The relationship between active peptic ulcer, endoscopic duodenitis and symptomatic state after treatment with cimetidine.

Fifty patients with active peptic ulcers on endoscopy were randomly allocated for treatment with placebo or cimetidine (1.0 g daily) over a period of four weeks. All patients had free access to antacids to relieve epigastric pain. In the cimetidine group a significantly higher proportion of the ulcers had healed (82.6% of the patients) compared with the placebo group (48.0%). There was poor correlation between the healing of the ulcer and dyspeptic symptoms in the placebo group. The results suggest that the presence of endoscopic duodenitis is to a great extent responsible for the dyspepsia. C…

Cytoprotection by prostaglandins in rats. Prevention of gastric necrosis produced by alcohol, HCl, NaOH, hypertonic NaCl, and thermal injury.

Oral administration to fasted rats of either absolute ethanol, 0.6 N hydrochloric acid, 0.2 N sodium hydroxide, 25% sodium chloride, or boil … ct on gastric secretion, and (b) anti-secretory compounds (cimetidine, methscopolamine bromide) and antacids are not cytoprotective. Although the mechanism of gastric cytoprotection is unknown, prostaglandins appear to increase the resistance of gastric mucosal cells to the necrotizing effect of strong irritants. These results suggest that certain prostagl …

Clinical pharmacokinetics of aspirin.

Aspirin is very rapidly absorbed from the gastrointestinal tract when administered as a solution, and somewhat more slowly when administered … sing daily dose. The renal clearance of salicylic acid increases markedly with increasing urine pH; antacids capable of increasing urine pH can therefore cause a pronounced lowering of steady-state salicylate concentrations under clinical conditions. There are pronounced intersubject differences in salicylate elimination kinetics; dosage must be individual …

Correlation of Anxiety and Depression to the Development of Gastroesophageal Disease in the Younger Population.

Gastroesophageal reflux disease (GERD) is a condition characterized by the reflux of stomach contents into the esophagus, which leads to heartburn and regurgitation. GERD has been categorized its types according to severity. The categories that have been discussed in this study are reflux esophagitis (RE), non-erosive reflux disease (NERD), and Barrett's esophagus. Our study compared various studies and showed that the subjects with GERD had a high level of anxiety and depression. Gastroesophageal reflux disease has a significant negative impact on the quality of life (QoL) by perturbing daily

Adolescent reproductive health practices in Nigeria.

Adolescents have become a focal point of discussions of sexuality and reproductive health matters because they belong to a most active segment of the population and because of practical concerns in resolving problems such as unintended pregnancy and sexually transmitted diseases, including HIV/AIDS Employing in-depth interviews and focus group discussion techniques, this study of 2,510 respondents from four tertiary educational institutions in Nigeria examined various forms of reproductive health practices, their origins, reproductive health services available in the institutions, and the role

Predictors of duodenal ulcer healing in sucralfate treated patients.

The objective of the study was to assess the efficacy of sucralfate in promoting duodenal ulcer healing and to assess the value of some variables in predicting outcome of such therapy. Following variables were tested for predicting the outcome: age at onset, age at presentation, duration of symptoms, sex, periodicity, smoking, nocturnal pain, relief by food, relief by antacid, gastric stasis like symptoms, associated irritable bowel syndrome, site, size and number of ulcers and degree of deformity of bulb. Sixty patients with uncomplicated DU confirmed at endoscopy were treated with sucralfate

A new technique for continuous measurement and recording of gastric potential difference in the rat: evaluation of NSAID-induced gastric mucosal damage.

Disruption of the gastric mucosal barrier by the so-called "barrier breakers" such as ethanol, aspirin, and bile is associated with an increase in gastric potential difference (GPD), that is, a decrease in its negativity. Because a good correlation between the degree of histological damage and changes in GPD has been observed, this parameter has been used increasingly as an index of mucosal integrity. However, the current methodology for measuring GPD is laborious due to the preparation and checking of KCl-agarose bridges prior to each experiment, and calculations--usually handmade--are time-c

The use of single dose of sodium citrate as a prophylaxis against acid aspiration syndrome in obstetric patients undergoing caesarean section.

The effectiveness of sodium citrate as a prophylaxis against acid aspiration syndrome was studied in 3 groups of obstetric patients. Group I was the control group which consisted of 20 patients in established labour who were not likely to require caesarean section. No antacid had been given to these patients. Group II consisted of 20 patients who underwent elective caesarean section, while Group III consisted of another 20 patients who underwent emergency caesarean section. Group II and III were given 30ml of 0.3M sodium citrate as soon as they arrived in the operation theatre. The gastric con

Comparative study of Riflux Forte and Riflux in cases of reflux oesophagitis.

In an open, randomised, comparative study of 68 endoscopically proven cases of reflux oesophagitis, one antacid with relatively high acid-neutralising capacity (Riflux Forte) and another with lower acid-neutralising capacity (Riflux) were compared. Except for a relatively higher number of cases of severe oesophagitis in the Riflux Forte group (31 vs 19), the patient populations were homogeneous. A consistent trend of rapid and increased diminution in severity of symptoms was seen with both Riflux and Riflux Forte. Although differences between the two groups were not statistically significant,

The metabolism of aluminum and aluminum-related encephalopathy.

The dialysis encephalopathy syndrome is at once the most widely recognized and most severe manifestation of aluminum toxicity. Evidence linking this syndrome and aluminum intoxication is virtually incontrovertible. The syndrome is characterized by speech and motor difficulties, dementia, and seizures. Less widely recognized symptoms include subtle changes in cognition and personality and directional disorientation. Since the widespread use of water treatment, aluminum exposure in the dialysis population has been primarily via intravenous (IV) medications and oral aluminum-containing, phosphate

[Combined effect of FM-100 and several anti-ulcer agents on ulcer induced by ligation of the pylorus in rats].

Single administration of FM-100 (50, 200 mg/kg, i.p.), cimetidine (200, 800 mg/kg, i.d.), pirenzepine (5, 20 mg/kg, i.d.) and ASS (20, 80 mg/kg, p.o.) dose-dependently prevented ulcer formation in pylorus-ligated rats. FM-100 and pirenzepine dose-dependently decreased the volume of gastric juice with a slight increase in the pH, but scarcely influenced pepsin activity. The inhibitory effect of cimetidine on gastric secretion was characterized by an elevation in the pH of the gastric juice. ASS at 80 mg/kg, showing antiulcerogenic activity, had a weak antacidic effect on gastric juice without a

Ranitidine and placebo in the treatment of reflux oesophagitis. A double-blind randomized trial.

A study was carried out to assess the effectiveness of ranitidine in the short-term treatment of reflux oesophagitis. In a double-blind randomized trial of 37 outpatients with symptomatic, endoscopically proven, moderate or severe reflux oesophagitis, 18 patients received ranitidine (150 mg twice a day) and 19 patients received identical-looking placebo tablets for a period of six weeks. Clinical, laboratory, and endoscopic assessments were made initially, and at the end of six weeks. Two patients withdrew during the trial. Endoscopic evidence of improvement was found in 15 of 17 ranitidine-tr

Alterations in Gut Microbiota and Stool Properties Following Treatment with Potassium-Competitive Acid Blocker and Proton Pump Inhibitor.

BACKGROUND: Proton pump inhibitors (PPIs) and potassium-competitive acid blockers (PCABs) have been reported to alter the gut microbiota. However, the relationship between these alterations and stool properties remains unclear. This study aimed to examine changes in the gut microbiota, stool consistency, and subjective symptoms related to defecation after antacid treatment. METHODS: Patients who received PPI or PCAB for 6 weeks following endoscopic submucosal dissection for upper gastrointestinal cancers between August 2017 and June 2020 were recruited. Fecal samples were analyzed using ampli

[Gastroesophageal Reflux Disease].

Gastroesophageal reflux disease (GERD) is a condition that develops when reflux of stomach contents causes troublesome symptoms and/or complications. The prevalence of GERD is increasing worldwide and in Asia-Pacific. The latest Korean guidelines for GERD were published in 2012, and several international guidelines and consensus statements for the management of GERD have also been recently published. Here, we review these guidelines and consensus statements in order to provide a better understanding of the diagnosis and treatment of GERD.

Rare complication of milk-alkali ingestion: severe pancreatitis and acute kidney injury in a chronic hypocalcaemic patient with DiGeorge's syndrome.

Injudicious use of over-the-counter calcium supplements has resulted in increased incidences of hypercalcaemia and related complications. We present a case of acute pancreatitis in a chronic hypocalcaemic patient of DiGeorge's syndrome. The patient came into the ED with sepsis syndrome, right upper quadrant and epigastric pain and no obvious source of infection. Lab results and imaging were indicative of acute pancreatitis. There was severe renal dysfunction. The patient needed haemodialysis and had a prolonged stay in intensive care. The medical history was negative for biliary duct pathology

[Overweight and gastro-oesophageal reflux disease].

The increase in the prevalence of obesity is paralleled by an increase in gastro-oesophageal reflux disease (GERD), and several mechanisms link GERD and obesity, so weight loss is a cornerstone in the treatment of GERD. Sustained weight loss often requires surgery, and fundoplication is the first surgical choice among normal weight patients with reflux; however, reflux complications increase with increasing BMI. Therefore, patients with obesity and GERD should be treated with gastric bypass surgery. The aim of this article is to discuss the relationship between overweight and GERD and outline

[Successful treatment of hemorrhagic cystitis after radiation therapy with intravesical instillation of aluminium hydroxide gel and magnesium hydroxide: report of a case].

A 77-year-old man was admitted to our hospital due to macrohematuria and bladder tamponade. Bladder hemorrhage caused by radiation therapy had not improved after bladder lavage, intravesical drip infusion, medication of hemostatics, and transurethral coagulation. Bladder hemorrhage had completely disappeared 7 days after the intravesical instillation of 50-100 ml aluminium hydroxide gel and magnesium hydroxide for an hour per day. This method is easy and can be performed without complications. This method might be useful as first-line therapy in the case of severe bladder hemorrhage.

Helicobacter pylori and gastro-oesophageal reflux disease: association and clinical implications. To treat or not to treat with anti-H. pylori therapy?

BACKGROUND: It has been reported that patients are at risk of developing reflux oesophagitis after successful anti-Helicobacter pylori therapy, and the presence of the bacterium might be protective against the development of reflux oesophagitis. METHODS: Review of the literature. RESULTS: H. pylori is relevant to the management of oesophagitis because it increases the pH-elevating effect of proton-pump inhibitors. which increase the tendency of H. pylori gastritis to progress to atrophic gastritis, and because eradication of H. pylori increases the likelihood of oesophagitis. H. pylori incre
