Adrenal Hyperplasia in the Newborn.

Congenital adrenal hyperplasia (CAH) is an autosomal recessive disorder that leads to the partial or complete deficiency of cortisol and aldosterone production from the adrenal glands. The lack of these key hormones can precipitate acute adrenal crisis during the newborn period. This disorder can further lead to the development of virilized female genitalia from exposure to increased levels of androgens during fetal development. Nonclassical CAH is a common autosomal disorder, affecting 1/200 live births. The classical form of CAH affects 1/10,000-16,000 live births. Infants affected by classi

A clinical test of women's self-diagnosis of genitourinary infections.

Vaginitis and cystitis result in 8 million ambulatory health care visits annually. For military women deployed to austere environments, gynecological problems are more likely to occur yet unlikely to be adequately diagnosed and managed. The Institute of Medicine has recommended that self-care kits be made available to military women under these circumstances. In this clinical comparative study, women's (N = 86) self-diagnoses of vaginitis and cystitis using an investigator-designed, self-diagnostic decision-making guide and kit were compared with clinical diagnoses by advanced practice nurses

Barriers and facilitators for mental-health professionals in the management and implementation of advance directives.

BACKGROUND: The Advance Directive (AD) serves as a fundamental legal and ethical instrument for safeguarding patient autonomy, ensuring that clinical decisions align with individual values when decision-making capacity is compromised. Despite the established legal framework in Spain since 2002, its practical implementation within mental health services remains largely under-researched. The objective of our study is to describe the barriers and facilitators perceived by mental-health professionals regarding the clinical management and implementation of the Advance Directive (AD). METHODS: A de

A Survey of State Correctional Health Care Providers on Advance Care Planning: Opportunity for Collaboration With Corrections.

Prison populations are rapidly aging. Persons in prison age quicker and suffer more chronic illness and disability than their nonincarcerated peers, posing challenges to caring for prisoners who are chronically ill and dying. The goal of our study was to describe state prisons' practices and policies addressing persons in prison with advanced chronic and life limiting illness through a national web-based survey of state-level prison health care professionals. In particular, we focused on advanced care planning, use of health care directives, decision-making about goals of care, including life

A shot of good cholesterol: synthetic HDL, a new intervention for atherosclerosis.

One of the American Heart Association's Top 10 Research Advances for the Treatment of Heart Disease is the use of a synthetic form of high-density lipoprotein (HDL) to reduce coronary atherosclerosis (JAMA. 2003;290:2292-2300). While HDL has not been a target for therapy for dyslipidemias, new insight into the major protein component of HDL, apolipoprotein A-I, may lead to new therapies. Apolipoprotein A-I was recently found to be a better predictor of cardiovascular events than is low-density lipoprotein (Am Heart J. 2003;146:227-233; J Intern Med. 2004;255:188-205). Navigating Asparaginase Treatment for Patients With Acute Lymphoblastic Leukemia: US Consensus Panel Recommendations on the Role of Multidisciplinary Care.

Asparaginase is a critical treatment component for patients with acute lymphoblastic leukemia/lymphoblastic lymphoma (ALL/LBL). However, the successful delivery of asparaginase-based therapy remains challenging across care settings due to its complex administration, distinct toxicity profile, prolonged treatment duration, and the need for coordinated multidisciplinary care. These challenges are further compounded by variability in institutional infrastructure, provider experience, and access to specialized supportive services. In this article, we describe key operational and clinical barriers
