In a series of 730 patients who underwent lens implant surgery, 44 (6%) had bilateral implants. Eighty-eight percent of these patients had at least one year's interval between operations and 50% more than two years. Surgery on the first eye resulted in vision of 20/40 or better in 86% of patients. Eighty-three percent of patients were 20/40 or better in the second eye following surgery. The principal cause of vision less than 20/40 was senile macular degeneration. Follow-up observation ranged from 4-96 months. Conservative criteria are advised for bilateral implant surgery, with an appropriate

OBJECTIVE: To assess the efficacy and safety of using the adjustable flanged technique for secondary implantation of four-point scleral‑fixated posterior chamber intraocular lenses with two parallel 6-0 polyglactin sutures.
METHODS: Two parallel 6-0 polyglactin sutures were passed separately through the two haptics on the horizontal line of the 4-haptic IOL. The four externalized sutures were then trimmed and cauterized to form flanges. The best corrected visual acuity, intraocular pressure, and complications in all patients were observed and recorded.
RESULTS: The flanged technique using tw

BACKGROUND/PURPOSE: Retinal toxicity associated with antimalarial drug use in inflammatory conditions is well described and may be more common than previously recognized. Antimalarial drugs bind to melanin in ocular tissues, particularly the retinal pigment epithelium, but the mechanism of toxicity and its relation to light is unclear.
METHODS: Case report.
RESULTS: A 62-year-old white woman with erosive rheumatoid arthritis developed hydroxychloroquine toxicity in her phakic eye, with her aphakic fellow eye only mildly affected.
CONCLUSION: We report the clinical evaluation of this rare ca

A new technique of correcting tube-corneal touch is described in a case of Ahmed glaucoma valve implantation. This technique repositions the intracameral tube without externalization unlike the standard procedures which aim to correct tube-corneal touch. The technique makes use of a transscleral supracapsular anchor suture to facilitate repositioning of the tube within the ciliary sulcus. Feasible only in pseudophakic and aphakic patients, either a preexisting iridectomy or intraoperative iridectomy is an essential prerequisite to perform this procedure. The final positioning of the tube withi

BACKGROUND: Congenital cataract is currently one of the leading blindness-causing eye diseases in children. Surgical treatment only opens the visual pathway for children. The postoperative recovery of visual function is also dependent on effective optical correction and visual function training. In this study, we analyzed the changes in eye-related parameters, adverse events and the annual cost of rigid gas permeable contact lens (RGPCL) and spectacles correction in infants with monocular aphakia after congenital cataract surgery.
METHODS: To analyze the postoperative visual acuity, strabismu

The management of childhood cataract begins at the initial contact with the family and typically extends far into the child's future. Decisions affecting long-term care and visual outcomes are often made in these initial preoperative encounters. Treatment will vary depending on whether the cataract is unilateral or bilateral and whether it is infantile onset or later. Thorough discussion of the treatment options is needed, especially with description of the life-long management issues for the child. Visual outcomes will vary, with the best visual acuity results being observed in older children

IMPORTANCE: Patching has been a mainstay in treating unilateral congenital cataract. However, its efficacy has not been rigorously assessed.
OBJECTIVE: To examine the association between patching and visual acuity in a cohort of children treated for unilateral congenital cataract.
DESIGN, SETTING, AND PARTICIPANTS: This study was a secondary analysis of a randomized clinical trial (Infant Aphakia Treatment Study) of infants born from August 1, 2004, through December 31, 2008, who were treated with 1 of 2 treatments for unilateral congenital cataract and followed up to 5 years of age. An 80-year-old Chinese man with high myopia and a history of right eye cataract extraction and superior peripheral iridectomy developed peripheral corneal oedema after a period of aphakia. The peripheral corneal oedema was static over years and did not progress to involve central cornea hence his visual acuity remained stable. The condition was compatible with the relatively rare Brown-McLean syndrome. Specular microscopy showed normal endothelial cell density in the oedematous peripheral as well as central cornea. While anterior segment optical coherence tomography demonstrated the cross-sect

BACKGROUND: Operations for congenital cataract in children in the past had resulted in aphakia. Improvement in surgical tools and techniques as well as in intraocular lens (IOL) implantation has led to correction of the aphakia by IOL implantation. We report the outcome of cataract surgery with and without IOL on these children in our institution between 1991-2008.
METHODS: In this retrospective cohort study, the medical records of all children who underwent surgery for congenital cataract were reviewed. The final study group included 144 children (218 eyes). Postoperative visual acuity (VA)

A 71-year-old man with penetrating keratoplasty (PKP) and aphakia in the right eye and a preoperative refraction of +20.0 -11.0 x 38 and logMAR visual acuity of 20/60 presented to our hospital. The capsular support was absent because of previous complicated intracapsular cataract extraction. The implanted IOL was a custom-made Rayner 570T with +20.5 diopters (D) sphere and +11.0 D cylinder. The IOL's special haptics allowed it to be safely fixated in the sagittal plane. The postoperative refraction was +1.0 -2.0 x 5 with logMAR acuity of 20/60. Follow-up of 2 years revealed no IOL decentration

A new, minimally invasive technique for clear cornea implantation of one-piece acrylic posterior chamber lenses for transscleral suture fixation in aphakic eyes without capsular support is described. Based on the techniques of modern small-incision cataract surgery, implantation was performed through a self-sealing, 2.75-mm temporal clear cornea incision using the injector technique. This was possible by modifying the insertion of the lens into the cartridge and introducing a new suturing technique of the lens. Suturing of the leading haptic end was performed while the lens was still securely
